Abstract

Cancer is a disease that causes a lot of misery and death in humans. According to data from the World Health Organization (WHO) in 2015, there are 8.8 million deaths caused by cancer and colorectal cancer ranked third with 774,000 deaths. One of the prevention effort for colorectal cancer is early detection in the population, especially in high-risk groups and in groups with non-specific symptoms. In general, two types of early detection tests are available: stool-based testing and structural testing. Fecal immunochemical test (FIT) is one type of examination that is categorized as the tool-based test. Meanwhile, the structural test include colonoscopy, CT colonography and flexible sigmoidoscopy. This study aimed to determine difference in the effectiveness of FIT against colonoscopy as an early detection tool for colorectal cancer. This study was a cross-sectional comparative diagnostic analytical observational study to compare FIT to colonoscopy for early detection of colorectal cancer. This study involved patients presented to Dr. Hasan Sadikin General Hospital Bandung, Indonesia. Analysis of diagnostic test data was carried out to obtain the sensitivity, specificity, and the positive and negative predictive values through the use of bivariate analysis in the form of Man Whitney test. Thirty-eight patients were included in this study, consisting of 27 cancer patients and 11 non-cancer patients. Results showed that the sensitivity level of FIT was 81.5%, with 72.7% specificity. There is no significant difference in the effectiveness between FIT and colonoscopy in screening for colorectal cancer.

Abstract

The high incidence of breast cancer cases in the world requires the use of applicative methods. The 7,12-dimethylbenz(a)anthracene (DMBA) induced breast cancer animal model is a widely used chemical-induced animal models for research on breast cancer. However, the molecular mechanism related to DMBA induction remains unclear. Good understanding on DMBA-induced animal models is crucial for studies related to future breast cancer treatments as animal models will provide a deeper understanding of anticancer medication, specifically those aimed for treating breast cancer. The aim of this study was to develop an DMBA-induced animal model for breast cancer. This study used female Wistar rats injected subcutaneously with DMBA as a carcinogen-induced agent (20 mg/kg) to induce tumor. Rat tumors were then evaluated and breast appearance was observed weekly, starting from day 28th after DMBA injection. Breast cancer tissue was then sampled and stored at -80°C until it was used for western blot and histological study. This study indicated that DMBA induced cancer in female Wistar rat’s breasts, and cytoplastic cells and lung metastatic was identified macroscopically and histopathologically. The metabolic sign was observed in the lung and breast sections. Interestingly, the DMBA induction in this study does not only induce organ cancers but also induces estrogen receptors and stimulates signaling of estrogen receptors α (ERα), ERβ, and Akt.

Abstract

Ninety percent of malaria cases in Indonesia come from Papua, West Papua, and East Nusa Tenggara provinces. The country has declared a target of malaria elimination by 2030. Malaria elimination efforts have been performed through case finding, treatment, surveillance, and risk factor prevention and control. Personal protection is crucial in preventing and reducing the risk for malaria infection. This study aimed to assess the significance of personal protection against mosquitoes in eastern Indonesia. This was a cross-sectional study using the secondary data from a previous study of malaria in eastern Indonesia in 2013–2014. Data were collected using a questionnaires and direct observation and analyzed using the chi-square with α 0.05 and 95% CI. A total of 551 data were collected (180 positive vs 371 negative). The use of mosquito nets at night (p-value: 0.038; OR: 3.127) has a significant relationship with the incidence of malaria while the use of ventilation screen (p-value: 0.191; OR: 0.839), vector shelter (p-value: 0.493; OR: 0.852), and mosquito repellent (p-value: 0.564; OR: 1.585) did not have significant relationships to the incidence of malaria. Mass distribution of mosquito nets by the local governments can constitute one of the efforts to prevent and control malaria in South Central Timor District.

