Abstract

Effect of "Taburia" Intervention Program on Hemoglobin Concentration Among Children Under-Five Years of Poor Families In North Jakarta.Background: Anemia is one of the main nutritional problems in Indonesia. Anemia in children under fives years will have an impact on growth and mental development. In an effort to improve the nutritional status of infants, especially in poor families, the Japan Funds for Poverty Reduction (JFPR) in collaboration with the Center for Nutrition and Food, Ministry of Health, Bogor, making multi-vitamin ingredients and micro nutrients called as TABURIA.Objectives: To study the influence of hemoglobin levels on child under fives years of poor families.Methods: Design of the study was Evaluation Before and After Treatment (Pre-Post Evaluation). This research was conducted in 9 urban villages in 3 Public health centers in North Jakarta City. The numbers of samples were 540 children aged 6-59 months from poor families. Tabuaria was distributed by a cadre posyandu to all children under fives years (5000 children) in the research area. The data analyzed included: hemoglobin levels, economic social family, compliance and child morbidity. Data were analyzed by descriptive to determine changes in status of anemia and haemoglobin levels before and after intervention.Results: Most of children under fives years (87.8%) to receive and consume Taburia. The average Hb levels increased from 10.5 mg/dl in baseline data to 12.0 mg/dl at endline evaluation. The proportion of anemia (Hb <11 mg/dl) decreased significantly from 62.3% (baseline) to 24.7%(endline-evaluation).Conclusion: Taburia be accepted by the majority of children under fives and can increase hemoglobin levels of children under fives.Recommendation: Taburia can increase the hemoglobin levels of children under fives years, but if would be implemented as national program, it should be study in others regions (pilot areas) to explore the appropriateness Taburia distribution model in different conditions.Keywords: taburia, hemoglobin levels, poor families

Abstract

The Evaluation of Management MP-ASI Local and The Influence Towards Body Heavy Enchanced and Children Under Five Nutrient Status Age 6-24 Months In Banyumas Regency.Background: Nutrient problem besides problem sindroma tight poverty with also concern behaviour change problem less support in alive pattern wells. In baby and child, nutrient deficit will evoke growth disturbance and development when not be overcome according to early continue up to adult. MP-ASI that given should made from cheap food stuff and easy goat local region. The wacthfullness aims to detect about management MP-ASI local and the influence toward age child body heavy enchanced 6 - 24 months.Objectives: This study aims to evaluate the local MP-ASI management and effect on growth of children aged 6 to 24 months in the Health Center of South Purwokerto, Banyumas.Methods: Study design is cohort retrospective. Total samples are 35 children under two years old (6-24 months) at Community Health Centre South Purwokerto. Samples has get local supplementary feeding (MP-ASI) in 2007. Data collected was socio economic household, weight of children and supplementary feeding management by interview with mother and report from Community Health Centre. Data analysis is descriptive. Nutritional status is base on Z score WHO-NCHS.Results: The local supplementary feeding (MP-ASI local) that given disagrees with guides from Health Department. A large of samples (42.9%) has given raw food stuff, 11.4% has give money and others 45.7%. There influence used MP-ASI local with body heavy enchanced especially 1 in 2 and 3 and the gift influence MP ASI local towards nutrient status enchanced especially in to 3, although low the connection with value p 0.032 and association value 0.289.Conclusions: Alternative nutrient problem in family necessary with approach multisector among others well being sector, industrial, agricultures and others. Necessary family enableness movement existence so that can increase economy social level and can run family function in especially well being area. Gift MP-ASI that done according to adekuat the nutrient value in target as according to age and instruction that determined can increase growth and children under five development in an optimal fashion.Keywords: colleague food, heavy body, nutrient status

Abstract

Test The Guidance For The Impelentation of "Dietary Guidelines" For Different Area Conditions.Background: Dietary guidelines for nutrition education tool developed by the health department until now still difficult in the field. Dietary guidelines should be arranged by local officials. Guidelines that needs to be made how to craft a balanced nutrition messages in accordance with local conditions.Objectives: To develop a guidance on how to implement the PUGS which is suitable for the local conditions.Methods: Exploratory research methods as applied research for local officials. Implematation manual has been composed PUGS who first disseminated to local officials at the district level. And than local officials to practice through the following stages; how do identify nutritional problems and then how to make balanced nutrition messages in accordance with local conditions. This guidance was tested in the district of Tasikmalaya, West Java and Magelang of Central Java.Results: Showed that there was a significant difference after and before test the guidance for the implementation of dietary guidelines in local officers were able to identify nutrition problems and to develop messages on PUGS.Keywords: PUGS, implementation of dietary guidelines and local condition.

