Abstract

Artritis tuberkulosis (TB) adalah infeksi sendi oleh kuman Mycobacterium tuberculosis, sering mengenai satu sendi terutama pinggul atau lutut; merupakan penyakit tuberkulosis muskuloskeletal kedua tersering setelah spondilitis tuberkulosis. Diagnosis sering tertunda karena sering tidak disertai gejala konstitusional, tidak ada keterlibatan paru, onset tidak disadari, dan temuan radiografi awal tidak khas. Obat anti-tuberkulosis masih menjadi terapi pilihan utama. Tuberculous arthritis is an infection of joint caused by Mycobacterium tuberculosis. It often affects one joint, especially large weight-bearing joint (hip or knee). It is the second most common musculoskeletal tuberculosis disease after tuberculous spondylitis. Diagnosis is often delayed due to lack of constitutional symptoms or pulmonary involvement, lack of awareness, insidious onset, lack of characteristic early radiographic findings. The main treatment is appropriate antituberculosis drug therapy.

Abstract

Coronavirus Disease 2019 (COVID-19) merupakan penyakit infeksi baru dengan tingkat penyebaran sangat cepat dan saat ini menjadi pandemi. Sampai Juni 2020, tercatat sekitar 8 juta kasus positif COVID-19 di dunia. Penyakit ini disebabkan oleh spesies coronavirus, SARS-CoV-2. COVID-19 ditularkan dari manusia ke manusia dan dapat menginfeksi seluruh kalangan usia. Sebagian besar kasus COVID-19 anak asimptomatik atau bergejala ringan, sehingga status infeksi COVID-19 anggota keluarga penting untuk deteksi infeksi COVID-19 pada anak. Konfirmasi kasus pada anak sama halnya pada kasus dewasa dengan pemeriksaan RT-PCR. Prinsip tatalaksana adalah isolasi, terapi suportif, dan simptomatik sesuai kondisi pasien. Coronavirus Disease 2019 (COVID-19) is a new communicable disease that has spread rapidly around the world and causing a global pandemic. As of June 2020, approximately 8 million confirmed cases of COVID-19 have been reported worldwide. This disease is caused by coronavirus species, the novel severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). COVID-19 spreads through human-to-human transmission and affected every age group. Most cases of COVID-19 in children were asymptomatic or mild; family clustering is a strong epidemiological link in pediatric COVID-19. Preferred diagnostic test in children is similar to adult, with RT-PCR test. Management is based on clinical conditions, which include self-isolation, supportive therapy, and symptomatic therapy.

Abstract

Sindrom Treacher Collins adalah penyakit genetik langka yang menyebabkan berbagai malformasi kongenital terutama di kraniofasial. Diagnosis dapat ditegakkan sejak periode prenatal atau postnatal, analisis genetik untuk menentukan adanya mutasi gen merupakan diagnosis definitif. Tatalaksana multidisiplin dengan perencanaan tindakan operatif ataupun non-operatif sejak lahir hingga usia dewasa. Treacher Collins syndrome is a rare genetic disease which causes various congenital malformations mainly in the craniofacial region. Diagnosis can be made in prenatal or postnatal period, genetic analysis to determine gene mutation remains as definitive diagnosis. Management requires a multidisciplinary approach, with of either operative or non-operative planning since birth to adult.

Abstract

Hand, foot, and mouth disease (HFMD) adalah infeksi enterovirus yang sering terjadi pada anak-anak di bawah 10 tahun. HFMD ditandai dengan lesi pada mulut, tangan, dan kaki. HFMD pernah menjadi Kejadian Luar Biasa dalam 1 dekade terakhir. HFMD sangat infeksius, biasanya ringan dan self limiting, akan tetapi dapat menimbulkan komplikasi berat. Hand, foot, and mouth disease (HFMD) is an enterovirus infection commonly found in children under 10 year-old. HFMD characterized by lesion in the mouth, hand, and foot. HFMD outbreaks occur in the last decade. HFMD is very infectious but mild and self-limiting; however, there were several severe complications of HFMD.

Abstract

Corticosteroids have been widely used in various medical situations and often used for their anti-inflammatory and immunosuppressive effects. Corticosteroids can be given by many routes such as topical, oral, intramuscular, or intravenous. However, prolonged use of steroids can resultin hypothalamic-pituitary-adrenal axis suppression and other side effects. Dosage and using strategy are needed to get the best benefits while minimizing the risk of side effects

Abstract

Pada tahun 2019, hipertensi diderita oleh 1,28 milyar jiwa di seluruh dunia dan diperkirakan terus meningkat setiap tahun. Saat ini, hipertensi masih menjadi salah satu penyebab utama morbiditas dan mortalitas global. Terdapat perubahan paradigma diagnosis hipertensi menjadi lebih direkomendasikan dengan pemeriksaan tekanan darah di luar klinik (out-of-office blood pressure measurement) berupa pemeriksaan tekanan darah ambulatoar (PTDA) jika fasilitas memadai atau pemeriksaan tekanan darah di rumah (PTDR) pada fasilitas lebih terbatas. Manajemen pasien hipertensi berfokus pada kontrol tekanan darah untuk meningkatkan kualitas hidup serta menurunkan angka kecacatan dan kematian dengan terapi farmakologis dan non-farmakologis. Terapi non-farmakologis telah terbukti membantu mengontrol tekanan darah, menurunkan morbiditas dan mortalitas, baik individu hipertensi maupun individu normal.   In 2019, hypertension was suffered by 1.28 billion people worldwide and is expected to increase every year. Hypertension is still one of the main causes of global morbidity and mortality. There is a paradigm shift that diagnosis of hypertension is more recommended by out-of-office blood pressure measurement. Blood pressure measurements outside clinic can be done with ambulatory blood pressure or home blood pressure in limited facilities. Management of hypertensive patients focuses on blood pressure control with pharmacological and non-pharmacological method to improve quality of life, as well as to reduce morbidity and mortality. Non-pharmacological approach has been proven to optimize blood pressure control, to reduce morbidity and mortality in both hypertensive and normal individuals.

