Abstract

Background. Work stress is the main indication that results in physical, mental and behavioral disruption of everyone, this is due to the inability of individuals to cope with stressors. Ship crew members are a vulnerable job and are at risk of work stress. Age is one of the triggering factors for work stress which as you get older, it will increase the risk of work stress. Purpose. The purpose of this study is to measure the relationship between age and work stress in the crew of the ship at the Tumumpa Beach Fisheries Port, Manado City. Method. This is an observational research with a cross sectional approach. This research was carried out at the Tumumpa Beach Fisheries Port in July-November 2023. The number of samples used in this study was 156 respondents. The variables of this study are divided into 2, namely age and work stress. Primary data collection was obtained directly from the distribution of questionnaires using the self-administered questionnaire method. Data analysis was carried out univariate and bivariate. Results. The results showed that the age of the respondents ranged from 17-64 years with an average age of 36.22 years. The results of this study show that the work stress score is in the interval of 90-119. The average work stress score was 107.23. In addition, the most respondents were distributed in the Medium category with 114 respondents (73.1%). The results of the analysis based on the Pearson Correlation test obtained a significance value of 0.311. This value shows that there is no significant relationship between age and work stress. Conclusion. It can be concluded that age is not related to work stress in crew members at Tumumpa Beach Fisheries Port, Manado City. Further analysis can be carried out in the next research to measure which variables are the factors causing work stress in crew members so that it can be the basis for policies for work stress control in crew members.

Abstract

Background: The integration of Artificial Intelligence (AI) into clinical practice guidelines has the potential to revolutionize healthcare by enhancing decision-making processes and improving patient outcomes. Despite its promising benefits, several challenges must be addressed to effectively incorporate AI into clinical guidelines. Objective: This research aims to explore the opportunities and challenges associated with integrating AI into clinical practice guidelines. The study seeks to identify the benefits AI can offer in clinical settings and the obstacles that may hinder its successful implementation. Methodology: A comprehensive review of existing literature on AI applications in healthcare was conducted, supplemented by expert interviews and case studies of AI implementations in clinical practice. The research analyzed AI’s impact on clinical decision-making, guideline development, and patient outcomes. Results: The findings reveal that AI integration can significantly enhance clinical practice guidelines by providing data-driven insights, improving accuracy, and personalizing patient care. However, challenges such as data privacy concerns, algorithmic bias, and the need for robust validation processes were also identified as significant barriers. Conclusion: Integrating AI into clinical practice guidelines offers substantial opportunities for improving healthcare delivery but requires careful consideration of associated challenges. Addressing these challenges through rigorous validation, ethical guidelines, and ongoing education will be essential for maximizing AI’s benefits in clinical settings. Future research should focus on developing strategies to overcome these barriers and ensure effective AI integration.

Abstract

Background: Chronic pain is a prevalent and debilitating condition affecting millions worldwide. With advancements in medical research and treatment modalities, there is a continuous need to update clinical practice guidelines (CPGs) to ensure optimal patient care. The frequent emergence of new evidence necessitates systematic updates to CPGs to maintain their relevance and efficacy. Objective: This systematic review aims to identify, evaluate, and summarize the latest updates in CPGs for the management of chronic pain, providing healthcare professionals with evidence-based recommendations to enhance patient outcomes. Methods: A comprehensive literature search was conducted across multiple databases, including PubMed, MEDLINE, and Cochrane Library, from January 2015 to December 2023. Inclusion criteria were studies that focused on updated CPGs for chronic pain management, written in English, and published in peer-reviewed journals. Data extraction was performed independently by two reviewers, and discrepancies were resolved by consensus. The quality of guidelines was assessed using the AGREE II instrument. Results: The review identified 32 updated CPGs across various chronic pain conditions, including neuropathic pain, fibromyalgia, and chronic low back pain. Key updates include the recommendation of multimodal approaches, incorporating pharmacological and non-pharmacological treatments, patient-centered care, and the importance of individualized treatment plans. Notably, recent guidelines emphasize minimizing opioid use and increasing the use of non-opioid analgesics and complementary therapies. Conclusion: The updated CPGs reflect a paradigm shift towards a more holistic and individualized approach in managing chronic pain. These guidelines underscore the importance of integrating patient preferences and multidisciplinary strategies. The findings of this review highlight the need for continuous education and training for healthcare providers to align with the evolving standards of chronic pain management.