Abstract

Acceptability of Multiple-Micronutrient Powders By Under-Fives of Poor Families In North Jakarta.Background: The final of three steps in developing the Indonesia Multiple-Micronutrient Powders (MMP) called TABURIA was to evaluate its effectiveness in improving the nutritional status of under-fives under programmatic setting. Together with execution of the final step, it was also necessary to conduct the subject's perception, satisfaction, and expectation toward TABURIA.Objectives: To find-out the acceptability of under-fives' mothers toward TABURIA.Methods: We assessed the acceptability of TABURIA for under-fives of poor families in North Jakarta. This under-fives beneficiaries of the MMP project participating in "the Effectiveness of TABURIA on Growth and Hemoglobin Concentration Study" were surveyed about the acceptability of TABURIA (n = 464). Acceptability of the MMP was evaluated based on mothers' perceptions and experiences with organoleptic qualities, ease of use, perceived satisfaction, and expectation. The median Likert scale ranking of organoleptic and use qualities for TABURIA was "I liked it" (3 on a scale of 1-5). However, responses to openended survey questions and semi-structured interviews showed decided preferences.Results: TABURIA was strongly preferred by under-fives. In interviews, most of mothers expressed like of the texture, color, odor, and taste of TABURIA; they found it practical to get, store and prepare, and reported the satisfaction and expectation with it.Conclusions: Most of under-fives' mothers were very like TABURIA product. They stated TABURIA was simple to use, more preferred it than the other consumed supplement products, willing to pay if it will be sold to the free market; they preferred to pay it maximum Rp 500 per sachet. Abstract

FOOD CONSUMPTION AND ANAEMIA STATUS ON PREGNANT WOMEN SAMPLE STUDY SUMMIT  (THE SUPPLEMENTATION WITH MULTIPLE MICRONUTRIENTS INTERVENTION TRIAL) IN LOMBOK.Background: Information on consumption pattern is key to the analysis of trial results suchas the SUMMIT (supplementation multiple micronutrients intervention trial) as consumption modifies the outcome of intervention. It is therefore important to prove that nutrient consumption is comparable between the two groups (iron folate vs. multiple micronutrient) in the SUMMIT trial.Objectives: to analyse food consumption and anemia status of pregnant women at enrolment to both groups in the SUMMIT trial.Methods: This paper analysed consumption data of 16,111 pregnant women in the SUMMIT cohort that was collected prior to supplementation. As many as 7722 of the samples belong to the iron folate (IFO) group and 8389 belong to women who received multiple micronutrients. Data was collected by trained field data collector using the 1x24 hr recall method. Around the same time of the recall, data of haemoglobin level were taken with Hemocue. Nutrient consumption status was assessed using the 2004 Indonesian recommended daily allowance (RDA). Calculation of the nutrient composition was done with Nutrisoft software. The difference and relation between consumption and anemia status between the two groups were determined using Chi-square and T-test.Results: The average consumption of macronutrient and micronutrient in the samples were only around 50% of RDA. Around 80% of the samples were energy deficient and around 75% were protein deficient (<70% RDA). Hemoglobin level of those enrolled in the first trimester was 11.7 (g/dl), which was higher compared to those enrolled in the second (10.9 g/dl) and third trimester (10.5g/dl). The proportion of anemic women enrolled in the first trimester was lower compared to those enrolled in the second and third trimester.Conclusions: Pregnant women in Lombok consumed macro and micronutrient far less than recommended. Both the IFO and MMN group of the cohort were comparable in terms of nutrient consumption.Recommendations: Because nutrient consumption of pregnant women was far lower than the RDA, both macro and micronutrient consumption of pregnant women need to be addressed in order to improve maternal nutritional status. The programmatic implication is that maternal micronutrient supplementation needs to be combined with macronutrient intervention in order to obtain its optimal outcome.Keywords: pregnant women, energy and protein consumption, anemia.