Abstract

Increased prevalence of diabetes mellitus (DM) leads to the increased of various complications including diabetic nephropathy, which can lead to the end stage renal disease (ESRD). The Epithelial sodium channel (ENaC), which is located in distal convoluted tubules, plays an important role in transepithelial sodium reabsorption for electrolyte homeostasis. Diabetes mellitus can cause changes in ENaC function which will interfere with kidney blood pressure control, worsening hypertension, and kidney injury that eventually may trigger diabetic nephropathy. The aim of this study was to determine the validity of urinary ENaC for screening diabetic nephropathy in type 2 diabetes mellitus (T2DM) with uACR as the gold standard. This was a cross-sectional analytical observational study conducted in Dr. Hasan Sadikin General Hospital Bandung, Indonesia, from December 2020 to July 2021. The subjects were 87 patients T2DM with hypertension with the majority of subjects (n=62) had a mean age of 56 years old and were experiencing albuminuria and hyperglycemia with DM for a duration of < 10 years. The poor glycemic control in these patients accelerated the occurrence of kidney damage. Result showed that urine ENaC level had a sensitivity and specificity of 82.3% and 48%, respectively, with 72.4% accuracy. The cut-off point of urine ENaC in this study was 0.98 ng/mL. Hence, urine ENaC level can be used as a test to screen for diabetic nephropathy with 82.3% sensitivity.

Abstract

Covid-19 patients with comorbidities, such as hypertension, diabetes, and respiratory diseases, have a higher risk for severe disease and mortality compared to patients without comorbidities. Some data shows that the administration of high dose vitamin C (1–2 g/day) can reduce the amount of the inflammatory mediators and shorten the length of hospital stay. This study aimed to determine the relationship between vitamin C administration and of length of hospital stay for Covid-19 patients with comorbidities. This study was a retrospective observational study on  consecutively sampled medical record data of UNS Hospital patients from July 2020 to January 2021. A total of 78 patient medical records were included of this study. The inclusion criteria were all medical records of patients who suffered from Covid-19 with comorbidities admitted to the hospital while the exclusion criteria were medical records of patients who did not get oral or intravenous vitamin C. The chi-square bivariate test was used to determine the relationship between the administration of vitamin C and the length of hospital stay for Covid-19 patients with comorbidities. Of the 78 subjects only 52 subjects were included in the statistical analysis. Medical records were divided into two groups: group 1, that consisted of medical records of patient who received more than 1,000 mg Vitamin C, and group 2, who received less than 1,000 mg Vitamin C. Results demonstrated that the administration of high dose vitamin C of more than 1,000 mg was not associated with the length of hospital stay (p=0.677). Thus, high dose of vitamin C is not associated with the length of hospital stay for Covid-19 patients with comorbidities, such as hypertension, diabetes, and respiratory disease.

Abstract

It is estimated that 6% of the US population suffer from diabetes with 15% of them experience diabetic foot. Several studies have explored the various benefits of sea cucumber for human being, including its antioxidant nature. This study aimed to investigate the effect of surf redfish, one species of the sea cucumbers, in diabetic wound healing. This was an animal experimental study performed at the Pharmacology and Pharmaceutical laboratory of the Faculty of Pharmacy, Universitas Tjut Nyak Dhien Medan, Indonesia, during the period of February- July 2019. Twenty five mice in diabetic condition were divided into five groups: control, standard, ethanol extract of surf redfish-1, ethanol extract of surf redfish-2, and ethanol extract of surf redfish-3 3. The surf redfish was obtained from Lamreh Village in Aceh and the ethanol was extracted using the maceration method. The parameters used for evaluating the efficacy of surf redfish ethanol extract were blood glucose level, length of wound incision, and histopathological features. Surf redfish extract presented a significant reduction of blood glucose level which that followed the increase in the dose of the ethanol extract (p-value <0.05) where the most significant reduction was seen in the group with the highest dose on the last day. This trend was also seen for the length of the wound, where the reduction was significantly higher in treatment groups when compared to control (p-value <0.05). The histological study also confirmed the improvement in the clinical appearance of wound by showing significantly increased fibroblast cell number and collagen density (p-value <0.05). Overall, the surf redfish can help better incision's wound healing in diabetic condition.

