Abstract

One of the five possible mechanisms of leptin resistance in human obesity is the defect in the leptin receptor (Ob-R). Evidence has accumulated that leptin-binding activity in human serum is related to a soluble form of the leptin receptor, and restriction of energy intake resulted a decrease in circulating leptin levels. Aim of this study is to examine the difference of serum soluble leptin receptor level and leptin receptor density in rat adipose tissue of adventitial aorta after four weeks treated with different restricted diets. Soluble leptin receptor level was measured by ELISA and leptin receptor density by using immuno-histochemistry. The soluble leptin receptor in group treated with 40% of normal daily calori diet was found significantly lower than control (p = 0.02). There were no any significant differences among group treated with 40 % of normal daily calori diet, “1 day fast-1day eat”, and ”1day fast-2 days eat” groups, and among 1 day fast-1 day eat”, ”day fast - 2 days eat” and control groups as well. On the other hand, leptin receptor density in adipose tissues was higher in restricted diet group than control. Diet of 40 % normal daily calorie for 4 weeks decreased soluble leptin receptor level, but increased adipocyte leptin receptor density of the adipose tissue of rat adventitial aorta. These changes may be resulted from an up regulation mechanism in relation with homeostatic maintenance. Abstract

Liver diseases have been a medical problem which is difficult to manage. Some of the problems in the treatment of these diseases lie in the lack of reliable drug available. Curcumin, an active ingredient of the rhizomes of plant Curcuma has been investigated in the treatment of various disorders incuding liver diseases. The therapeutic effects of curcumin on liver diseases have been thought to be associated to its antioxidative properties. In the present study, we investigated the effects of curcumin on mitochondrial swelling in vitro induced by tert-butylhydroperoxide (t-BuOOH). Liver mitochondria were homogeneously isolated from Sprague-Dawley rats (the relative specific activity of succinate dehydrogenase was 35.73 ± 2.78). Addition of 90 µM of t-BuOOH caused a typical 2-phase swelling of the mitochondria. The pattern of swelling was influenced by various factors such as buffer composition, concentrations of t-BuOOH, amount of isolation buffer and mitochondrial proteins and incubation temperature.The swelling could be reduced by as much as 85 ± 3% by 2.50 µM of curcumin. At lower (1.25 µM) or higher (5.00 µM) concentrations, the protection against swelling by curcumin were less effective (respectively were 41 ± 3% and 77 ± 6%). Swelling might occur due to the opening of mitochondrial transition pore and could be an initial indication in the cascade process leading to cell death. The inhibition of t-BuOOH-induced mitochondrial swelling by curcumin might be because of the antioxidant effects of the compound. Abstract

There is limited information about bone implant restoration treatment on edentulous patient with hyperthyroidism. This clinical report is presenting the making of dental bone implant restoration on patient with hyperthyroidism history. A 60 years old male patient with hyperthyroidism came to Prosthodontic Clinic Faculty of Dentistry University of Indonesia to improve his ailing denture. After comprehensive evaluation we treated the patient with Implant-Tissue-Supported Overdenture (4 Implants) for rehabilitating upper edentulous jaw, and 2 Implant-Tooth-Supported Fixed Partial Denturesfor rehabilitating Kennedy class II lower edentulous jaw respectively. Abstract

Patients with chronic obstructive pulmonary disease (COPD) have been shown to be benefit from pulmonary rehabilitation programs. We assessed an entirely outpatient-based program of pulmonary rehabilitation in patients with COPD, using the St George’s Respiratory Questionnaire (SGRQ) and six minutes walking distance test (6MWD) (which measures health-related quality of life and functional exercise tolerance) as the primary outcome measure. We undertook a randomized, opened, prospective, parallel-group controlled study of outpatient rehabilitation program in 56 patients with COPD (52 men and 4 women). The active group (n=27) took part in a 6-weeks program of education and exercise. The control group (n=29) were reviewed routinely as medical outpatients. The SGRQ and 6MWD were administered at study entry and after 6 weeks. Outcome with SGRQ and 6MWD before and after therapy was performed. Decrease score SGRQ and increase 6MWD in both groups of study, it was analyzed by statistic study and in active group the decrease of SGRQ and the increase of 6MWD was statistically significant. In conclusion 6-weeks outpatient-based program significantly improved quality of life and functional capacity in mild-to-moderate COPD patient. Abstract