Abstract

Background: The utilization of Evidence-Based Clinical Practice Guidelines (EBCPGs) in primary care has been advocated to enhance patient outcomes by providing standardized, high-quality care. Despite widespread promotion, there is a need for empirical evidence evaluating their actual impact on patient outcomes. Objective: This study aims to evaluate the impact of implementing EBCPGs on patient outcomes in primary care settings, focusing on common chronic conditions such as diabetes, hypertension, and asthma. Methods: A systematic review and meta-analysis were conducted, analyzing data from randomized controlled trials (RCTs) and observational studies published between 2000 and 2023. Studies included in the review examined the implementation of EBCPGs in primary care and reported on patient outcomes such as disease control, hospitalization rates, and patient satisfaction. Data extraction and quality assessment were performed independently by two reviewers. Statistical analyses were conducted using a random-effects model to calculate pooled effect sizes. Results: A total of 30 studies met the inclusion criteria, encompassing 15 RCTs and 15 observational studies. The pooled analysis revealed that the implementation of EBCPGs significantly improved disease control outcomes, with a moderate effect size (Hedges' g = 0.45, 95% CI: 0.30 to 0.60). Hospitalization rates for chronic conditions decreased by 20% (RR = 0.80, 95% CI: 0.72 to 0.89), and patient satisfaction scores improved significantly (mean difference = 0.25, 95% CI: 0.10 to 0.40). Conclusion: The implementation of EBCPGs in primary care settings is associated with improved patient outcomes, particularly in terms of disease control, reduced hospitalization rates, and enhanced patient satisfaction. These findings support the continued promotion and integration of EBCPGs in primary care practice to achieve better health outcomes.

Abstract

Background: The rise of digital communication has led to the widespread use of emojis, transforming them into a new form of language. Emojis are not only visual representations but also convey emotions, context, and nuances that words alone may not fully express. Understanding the evolution of emoji language is crucial for comprehending its impact on communication and social interactions. Objective: This study aims to explore the linguistic and social implications of emoji usage in digital communication. The research focuses on how emojis influence language, affect interpersonal interactions, and contribute to the development of digital communication norms. Methods: A mixed-methods approach was employed, combining quantitative analysis of emoji usage patterns with qualitative interviews. The quantitative analysis involved examining large datasets of text messages and social media posts to identify trends and patterns in emoji usage. Qualitative interviews were conducted with diverse participants to gain insights into their perceptions and experiences with emojis in communication. Results: The findings indicate that emojis serve multiple functions in digital communication, including emotional expression, tone modulation, and contextual clarification. Emojis were found to enhance the expressiveness of messages, reduce misunderstandings, and create a sense of closeness between communicators. The study also revealed variations in emoji usage based on age, gender, and cultural background, highlighting the role of emojis in reflecting social identity and group dynamics. Conclusion: Emojis have evolved into a significant component of digital communication, influencing both linguistic practices and social interactions. Their ability to convey emotions and context makes them a powerful tool for enhancing communication in digital spaces. Understanding the implications of emoji usage can inform the development of more effective communication strategies and digital platforms.

Abstract

Background: The inclusion of patient and family engagement (PFE) in the development of clinical practice guidelines (CPGs) is increasingly recognized as crucial for creating guidelines that are both relevant and effective. Engaging patients and families ensures that the guidelines address real-world concerns and preferences, potentially leading to better adherence and improved healthcare outcomes. Objective: This study aims to explore the role of patient and family engagement in the formulation of CPGs, examining how their involvement influences the content and quality of the guidelines. Methods: A systematic review was conducted, analyzing CPGs across various medical fields published between 2015 and 2023. The review focused on guidelines that explicitly reported PFE in their development process. Data was extracted on the methods of engagement, the extent of patient and family input, and the resultant changes in guideline recommendations. Quality assessment of the guidelines was performed using the AGREE II instrument. Results: The review identified 45 CPGs that incorporated patient and family engagement in their development. The methods of engagement varied, including surveys, focus groups, and direct participation in guideline panels. Guidelines with robust PFE tended to have higher AGREE II scores, particularly in the domains of 'stakeholder involvement' and 'clarity of presentation'. Notably, these guidelines often included recommendations that better aligned with patient values and preferences, emphasizing shared decision-making and personalized care. Conclusion: Patient and family engagement significantly enhances the relevance and quality of clinical practice guidelines. Involving patients and families in the guideline development process ensures that the recommendations are patient-centered, fostering greater trust and adherence to the guidelines. The findings underscore the importance of integrating PFE into all stages of guideline development to improve healthcare delivery.

Abstract

Background. Premarital education has an important role in preparing prospective brides and grooms to face various aspects of married life, including pregnancy readiness. Purpose. This study aims to compare the results of 20 journals regarding the effectiveness of premarital education in improving readiness for pregnancy. Method. This study covers various methods of premarital education such as counseling, educational programs, the use of leaflet media, and digital applications. The population in these studies varied from brides-to-be to young couples, with research locations spread across different regions of Indonesia. The sampling techniques used include purposive, random, and stratified sampling. The most common data collection tools are questionnaires, in-depth interviews, and documentation analysis. Results. Results from the 20 journals consistently showed that premarital education significantly improved readiness for pregnancy, with increased preparedness ranging from 30% to 70% after educational interventions. The statistical analysis used included  the Wilcoxon Test, T Test, and Chi-Square Test, all of which showed a significant p-value (p-value  < 0.05), indicating a significant influence of premarital education on readiness to face pregnancy Conclusion. The conclusion of this study is that various forms of premarital education, both through face-to-face, print and digital media, are effective in preparing the bride-to-be to face pregnancy, so it is important to be implemented more widely.

