Abstract

Riverine populations that rely on contaminated water bodies for everyday activities and food sources are most affected by the growing environmental and public health concern of microplastic pollution in river ecosystems. One of the world's most polluted rivers, the Citarum River is a high-risk environment for long-term exposure to microplastics, which may have an impact on women's reproductive health. An analytical cross-sectional design with an environmental and biomonitoring approach was applied to women of reproductive age (18–45 years) who had resided in the study area for at least two years, representing chronic exposure conditions. Environmental samples of river water, sediment, and locally consumed fish were analyzed using micro-FTIR or Raman spectroscopy, with polymer confirmation by Py-GC-MS. Individual exposure was assessed through fecal microplastic counts and urinary measurement of plastic-related chemical biomarkers, including phthalate metabolites (MEHP) and bisphenol A (BPA). The results demonstrated elevated levels of microplastics in environmental matrices and biological samples. Higher exposure categories were consistently associated with increased oxidative stress and inflammatory biomarkers, reduced anti-Müllerian hormone levels, elevated gonadotropins, and a higher prevalence of menstrual cycle disturbances. These findings indicate a dose–response pattern linking chronic microplastic exposure with biological stress and reproductive dysfunction. Overall, this study provides empirical evidence that long-term exposure to microplastics in contaminated river environments may pose a significant risk to women’s reproductive health, underscoring the need for environmental control and targeted public health interventions.

Abstract

Improper household waste management is a major risk factor for the presence of disease vectors in residential environments. This study aimed to analyze the association between household waste management and the presence of disease vectors in residential areas in Medan Marelan Subdistrict, Medan City, Indonesia. This research employed a quantitative analytic design with a cross-sectional approach. A total of 120 households were selected using simple random sampling. Data were collected through structured interviews and environmental observations. Data analysis was conducted using univariate and bivariate methods, including the Chi-square test with a 95% confidence level and Odds Ratio (OR) analysis. The results showed that the majority of respondents had poor waste management practices (61.7%), and disease vectors were present in 65.0% of households. Statistical analysis indicated a significant association between household waste management and the presence of disease vectors (p = 0.001), with an OR value of 6.67 (95% CI: 2.85–15.62). This finding suggests that households with poor waste management were 6.67 times more likely to have disease vectors compared to those with proper waste management. It can be concluded that household waste management has a significant association with the presence of disease vectors in residential environments. Therefore, strengthening community education regarding waste management, improving sanitation facilities, and increasing collaboration between health workers and local communities are important to support environmental sanitation and reduce vector-related health risks.

Abstract

Flooding is a hydrometeorological disaster that increases the risk of skin diseases due to poor sanitation and inadequate personal hygiene. This study aimed to analyze the relationship between access to basic sanitation and personal hygiene with the occurrence of skin diseases among flood-affected communities in Padang City in 2025. A quantitative study with a cross-sectional design was conducted involving 150 respondents selected using simple random sampling. Data were collected through structured questionnaires and observations and analyzed using chi-square tests and logistic regression. The results showed that 62.7% of respondents experienced skin diseases. There were significant associations between access to basic sanitation (p = 0.003) and personal hygiene (p = 0.001) with the occurrence of skin diseases. Multivariate analysis indicated that inadequate sanitation was associated with a 2.4-times higher likelihood of skin diseases (OR = 2.40; 95% CI: 1.35–4.26), while poor personal hygiene was associated with a 3.1-times higher likelihood of skin diseases (OR = 3.10; 95% CI: 1.68–5.72). It was concluded that personal hygiene was the most dominant factor associated with the occurrence of skin diseases. Therefore, integrated hygiene-based interventions and the provision of adequate sanitation are important to reduce the risk of skin diseases among flood-affected communities.