The purpose of this study is to know whether there are differences between hematology profile and morphology of erythrocytes of blood specimens which are prepared with excessive Na2EDTA anticoagulant in different concentration. This study was conducted in Faculty of Medicine Gadjah Mada University. The criteria of subject were male, age from 18 until 22 years old and healthy, ascertained from history taking and vital sign examination. Blood samples from 33 subjects were taken using vein puncture. Two millimeters blood was divided into 4 Na2EDTA-containing tubes. Before that, one drop of blood without Na2EDTA anticoagulant was used for making control blood film right after vein puncture. Each tubes contained different concentration of anticoagulant. The first tube contained Na2EDTA in standard concentration 2 mg/dl; the remaining tubes contained consecutively, 4 mg/dl, 6 mg/dl, and 8 mg/dl. Those samples were immediately examined using SYSMEX SE-9500 automatic analyzer for measuring erythrocytes hematological profile and were stained with Wright staining for morphological examination. These procedures were done before 20 minutes of vein puncture. There were significant decrease of RBC count, HGB, HCT, and MCHC and also significant increase of MCV and RDW between different concentrations of excessive Na2EDTA anticoagulant. MCH did not have significant result. Morphological examination showed significant morphological changes in the form of echinocytes and appearance of ghost cells in the sample treated with excessive Na2EDTA anticoagulant concentration. In conclusion, there are differences in hematological profile and morphology of erythrocytes among blood specimen which are prepared with excessive Na2EDTA anticoagulant in different concentration, except for MCH. Excessive Na2EDTA anticoagulant concentration will affect the blood specimen for peripheral blood examination of erythrocytes by interfering morphology and some of hematological parameters. Abstract

Atrial fibrillation (AF), which is called as a global epidemic disease, frequently found in hypertensive patients with left ventricular hypertrophy (LVH). Pulmonary vein (PV), which is known to have an important role in AF initiation and maintenance, increases in its diameter during AF. We sought to investigate PVs diameter changes in LVH with sinus rhythm. Of 70 hypertensive patients with sinus rhythm, 42 subjects demonstrated LVH. The mean ostial diameter of patient with and without LVH, assessed by doing spiral multisliced CT scan in the axial plane, were as follow: right superior (RSPV) of 19.6±2.78 vs 17.8±1.93 (p = 0.003), right inferior (RIPV) of 18.4±3.12 vs 16.0±2.19 (p < 0.001), left superior (LSPV) of 18.1±2.62 vs 16.0±2.16 (p < 0.001), and left inferior (LIPV) of 15.9±1.93 vs 15.4±1.85 mm (p = 0.284), respectively. Even during sinus rhythm, LVH causes PV dilation. This result might give an explanation of frequent AF prevalence in hypertensive patients. Abstract

Helicopter pilots exposed to high intensity noise and other risk factors had increased risk to be noise-induced hearing loss (NIHL). Therefore, it is beneficial to study several risk factors related to NIHL. This study was a nested case-control. Data was extracted from available medical records among helicopter pilots who performed routine aerophysiology training indoctrination (ILA) during 1980 through March 2004 at Saryanto Institute for Aviation and Aerospace Medicine (Lakespra), Jakarta. Case was those who had audiogram with a notch of 40 dB or more and of 4000 Hertz on one site or bilateral ears. A case was matched by two controls who free from NIHL up to 2004. All risk factors for cases and controls were counted as of reference date of cases diagnosed. There were 187 medical records available for this study. A number of 32 cases and 64 controls were identified. The final model reveals that NIHL was related to total duration of works, flight hours, and blood pressure. Those who had 500 hours or more than less 500 hours had a moderate increased risk for 2.5 to be NIHL [Adjusted odds ratio (ORa)= 2.50; 95% confidence intervals (CI) = 0.66-9.29; p = 0.180]. Those who had total duration works 11-24 years had a moderate increased to be NIHL for 2.7 times (ORa = 2.71; 95% CI=0.90-8.10; p = 0.075). Furthermore, prehypertension and hypertension stage 1 subjects than normal blood pressure had moderate trend increased risk to be NIHL. In conclusion total flight hours for 500 hours or more, total duration works 11-24 years, or prehypertension and hypertension stage 1 increased risk NIHL. Abstract

