Abstract

Early identification of pregnancy risk factors using the Maternal and Child Health (MCH) handbook and the Poedji Rochyati Score Card can be carried out by pregnant women or community health volunteers (cadres). Supporting cadres in identifying high-risk pregnancies facilitates timely detection and appropriate intervention. This study aimed to develop a pregnancy risk detection booklet based on the MCH handbook. A cluster randomized controlled trial with a pretest-posttest control group design was conducted in eight villages, involving 30 selected cadres recommended by local midwives based on their active participation. The intervention group received training and assistance using the developed booklet. The results demonstrated that cadres in the intervention group had significantly greater improvement in pregnancy risk detection skills compared to the control group, with a mean difference of 18.7 (95% CI: 15.2–22.2, p=0.001). In addition, cadre performance in antenatal care using the 10T approach improved by 15.7% (95% CI: 11.3–20.1, p=0.001) in the intervention group relative to the control. These findings suggest that the booklet is an effective tool for enhancing cadres’ competencies in early pregnancy risk screening and service delivery. Further development of the booklet with more engaging and culturally relevant content may enhance its utility and adoption in community health settings.

Abstract

Hypertension is a non-communicable disease with a high prevalence that can cause serious complications and even death if not treated optimally. To prevent hypertension complications, hypertension control efforts are needed, one of which is with non-pharmacological therapies such as the use of Family Medicinal Plants (TOGA) as a natural alternative in controlling blood pressure. This research aims to increase the knowledge, skills, and independence of the community in managing hypertension through independent care of TOGA, especially in the Tera Gymnastics group in Blitar Village. The implementation of activities included health education, demonstrations, and blood pressure measurements, which were attended by 44 respondents. Evaluation was carried out through pretest and posttest knowledge, as well as blood pressure measurements before and after intervention. The results of the study used the Wilcoxon Signed Rank Test, which showed an increase in knowledge with a value of p < 0.005, which means that there is an influence on the knowledge of partners. In the post-test after being given herbal drinks, a decrease in systolic and diastolic blood pressure was obtained; the average results were that the systolic blood pressure value decreased by 7.95 mmHg and the diastolic by 2.43 mmHg. TOGA Independent Care has proven to be effective as a complementary therapy in efforts to control hypertension. It is hoped that this program can continue and be developed in other communities as a sustainable promotive and preventive effort.

Abstract

Diabetes Mellitus (DM) is a growing public health problem in Indonesia, particularly among the elderly. Type 2 DM develops slowly and is closely linked to unhealthy lifestyles. Elderly people with DM require family support to prevent complications and improve their quality of life. This study aims to describe the implementation of family nursing care for DM patients at the Sananwetan Community Health Center in Blitar City. The research design was a case study involving four families with elderly people with DM. The nursing care process consisted of assessment, diagnosis, planning, implementation, and evaluation. The instruments used included the family assessment format, the Indonesian Nursing Diagnosis Standards (SDKI), the Indonesian Nursing Outcome Standards (SLKI), and the Indonesian Nursing Intervention Standards (SIKI). Implementation was carried out through an educational approach using leaflets and calendars containing information about acupressure therapy and DM management. This study identified six nursing diagnoses: ineffective family health management, readiness to improve family coping, unstable blood glucose levels, knowledge deficit, readiness to improve knowledge, and non-compliance. The interventions provided demonstrated improved family knowledge, active involvement in care, and patient adherence to diabetes treatment and management. Family nursing care with an educational approach and supporting media has proven effective in enhancing the family's role in diabetes management in the elderly. Family support is a key factor in successful disease control. It is recommended that families continue to develop health management skills, healthcare providers improve family education, and community health centers strengthen ongoing education programs.

Abstract

Gout arthritis is a non-communicable disease commonly found in the elderly, characterized by joint pain due to monosodium urate crystal deposition. Its high prevalence in Indonesia is associated with a high-purine diet, low physical activity, and limited knowledge of disease management. Complementary therapy using boiled bay leaves offers a safe and affordable method to reduce uric acid levels. This case study was conducted on four families with elderly members aged 60–75 years suffering from gouty arthritis in the Sananwetan Community Health Center, Blitar City. Nursing care was provided through assessment, nursing diagnosis, planning, intervention, implementation, and evaluation. Interventions included health education on a low-purine diet and administration of 10–15 bay leaves boiled in 200–400 ml of water, consumed once daily for six days. Data collection was performed through interviews, observations, and physical examinations, including uric acid level monitoring. The main nursing diagnoses established were ineffective family health management, chronic pain, knowledge deficit, readiness to improve coping, readiness to enhance knowledge, and impaired family processes. Results showed a reduction in uric acid levels (average 6.2 mg/dl), decreased joint pain, and improved family knowledge and involvement in patient care. This study highlights the effectiveness of integrating family-based health education and complementary therapy using bay leaves in managing gouty arthritis. Family support proved to be an essential factor in successful disease control and quality of care for elderly patients.

Abstract

Aging leads to physiological changes such as decreased vascular elasticity and impaired blood pressure regulation, increasing the risk of hypertension. Demographic factors such as age, sex, education level, and socioeconomic status also contribute. This study aimed to analyze the relationship between demographic characteristics and the prevalence of hypertension among older adults at Sananwetan Primary Health Center. A quantitative descriptive design was employed, with purposive sampling of 100 elderly individuals diagnosed with hypertension. Primary data were collected through interviews using a modified WHO STEPS questionnaire, and secondary data were obtained from health records. Data were analyzed using descriptive and inferential statistics (Chi-Square test). Most respondents were aged 60–74 years (77%), female (68%), had a high school education (50%), and belonged to the middle socioeconomic group (85%). The Chi-Square test showed significant associations between age (p = 0.019), sex (p = 0.007), education level (p = 0.034), and socioeconomic status (p = 0.016) with the prevalence of hypertension. In conclusion, demographic factors influence the risk of hypertension in older adults. Health education-based interventions, improved access to services, and promotion of healthy lifestyles are essential strategies to reduce hypertension prevalence in high-risk elderly populations.