Abstract

Caring Experiences of Mothers Who Have Malnourished Infants In Rejang Lebong District, Bengkulu Province: A Phenomenological Study).Background: Rejang Lebong District has the highest prevalence of malnourished under-fives among all districts of Bengkulu Province. Theoretically, to overcome malnutrition a good strategy of nutrition education to mothers and nurturing the children were really required. Therefore, the problem came out how mothers nurturing their malnourished children in Rejang Lebong District.Objectives: This research aims was to find out the experience of nursing malnourished children by mothers in Rejang Lebong Municipality.Methods: A qualitative approach was used to conduct this research by using Focus Group Discussion particularly phenomenology approach. In order to collect information 20 respondents were interviewed consisted of: 4 midwives, 3 community leaders, 6 village leaders, and 7 cadres.Results: The result showed that colostrums was not given to babies, because of dirty milk, children were not given breast milk cause of lack of breast milk mother. Moreover, children frequently was given snack which was not nutritious food. Other aspects were that people used drinking water from the river which was not good quality, moreover them others did not use soap to wash the dishes.Conclusions: Malnourished children frequently got sick accompanied by fever, diarrhea, cough, influenza, and to overcome these diseases the mothers brought their children to the health center and midwife, however, most of them cured by buying free medicine. The knowledge of mothers about the usage of colostrums, breast care, nutritious food, avoiding from infection diseases and nurturing their children still were not good enough.Keywords: malnourished infants, mothers' caring experiences, phenomenological study.

Abstract

The Effect of Combination of Iron and Selenium Supplementation on the Status of Thyroid Hormone and Haemoglobin Levels of Child Bearing Age Women In Endemic Goitre Area.Background: Iodine Deficiency Disorder (IDD) and Iron Deficiency Anemia (IDA) still considered as a public health problem in Indonesia. It is possible that some people in endemic goiter area not only suffering from IDD but also IDA, particulary among Childbearing Age Women (CBAW). Based on the national household survey (SKRT, 2001), it was reported that the prevalence of IDA among CBAW in Central Java, 22.4%, while in Cianjur District was 36.6%. Iron deficiency not only caused the decreament of hemoglobin value, but also decreased the Tyroxin (T4) and triiodotironin (T3) hormon syntesis. The trace element which has important role for iodine metabolism is selenium (Se). So far IDA and IDD control programme in Indonesia have been conducted separately, by means not integrated in strategy and not using combination of iron and selenium supplementation.Objectives: The main objectives of this study are to find out the possibilities of integrated solution of IDD and IDA. To determine the effect of combination iron and selenium supplementation to iodine status and Hb level of CBAW in the endemic goiter area.Methods: Study design was randomized control trial. The study was conducted in Cangkringan Subdistrict of Sleman District of D.I. Yogyakarta. A total of 96 CBAW were randomly assigned into 3 groups. Each group received different treatment, but before starting, all subjects were received pirantel for deworming. Group A (n=31) received 400 mg of iodine (2 capsules) + ferrosulfate (60 mg elemental iron + 0.25 mg of folic acid) + 420 ug of selenium (yeast selenium); group B (n=31) received 400 mg of iodine + ferrosulfate and group C (n = 34) received 400 mg of iodine only (control group). Two capsules of iodine were given once in the beginning of study to all subjects, while ferrosulfate and selenium were given twice a week for 16 weeks. Data of hormon thyroxin (serum T4) and Hb was collected before and after intervention. Data of socialeconomi and selenium were collected at the first of study.Results: It was revealed that before treatment there were no significant difference of the average of Hb levels and the median value of serum T4 amongs 3 groups of study (p> 0.05). After 16 weeks intervention there were thyroid hormone status improvement (serum T4) to all groups as follows: group A (100%), group B (80%) and group C (66.7%) respectively.Conclusions: After 16 weeks intervention of iron and selenium preparate to CBAW who had received iodine supplementation, was able to improve status of thyroid hormone (serum T4) higher than to those who received iodine only (33.3%). The increment of Hb levels of CBAW who received iron and selenium supplementation (group A) was lower than those who received iron only (group B).Keywords: Childbearing Age Women, IDD, IDA, anemia and selenium.