Almost everyone, including employees, may develop hypertension. Several risk factors, including stresses in the work environment, are related to hypertension. The aim of this study is to identify these work-related risk factors in hypertension. A nested case-control study was conducted among office employees in Jakarta during May 2004. Employees with stage 1 or stage 2 hypertension (based on the United States of America Joint National Committee on high blood pressure 2003), or those taking antihypertensive drugs were designated as cases. As controls were employees with no history of hypertension. One case was randomly matched by gender with two controls. All risk factors for cases and controls were counted as of the reference date of diagnosis for cases. There were 70 cases and 140 controls aged 25 to 65 years. Hypertension was found to be related to the qualitative and quantitative increase in the workload, career development, age, obesity, current and past smoking habits, and a family history of hypertension. However, it was noted that role of ambiguity, role of conflict, and personal responsibility did not increase the risk of hypertension. Compared to those with low qualitative job stressor, those who had moderate or high qualitative job stressor had a seven-fold risk to be hypertensive [adjusted odds ratio (ORa) = 7.47; 95% confidence interval (CI) = 1.40-39.76]. In addition, relative to those who had low quantitative job stressor, those with moderate or high stressors were four times at risk to be hypertensive (ORa = 4.10; 95% CI = 1.06-15.90). In conclusion moderate or high qualitative and quantitative job stressors as well as career development increased risk hypertension. Therefore these stressors need to be prevented. Abstract

The objective of this study is to investigate the anticancer activity of 70 % ethanol extract of Mahkota dewa fruit pulp Phaleria macrocarpa (Scheff.) Boerl. using C3H mouse mammary tumor induced by transplantation. Thirty two C3H mice were devided into 4 groups i.e. control and 3 groups of mice treated with ethanol extract of Mahkota dewa fruit pulp with the dose of 20, 40, and 80 fold human dose respectively, given orally by gastric tube after tumor transplantation for 30 days. Body weight and tumor volume measured twice a week. Tumor weight was measured after the animal was sacrificed, then fixed in formaldehyde for making histopathological preparation. The proliferation activity of tumor cells were examined by counting the AgNOR deposits detected after colloidal AgNOR staining. Apoptosis was assessed by mean of Tunel staining, and the width of necrotic area was identified by hematoxyllin eosin staining of the histological specimen. The results of the study showed that there were no statistical differences in tumor volumes, tumor weights, AGNOR values, and the necrotic area among control and the three treated groups (p>0,05), but apoptosis index significantly increased in the D3 (Mahkota dewa extract of eighty fold human dose) group (p<0,05). It was concluded that ethanol extract of Mahkota dewa fruit pulp at the dose of 20,40, and 80 fold human dose given orally after tumor transplantation for 30 days, did not inhibit the C3H mouse mammary tumor growth induced by transplantation, but the increased apoptosis was found in the group receiving ethanol extract of Mahkota dewa fruit pulp at the dose of 80 fold human dose. Abstract

Low birth-weight infant and intrauterine growth retardation are still a health problem, especially in Indonesia due to high prevalence and need to be reduced. Malnutrition in infants are most common occur in low birth-weight infants. Malnutrition in rats resulted in hypotrophic and normoplastic mucosa of the small intestine. The finding was not only showed that small intestine was able to maintain its cell number in condition with restriction nutrient, however also suggested the posibility of epithelial regeneration if given adequate nutrient intake. Did realimentation recover the hypotrophic normoplastic mucosa to normotrophic normoplastic? The study aim to answer that question. Experimental animal study with post test-control group design was performed using 40 male litter of Sprague-Dawley rats, was fed standard chow. The study was divided into phases prenatally-induced malnutrition and continued with phase realimentation. The result of this study is the body weight, mucosal thickness, villus height, cryptus depth, ratio of villus/ crypt, number of villi, protein content, and disaccharidases of rats realimentation group was higher than non-realimentation group, but lower than control group. Prenatally-induced malnutrition did not reduced the population of small intestinal enterocytes. Realimentation in rats in prenatally-induced malnutrition was able to improve the hypotrophy of small intestinal mucosa and to increase the disaccharidases activities but did not reach the normal values. Realimentation in rats in prenatally-induced malnutrition was able to improve the maturity of small intestine mucosa but did not reach the normal values. The information will be helpfull to decide the policy of maternal malnutrition.