Abstract

Beberapa studi efikasi suplementasi vitamin D pada diabetes melitus gestasional hasilnya masih bertentangan. Laporan kasus berbasis bukti ini bertujuan untuk konfirmasi efikasi suplementasi vitamin D terhadap kontrol glikemik pada wanita hamil dengan diabetes melitus gestasional dan hipovitaminosis D. Pencarian artikel komprehensif melalui PubMed dan Scopus. Studi yang dipilih adalah studi dengan desain randomized controlled trial atau systematic review pada wanita hamil dengan diabetes melitus gestasional dan hipovitaminosis D membandingkan terapi standar obat anti-diabetes (OAD) dengan obat terapi standar dan suplementasi vitamin D. Telaah kritis sesuai perangkat dari Center for Evidence Based Medicine (CEBM). Diperoleh 3 studi systematic review yang memenuhi kriteria eligibilitas. Dua studi menunjukkan perbedaan kadar glukosa darah puasa dan HbA1C yang bermakna secara statistik dan klinis pada kelompok yang mendapat suplementasi vitamin D dibandingkan kelompok kontrol. Satu studi hasilnya tidak bermakna. Results of recent studies on the use of vitamin D supplementation in gestational diabetes mellitus were conflicting. This evidence-based case report aimed to confirm the efficacy of vitamin D supplementation on glycemic control in pregnant women with gestational diabetes mellitus and hypovitaminosis D. A comprehensive search for articles in PubMed and Scopus included randomized controlled trial or systematic reviews in gestational diabetes mellitus and hypovitaminosis D comparing standard anti-diabetic drug therapy (OAD) with standard therapeutic drugs and vitamin D supplementation. Critical appraisals were done using the tools from Center for Evidence Based Medicine (CEBM). Three systematic reviews met the eligibility criteria. Two studies showed statistically and clinically significant differences in fasting blood glucose and HbA1C level in vitamin D supplementation group compared to the control group, while the result of the third study was not significant.

Abstract

Idiopathic fascicular left ventricular tachycardia (IFLVT) is the most common idiopathic ventricular tachycardia (IVT) of the left ventricle. ILFVT is characterized by right bundle branch block and left axis deviation. We report a case of IFLVT in a 34-year-old woman presenting with palpitation. The ECG revealed a wide QRS complex tachycardia with a right bundle branch block (RBBB) and right axis deviation. Vagal maneuvers were ineffective. Conversion to sinus rhythm was obtained 10 hours later, with i.v amiodarone. The patient was discharged after 3 days on oral verapamil. Idiopathic Fascicular Left Ventricular Tachycardia (IFLVT) adalah bentuk paling sering Idiopathic Ventricular Tachycardia (IVT) ventrikel kiri. ILFVT ditandai dengan blok cabang berkas kanan dan penyimpangan sumbu ke kiri. Kami melaporkan kasus IFLVT pada wanita 34 tahun yang mengalami palpitasi. EKG menunjukkan takikardi kompleks QRS lebar dengan morfologi blok cabang berkas kanan dan penyimpangan sumbu ke kanan. Manuver vagal tidak efektif. Konversi ke irama sinus diperoleh 10 jam kemudian dengan amiodaron i.v.. Pasien dipulangkan setelah 3 hari dengan verapamil oral.

Abstract

Anjuran skrining mammografi untuk deteksi dini kanker payudara menghasilkan bertambah banyaknya penemuan lesi payudara yang masih terlokalisasi. Selanjutnya, lesi baru tersebut membutuhkan tindakan biopsi untuk mendapatkan diagnosis histopatologi. Biopsi lesi payudara perkutan dengan panduan pencitraan merupakan metode diagnostik terpilih dibandingkan teknik konvensional biopsi terbuka. Beberapa teknik biopsi perkutan pada payudara, yaitu biopsi aspirasi jarum halus/fine-needle aspiration biopsy (FNAB), biopsi core, dan vacuum-assisted breast biopsy (VABB). Mammography screening results in more findings of localized breast lesion. These lesions need biopsy to acquire histopathological diagnosis. Percutaneous biopsy with imaging guidance is the preferred method as compared with open conventional biopsy. Techniques for breast percutaneous biopsy includes fine-needle aspiration biopsy (FNAB), core biopsy, and vacuum-assisted breast biopsy (VABB).

Abstract

GLP-1 receptor agonist secara struktur mirip hormon alami GLP-1 yang memiliki efek incretin, namun degradasinya dapat dicegah oleh DPP-4. Saat ini terdapat 8 jenis yang terdiri dari exenatide, exenatide ER, liraglutide, albiglutide, dulaglutide, lixisenatide, semaglutide injeksi, dan semaglitode oral. GLP-1 receptor agonist merupakan pilihan pengobatan DM tipe 2 baik sebagai tambahan maupun monoterapi. The GLP-1 receptor agonist is structurally similar to the natural hormone GLP-1 which has an incretin effect but prevented from breakdown by DPP-4. There are currently 8 types of GLP-1: exenatide, exenatide ER, liraglutide, albiglutide, dulaglutide, lixisenatide, injection semaglutide, and oral semaglutide. GLP-1 receptor agonist is an option for the treatment of type 2 DM both as adjunctive and as monotherapy.