Abstract

The classroom physical environment plays a critical role in shaping students’ learning concentration. Suboptimal conditions of temperature, humidity, and noise can reduce comfort and impair students’ ability to maintain focus during instructional activities. This study aimed to examine the relationship between classroom environmental factors (temperature, humidity, and noise) and learning concentration among senior high school students. An analytic observational study with a cross-sectional design was conducted involving 120 students from grades X and XI, selected through simple random sampling. Temperature and humidity were measured using a thermohygrometer, while noise levels were assessed with a sound level meter. Learning concentration was evaluated using a structured questionnaire. The questionnaire had been tested for validity and reliability (Cronbach’s alpha > 0.70), and ethical approval along with informed consent was obtained prior to data collection. Data were analyzed using univariate and bivariate approaches, with the Chi-square test applied at a significance level of 0.05. The findings indicated that most classroom conditions did not meet recommended standards, including temperature (58.3%), humidity (55.0%), and elevated noise levels (60.0%). Furthermore, the majority of students demonstrated low levels of learning concentration (56.7%). Bivariate analysis showed that temperature was not significantly associated with learning concentration (p = 0.231), whereas humidity (p = 0.002) and noise (p = 0.001) were significantly associated. In conclusion, humidity and noise are key determinants of students’ learning concentration, while temperature appears to have no significant effect. These findings underscore the importance of optimizing classroom environmental conditions to support effective learning.

Abstract

Tidal flooding (rob) has become a persistent environmental hazard in Indonesia’s coastal regions, particularly in Pekalongan City, Central Java, leading to significant degradation of basic sanitation infrastructure and increased health risks. This study aims to analyze the impact of tidal flooding on sanitation facility damage and the shifting patterns of disease among coastal communities. A quantitative cross-sectional design was employed involving 120 households in flood-prone areas of North Pekalongan. Data were collected through structured interviews, direct observation, and secondary health records from local health centers. Variables included sanitation condition, water quality, and incidence of waterborne and environmentally related diseases. Statistical analysis using chi-square and logistic regression revealed a significant relationship between sanitation damage and increased incidence of diarrhea, skin diseases, and acute respiratory infections (p < 0.05). Households with damaged sanitation facilities had a 2.8 times higher risk of experiencing diarrhea (OR = 2.8; 95% CI: 1.4–5.6). Approximately 65% of sanitation facilities were found to be damaged or non-functional during flooding periods, leading to contamination of clean water sources. These findings highlight the urgent need for climate-resilient sanitation systems, including flood-adaptive septic tank designs and improved wastewater management in coastal settlements. The study concludes that tidal flooding significantly contributes to sanitation degradation and alters disease patterns in coastal populations.

Abstract

Air pollution from transportation activities at bus terminals poses significant environmental health risks, particularly in densely populated urban areas. This study aims to assess the health risks associated with exposure to air pollutants among individuals working at bus terminals in Medan City, Indonesia. A quantitative descriptive approach using Environmental Health Risk Assessment (EHRA) was applied. Data were collected through direct measurements of carbon monoxide (CO), nitrogen dioxide (NO₂), and particulate matter (PM2.5), as well as interviews to determine exposure duration, frequency, and body weight. The study involved 60 respondents, including traders, drivers, and terminal officers. Risk characterization was conducted using the Risk Quotient (RQ). The results indicated that the concentrations of CO and PM2.5 exceeded WHO air quality standards during peak hours. The RQ values for a majority of respondents were greater than 1, indicating potential non-carcinogenic health risks. The highest risk was observed among traders due to prolonged exposure duration. These findings highlight the urgent need for emission control strategies, improved environmental management, and health protection measures for vulnerable populations. In conclusion, EHRA is an effective tool to evaluate environmental health risks in transportation hubs.

Abstract

Irritant contact dermatitis is one of the most common occupational skin disorders among informal sector workers, including waste collectors who are routinely exposed to irritant materials. This study aimed to analyze factors associated with irritant contact dermatitis symptoms among waste collectors in Pasar Raya Padang. An analytical observational study with a cross-sectional design was conducted involving 75 respondents selected using a total sampling technique. Independent variables included duration of exposure, length of employment, personal hygiene, and use of personal protective equipment (PPE). Data were collected using structured questionnaires and direct observation, then analyzed using the chi-square test with a significance level of 0.05 and presented as odds ratios (OR). The results showed that 58.7% of respondents experienced dermatitis symptoms. Significant associations were found between length of employment (p=0.021; OR=3.10) and PPE use (p=0.013; OR=3.85) with dermatitis symptoms. Meanwhile, duration of exposure (p=0.642; OR=1.28) and personal hygiene (p=0.274; OR=1.54) were not significantly associated. Waste collectors with longer working periods and non-compliance with PPE use had a higher risk of experiencing dermatitis symptoms. In conclusion, length of employment and PPE use were significant factors associated with irritant contact dermatitis symptoms among waste collectors. Strengthening PPE compliance and occupational health education are recommended to reduce the risk of occupational skin disorders in informal sector workers.