Abstract

Biochemical Profile of Male Workers According To Hard or Light Works.Background: Various occupations could be categorized as light, moderate and heavy activities as well. Heavy workers by mean those who mainly work based on muscle strength usually received low salary, therefore they have low purchasing power for providing better nutrition, including for adequate energy intake. In contrary for those who work mainly using brain capacity rather than muscle strength usually received higher salary and consumed food with high content of fat. The difference of income could result difference of nutrition intake, even different impact to health/nutrition status and sensitivity to certain diseases as well. The profile of nutritional biochemistry could indicate the health status of people.Objectives: To study the relationship between biochemical profile of worker and different type of work.Methods: The subjects were 43 worker of sandal handicraft considered as light level workers and 44 workers of sand and stone as heavy workers whose met criteria 30-55 years old, physically healthy and agreed to involve in this study. Body composition was measured using anthropometry and blood analysis. Anthropometric measurement was applied including body weight, height, MUAC and skin fold thickness. Blood analysis was hemoglobin, albumin, protein and triglyceride. Nutrient intake was collected by combination weighing and 24 hours recall method for 3 consecutive days. Other data collection includes physical examination by a medical doctor and interview was done to identify their characteristic and socio demografi.Results: The proportion of workers who had serum albumin below normal was higher in heavy worker compared to light worker (56.8% vs. 23.3%). This finding indicated that more than half of hard workers had protein depletion as result of break down protein stores in thebody. The average of serum albumin of light workers was significantly higher compared to heavy workers. No significantly different based on the hemoglobin, protein and triglyceride concentration between the heavy and the light workers. The average energy and protein intake shows the heavy worker had higher intake on both nutrients compared to the lightworkers. The average energy intake of light worker was 1923 ± 295 Kcal and protein 41.6 ± 8.99 g, while the energy intake of heavy worker was 2232 ± 500 Kcal and protein 46.7 ± 14.7g.Conclusions: There was no significantly different found regarding biochemical profile oftwo workers, except serum albumin concentration. The lower profile of serum albumin concentration indicated that the heavy workers sacrificed of prolonged protein depletion of the body.Keywords: hemoglobin, protein, albumin, triglyceride, male worker.

Abstract

A Simple Field Kit-Test For Semi-Quantitative Iodine Content In The Iodated Salt of Households: An Appropriate-Test In The Field.Background: A programme of iodized salt for human consumption has been in operation in Indonesia for over 3 decades. Effectiveness of the programmed is to control and regular monitoring of edible salt for iodine. Therefore, it is required a simple field kit for semi quantitative estimation of iodine content in the iodated salt in the households.Methods: Thetest was performed by forty-five of house wives and forty-five of "kader". The samples of salt respectively were provided from their houses. The test was carried out by dropping of 2-3 drops of test-kit solution to surface of the salt. Observe the color appears on the surface of salt, and then compare to the color of photo of standard. The yellow color (code 1) was equalto 0 part per million of iodine (0 ppm), the color of yellow greenish (code 2) was equal to the iodine content of upper zero up to 15 ppm (>0 ppm - <15 ppm), the color of green yellowish (code 3) was equal to the iodine content between upper 15 to the below of 30 ppm (>15 ppm- < 30 ppm), and the color of green bluish (code 4) was equal to 30 ppm and above (> 30ppm). As comparison, the samples of salt were also analyzed quantitatively for the iodine content in laboratory using the standard method.Results: The result of iodine analysis in sample of salt done by using the standard method in the laboratory showed that the iodine content was varied. The estimate precision of iodine content in the salt by the house wives were 63,6% for the iodine content > 0 ppm-< 15 ppm; 87.5% for the iodine content > 15 ppm- < 30 ppm; 61.1% for the iodine content >30 ppm. While"kader" were 37,5% for the iodine content > 0 ppm - <15 ppm; 86.7% for the iodine content > 15 ppm - < 30 ppm; 72.7% for the iodine content > 30 ppm. Its Predictive Positive Value (PPV) in the iodine content 15 ppm > -< 30 ppm was 88% for the house wives and 87% for "kader". Its sensitivity was above 45% and its true prevalence (Se + Sp) was above 100% for all iodine concentration, except for zero ppm.Conclusions: The highest precisian of the simple field kit in estimation of iodine content in the salt was in the content > 15 ppm -< 30 ppm, that were 87.5% for house wives and 86.7% for "kader". It's Predictive Positive Value (PPV) in that iodine concentration was 88% for the house wives and 87% for the caders. Its sensitivity was above 45% and its true prevalence (Se + Sp) was above 100% for all iodine concentration.Recomendatlons: Further test for this field kit was suggested to be performed particularly its stability during storage.Keyswords: Iodated salt, a simple field kit, semi-quantities.