Abstract

Spinocerebellar ataxia (SCA) 3 is a neurodegenerative disease which involves cerebellum and extra cerebellum. Neuropathy in SCA3 manifests in various ways, including axonal and demyelination lesions in sensory and motor nerves. There has not been any study that describes the peripheral neuropathy characteristics of SCA3 patients in Indonesia at the time of this publication. This paper reports a case of a 43-year-old male with known spinocerebellar ataxia 3 presented with hereditary ataxia and mild numbness in both palms since two years before. No abnormalities were found during the sensory examination. The NCS showed severe axonal demyelinating sensorimotor peripheral neuropathy. In magnetic resonance imaging (MRI), an atrophy in the cerebellum with cerebral multiple lacunar infarction was identified. Electrophysiological results revealed profound axonal lesion in peripheral nerves. To conclude, peripheral neuropathy in SCA3 represents the dominance of axonal lesions in motor nerves.

Abstract

Mortality and health disturbances in children often correlate with maternal health and fertility. Avocado, mung bean sprouts, and holy basil have been traditionally used to improve maternal health, before and during pregnancy. This study was aimed to assess the efficacy of herbal combination of avocado, mung bean sprouts, and holy basil (Jamu ATOKE) in optimizing reproductive health. Eighteen female Sprague Dawley rats (9–10 weeks old, BW: 180–250 g) were randomly divided into 3 treatment groups (n=6 rats for each group) of control, group I and II. ATOKE were added into rats drinking water and consumed for 30 days before pregnancy. After the rats were pregnant and gave birth, pup per parent ratio, pup mortality, and pup health performance (body weight gain, feed and drink consumption, motor activity, pup speed in finding light and feed, red blood cell (RBC) count, white blood cell (WBC) count and differential, SGPT, SGOT, blood urea nitrogen (BUN), creatinine, and proinflammatory mediators (TNF-α and IL-6) were analyzed. Pups born to in group I had a generally better health performance compared to the control group. There was a 100% mortality in the litter of group II due to the excessive aggressive behavior of the dams as a sign of toxicity. Administration of 2.5% Jamu ATOKE showed the best effects on the health and intelligence of the rat pup.

Abstract

In facilities where a track required is not available for gait speed test, a simpler method using a shorter track and test duration is needed. One of such methods is the 4-meter gait speed. This study aimed to determine the cut off values for 4MGS and its correlation with the 6-minute walk test (6MWT), a more common functional performance test with shorter track and time. Data for this study were collected at the Department of Rehabilitation of Dr. Cipto Mangunkusumo General Hospital during the period of August 2016 to March 2017. The consecutive sampling approach was used to recruit healthy Indonesian adults (n=61) aged 18–50 years into this cross sectional study. The anthropometric and respiratory variables were measured as descriptive data and the cut-off values were determined by calculating the mean 4-meter gait speed (4MGS)-2SD for each gender. The 6MWT was performed afterwards to assess the correlation between 4MGS and 6-minute walk distance (6MWD). Results presented a mean 4MGS value of 2.114±0.309 m/s (male) and 1.908±0.227m/s (female) with the cut-off values for normal gait speed of 1.496 m/s and 1.454m/s for male and female, respectively. Factors affecting 4MGS were body height, body weight, forced vital capacity (FVC), and forced expiratory volume in 1 second (FEV1). The 4MGS is only shown to be weakly correlated with 6MWD (r=0.314, p=0.014). The cut-off values for 4MGS can be used as a reference to assess the normality of gait speed as a parameter of functional mobility in healthy Indonesian adults and should be used in adjunct to 6MWT to represent a comprehensive functional capacity status.
