Abstract

Drug interactions are an important medication safety concern among elderly patients with ischaemic heart disease (IHD), particularly because age-related physiological changes and comorbidities may increase vulnerability to adverse drug effects. This study aimed to identify potential drug interactions and describe the sociodemographic and clinical characteristics of elderly patients with IHD. A quantitative, descriptive study with a retrospective cross-sectional design was conducted using medical records of elderly patients treated between January and December 2025. The study included 100 patients aged 60 years or older who had an IHD diagnosis and documented concurrent use of more than one medication. Data were collected through medical record observations and analyzed using descriptive statistics, including frequency distributions and percentages. Potential drug interactions were screened using the Medscape Drug Interaction Checker and classified according to their mechanisms and severity. The results showed that 58% of patients were male, while 83% were aged 60–74 years. Bisoprolol was the most frequently prescribed medication, accounting for 92% of patients. Furthermore, 96% of patients had comorbidities, and 62% received fewer than five medications. Potential drug interactions were identified in 90% of patients, with a total of 185 interaction events. Pharmacodynamic interactions predominated, accounting for 94.6% of all identified interactions, while pharmacokinetic interactions represented 5.4%. Regarding severity, moderate interactions were the most common (82.7%), followed by major interactions (16.2%) and minor interactions (1.1%). The aspirin–bisoprolol combination was the most frequently identified interacting drug pair, accounting for 31.4% of interaction events. These findings demonstrate the high prevalence of potential drug interactions among elderly patients with IHD, even when most patients receive fewer than five medications. Routine medication review, systematic interaction screening, and continuous clinical monitoring are therefore essential. Pharmacists should collaborate with physicians and other healthcare professionals to assess interaction significance, optimize pharmacotherapy, and improve medication safety.

Abstract

Cancer remains a leading cause of mortality worldwide, and conventional treatments continue to face challenges related to drug resistance, adverse effects, and limited therapeutic efficacy. Drug repurposing has emerged as a promising strategy for identifying alternative anticancer agents using existing medications. This review aimed to evaluate the potential anticancer effects and mechanisms of action of albendazole and ivermectin, two anthelmintic drugs traditionally used to treat parasitic infections. A narrative literature review was conducted using PubMed, Google Scholar, and Elsevier databases to identify relevant studies published between 2020 and 2025. Studies investigating the anticancer effects of anthelmintic agents in clinical or preclinical settings were considered, and 15 studies were included in the synthesis. The extracted data covered cancer types, experimental models, treatment concentrations, exposure durations, half-maximal inhibitory concentration (IC₅₀), anticancer outcomes, and proposed mechanisms. The findings indicated that albendazole demonstrated antiproliferative activity against colorectal, gastric, breast, prostate, melanoma, and cervical cancer models. Its proposed mechanisms included G2/M cell cycle arrest, apoptosis induction, increased reactive oxygen species, and inhibition of angiogenesis and metastasis. Ivermectin exhibited anticancer effects across several cancer models, including colorectal, osteosarcoma, urothelial, breast, esophageal, and prostate cancers. Its mechanisms involved oxidative stress, mitochondrial dysfunction, DNA damage, apoptosis, and modulation of the tumor microenvironment through increased T-cell infiltration and reduced immunosuppressive activity. Both agents also demonstrated potential synergistic effects when combined with selected chemotherapeutic drugs or immune checkpoint inhibitors. However, IC₅₀ values varied according to cancer cell type, drug concentration, and treatment duration. In conclusion, albendazole and ivermectin represent promising candidates for further investigation through drug repurposing. Nevertheless, the available evidence remains predominantly preclinical, and well-designed clinical trials are necessary to establish their safety, optimal dosage, pharmacokinetics, drug interactions, and therapeutic efficacy in cancer patients.

Abstract

Vitamin D plays an important role in immune regulation and may influence the clinical course of autoimmune diseases. However, evidence regarding vitamin D status and its relationship with demographic characteristics, clinical factors, and disease recurrence remains limited in Indonesian clinical settings. This study aimed to evaluate vitamin D status and analyze its associations with demographic and clinical characteristics and patient outcomes among autoimmune patients at Fatmawati General Hospital, Jakarta. An analytical observational study with a retrospective cross-sectional design was conducted using medical records from 2022 to 2025. A total of 135 eligible patients were included through total sampling. Data on sex, age, autoimmune disease type, treatment duration, serum vitamin D status, and clinical outcomes were analyzed using descriptive statistics and the Chi-square test or Fisher’s Exact Test, as appropriate, with statistical significance set at p < 0.05. Most patients were female (87.4%), aged over 55 years (28.1%), and diagnosed with rheumatoid arthritis (39.3%), followed by systemic lupus erythematosus (37.8%). Normal vitamin D status was identified in 31.9% of patients, while 31.1% had vitamin D insufficiency, 18.5% had deficiency, and 18.5% had excess levels. Age (p = 0.012) and type of autoimmune disease (p < 0.001) were significantly associated with vitamin D status, whereas sex (p = 0.105) and treatment duration (p = 0.881) were not. Treatment duration was significantly associated with clinical outcomes (p < 0.001), while autoimmune disease type showed no significant association with outcomes (p = 0.227). These findings highlight the importance of monitoring vitamin D status, particularly among older patients and individuals with specific autoimmune conditions. However, the relationship between vitamin D status and autoimmune disease recurrence could not be established because it was not directly analyzed. Further prospective studies are needed to investigate this relationship and identify additional factors influencing vitamin D status and clinical outcomes.

Abstract

Coronary heart disease (CHD) is a major cause of morbidity and mortality worldwide, and its pharmacological management often involves multiple medications that may increase the risk of potential drug–drug interactions (pDDIs). This study aimed to describe the profile of pDDIs and evaluate their associations with age, gender, number of medications, and comorbidities among outpatients with CHD at Jakarta Cempaka Putih Islamic Hospital. An analytical observational study with a retrospective cross-sectional design was conducted using medical records of 200 outpatients treated between January and March 2026. Total sampling was applied to eligible patients with complete medical records and concurrent use of at least two medications. Potential drug interactions were identified using Drugs.com and Medscape, while descriptive and bivariate analyses were performed using SPSS. Of the 200 patients, 190 (95.0%) had at least one potential drug interaction, with 848 drug-pair occurrences identified. Pharmacodynamic interactions predominated, accounting for 713 occurrences (84.1%), while pharmacokinetic interactions accounted for 135 (15.9%). Most interactions were classified as moderate in severity (782; 92.2%), followed by major (38; 4.5%) and minor interactions (28; 3.3%). Bisoprolol and candesartan constituted the most frequently identified interacting drug pair, with 68 occurrences (8.02%). Bivariate analysis revealed no statistically significant associations between pDDIs and age (p = 0.499), gender (p = 1.000), or number of medications (p = 0.603). However, cardiovascular disorders (p < 0.001) and other comorbidities (p = 0.017) were significantly associated with pDDIs, whereas hypertension, diabetes mellitus, dyslipidemia, and renal disorders were not. These findings demonstrate the high prevalence of potential drug interactions among CHD outpatients and emphasize the importance of routine medication review, systematic interaction screening, and appropriate clinical monitoring. Collaborative pharmaceutical care is essential to support rational prescribing and improve medication safety.