Abstract

Background. Old age is often considered a burden for productive people. This is motivated by the aging process, which includes physical decline and psychological and social problems. Seeing this, the church must take a role in discipling its people and not ignore the needs experienced by old age. Purpose. This is intended so that the church can take part in the welfare of the elderly by paying attention to every aspect of their growth and making it contextual with the Christian religious guidance provided. Methods. This study uses qualitative methods with descriptive types and data collection techniques, namely in-depth interviews, observation, and documentation. Results. The research results obtained from defining psychological and psychological-sociological needs based on B.S. Sidjaga, Ed.D is that the elderly tend to feel sad, easily suspicious, defend themselves against experiences, egocentrism, stress due to family, and tend to have irritable emotions. Furthermore, in the psychological-sociological phase, which tends to be in the phase of despair in line with Erik Erikson's theory of feeling worried, anxious, and afraid, This feeling is felt by almost all the elderly at the Tresna Werda Sinta Rangkang Social Institution, due to their background age before entering the orphanage and their decline. Conclusion. The elderly require coaching in psychological and psychological-sociological fields, as they often experience the indicators of integrity vs. despair, as per Erik Erikson's developmental theory. However, Christian religious formation at PSTW Sinta Rangkang has not prioritized these psychological conditions, leading to general teaching materials and monotonous coaching. To address this, Christian Religious Development should collaborate with churches, communities, and other Christian institutions, provide teaching materials that address the needs and struggles of the elderly, and create a written curriculum that outlines objectives, materials, methods, and evaluation of coaching activities

Abstract

Background. Mentorship is widely regarded as a crucial element in midwifery education, particularly in facilitating students’ transition from academic learning to professional clinical practice. Beyond technical skill acquisition, mentorship plays a significant role in shaping professional identity by influencing how midwifery students internalize professional values, responsibilities, and ethical standards within clinical settings. However, the mechanisms through which preceptors contribute to professional identity formation remain insufficiently explored. Purpose. This study aimed to examine the role of clinical preceptors in shaping the professional identity of midwifery students during clinical placements, with a particular focus on mentorship practices that support identity development. Method. The study employed a qualitative interpretive research design. Data were collected through in-depth interviews, reflective journals, and non-participant observations involving midwifery students and experienced preceptors in clinical learning environments. The data were analyzed thematically to identify recurring patterns related to mentorship interactions and professional identity formation processes. Results. The findings indicate that preceptors exert a substantial influence on students’ professional identity through role modeling, professional validation, reflective dialogue, and the gradual negotiation of autonomy in clinical decision-making. Supportive and consistent mentorship was associated with enhanced self-confidence, ethical awareness, and a stronger sense of professional belonging. Conversely, fragmented or limited mentorship contributed to uncertainty and weaker integration of professional identity. Conclusion. The study concludes that preceptorship functions as a formative relational process that extends beyond clinical supervision. It serves as a critical mechanism in the development of professional identity among midwifery students. Strengthening mentorship structures and enhancing preceptor preparation are therefore essential to fostering resilient, competent, and professionally grounded future midwives.

Abstract

Background. Historical studies of childbirth have largely relied on aggregate statistics and institutional archives, resulting in the marginalization of experiential knowledge and everyday practices of community-based midwives. Such approaches risk reducing complex care systems into depersonalized data, thereby overlooking how knowledge, authority, and maternal agency were constructed in practice. The diary of Martha Ballard offers a rare microhistorical account that captures the lived realities of maternal care in the eighteenth century. Purpose. This study aims to examine how Ballard’s diary reflects patterns of midwifery practice, forms of maternal agency, and modes of knowledge production that challenge dominant institutional and statistical narratives of pre-modern medicine. Method. This study employs a qualitative biographical research design, integrating microhistorical analysis with thematic coding of selected diary entries. Supporting historical sources are used to contextualize findings and enhance interpretive validity. Results. The findings reveal that Ballard’s midwifery practice functioned as a structured and adaptive system of care grounded in experiential knowledge, relational trust, and sustained community engagement. Evidence indicates that midwifery operated effectively despite the absence of formal institutional frameworks. Furthermore, the diary demonstrates that midwives acted not only as healthcare providers but also as producers of knowledge and holders of social authority within their communities. Conclusion. This study highlights the importance of incorporating narrative-based and experiential evidence into historical analysis. It argues that personal documents such as Ballard’s diary provide critical insights into alternative healthcare epistemologies, enabling a more comprehensive understanding of maternal care beyond institutional perspectives.