Abstract

Occupational noise exposure remains a major health risk among fishermen, particularly those operating motorized boats. This study aims to analyze the correlation between noise exposure intensity, length of employment, and hearing loss among fishermen in Pesisir Selatan Regency, West Sumatra, Indonesia. A cross-sectional quantitative design was applied involving 60 fishermen selected through purposive sampling. Noise intensity was measured using a sound level meter, while hearing loss was assessed using audiometric examination. Length of employment was obtained through structured interviews. Data were analyzed using Pearson correlation and multiple regression tests. The results showed that the average noise exposure exceeded the permissible limit of 85 dB. There was a significant positive correlation between noise intensity and hearing loss (r = .62; p < .01), as well as between length of employment and hearing loss (r = .58; p < .01). Simultaneously, both variables significantly influenced hearing impairment among fishermen (p < .001). These findings indicate that prolonged exposure to high-intensity noise increases the risk of Noise-Induced Hearing Loss (NIHL). The study suggests the need for occupational health interventions, including the use of hearing protection devices and routine health monitoring. In conclusion, noise exposure and working duration are important determinants of hearing loss among fishermen and should be addressed in policy and practice.

Abstract

Acute Respiratory Infection (ARI) remains a major health problem among children under five in Indonesia, including in Padang City. Poor housing physical conditions such as inadequate ventilation, insufficient natural lighting, high residential density, and improper humidity levels may increase the risk of ARI among toddlers. This study aimed to determine the relationship between housing physical conditions and the incidence of ARI among toddlers in Andalas Padang 2026. This study used an analytic observational design with a cross sectional approach. The study population consisted of mothers with toddlers aged 12–59 months in the Andalas, Padang City. A total of 80 respondents were selected using proportional random sampling technique. Data were collected through interviews, direct observation of housing conditions, and measurement of temperature, humidity, and lighting using environmental measuring instruments. Data were analyzed using univariate and bivariate analysis with the Chi-Square test at a 95% confidence level (α = 0.05). The results showed significant relationships between house ventilation (p-value = 0.004), natural lighting (p-value = 0.018), residential density (p-value = 0.001), and house humidity (p-value = 0.015) with the incidence of ARI among toddlers. Residential density was the variable most strongly associated with ARI incidence. In conclusion, housing physical conditions are associated with the incidence of ARI among toddlers. Abstract

Flooding is one of the most common disasters in Indonesia and often causes deterioration in clean water quality, particularly through increased turbidity, color, and suspended solids. One alternative for improving post-flood water quality is the use of simple filtration systems with different filter media combinations. This study aimed to analyze the effect of filtration media variations on the physical quality of post-flood water. An experimental study using a Completely Randomized Design (CRD) was conducted with four treatments and three replications. Water samples were collected from flood-affected areas in Padang City, West Sumatra, Indonesia. The treatments consisted of P0 (control), P1 (sand and gravel), P2 (sand, gravel, and activated carbon), and P3 (sand, gravel, activated carbon, and zeolite). The observed parameters included turbidity, color, and Total Suspended Solids (TSS). Data were analyzed using One-Way Analysis of Variance (ANOVA) at a 95% confidence level. The results showed that variations in filtration media significantly affected turbidity, color, and TSS (p < 0.05). Descriptively, treatment P3 showed the lowest mean values of turbidity (7.8 NTU), color (11.7 TCU), and TSS (12.0 mg/L) among all treatments. However, because no post hoc analysis was performed, these findings should not be interpreted as evidence that P3 was statistically superior to the other treatment combinations. The values obtained in P3 met the applicable water quality standards for all observed physical parameters. It can be concluded that variations in filtration media significantly influence the physical quality of post-flood water, and a filtration system combining sand, gravel, activated carbon, and zeolite has potential as an appropriate technology for post-disaster clean water provision.
