Abstract

Clinically, Magnetic Resonance Imaging (MRI) is a diagnostic imaging modality that does not use ionizing radiation and has a high capability for visualizing soft-tissue details, including the liver. In this matter, the dual-echo sequence (in-phase and out-of-phase) is a technique that utilizes the chemical shift phenomenon between water and fat, thereby enabling the identification of hepatic steatosis and the differentiation of fat-containing and non-fat-containing lesions. For this reason, this technique has become an important component in the evaluation of liver diseases with increasing prevalence, such as fatty liver disease and other hepatobiliary disorders. This research aims to analyse the implementation of the dual-echo MRI sequence in visualizing any liver abnormalities undertaken at Bakti Pajajaran Regional General Hospital, as well as to assess image quality and characterize the differences between in-phase and out-of-phase images. This research was conducted from September to November 2025 using a qualitative descriptive analytic method, involving among the other things observation of the examination process, in-depth interviews with radiographers and radiologists, and analysis of MRI images. As for the research population, it consisted of 44 patients undergoing MRCP examinations, and the aforementioned 14 patients who underwent imaging with the dual-echo sequence were accordingly selected as the research sample using purposive sampling. Furthermore, the research instruments included a 3-Tesla MRI scanner, observation sheets, interview guidelines, and DICOM viewer software. Primary data consisted of interview results with radiologists and radiographers, while secondary data included acquisition parameters, image quality assessments, and documentation of MRI findings. Data processing was conducted by collecting, selecting, organizing, and categorizing the data according to the research themes. Data analysis focused on the ability of the dual-echo sequence to depict fatty liver, focal lesions, diffuse liver abnormalities, and biliary abnormalities, using a qualitative approach that included data reduction, data presentation, and conclusion drawing. The results of the research indicate that the dual-echo sequence is highly effective in detecting hepatic steatosis, as demonstrated by a distinct signal drop on out-of-phase images. Non–fat-containing lesions, such as solid masses in the distal common bile duct (CBD) and hepatic cysts, did not show signal loss, allowing them to be clearly differentiated from fat-containing lesions. In addition, this technique provides supplementary information on hepatobiliary abnormalities, such as ductal dilatation and CBD stones, although it is not the primary MRCP sequence. Overall, image quality was good, influenced by consistent acquisition parameters and the successful implementation of breath-hold techniques. Ultimately, radiologists reported that this sequence is highly useful as an adjunct examination for assessing liver tissue composition.

Abstract

Research Background: Magnetic Resonance Imaging (MRI) is a non-invasive imaging modality that does not involve ionizing radiation and is highly effective in evaluating the soft tissue abnormalities, including those of the cervical vertebrae. In this matter, one of the most frequently encountered disorders in this region is cervical herniated nucleus pulposus (HNP), which may cause neck pain and radiculopathy and consequently reduce the patients’ quality of life. MRI is the primary imaging modality for establishing the diagnosis of HNP because it can clearly visualize among the other things the intervertebral discs, spinal cord, and intervertebral foramina. In clinical practice, however, cervical MRI examinations often encounter the motion artifacts, particularly in the patients with psychiatric disorders who tend to be uncooperative. In relation to which, T2 PROPELLER (Periodically Rotated Overlapping Parallel Lines with Enhanced Reconstruction) technique is known to be able to reduce the motion artifacts, thereby improving image quality and diagnostic value compared to the conventional T2-weighted sequences. Research Objective: This research aims to analyse the examination procedure and the quality of anatomical image information obtained from sagittal T2 Propeller sequences in cervical MRI for the cases of herniated nucleus pulposus (HNP) in the uncooperative patients with psychiatric disorders at RSKD Duren Sawit. Research Design: This research constitutes a qualitative study which was undertaken at Radiology Department of Duren Sawit Regional Special Hospital starting from August up to November 2025. The research employed an analytical approach to cervical MRI examinations using an additional sagittal T2 Propeller sequence in clinically diagnosed cases of herniated nucleus pulposus (HNP) in the uncooperative patients. As for the sample, it consisted of three patients selected based on the predefined inclusion and the exclusion criteria. While primary data were then obtained from direct observation of the MRI examination procedures and image acquisition results. Research Results: Based on the findings of this research conducted at Duren Sawit Regional Special Hospital, it can be concluded that the cervical MRI examination procedure for clinically diagnosed cases of herniated nucleus pulposus (HNP) in the uncooperative patients with psychiatric disorders was performed in accordance with the established theoretical principles, with the addition of sagittal oblique and sagittal T2 PROPELLER sequences. The sagittal T2 Propeller sequence was shown to produce greater image quality compared to the routine T2-weighted sequences, as it can effectively reduce the motion artifacts and enhanced the visualization of the spinal cord, intervertebral discs, and vertebral bodies.

Abstract

Background: Optimizing peak bone mass in early adulthood is important for long-term skeletal health. Milk provides calcium and high-quality protein that may support bone mineral density (BMD). However, evidence regarding the association between milk consumption and BMD in young adult women, particularly in non-Western populations, remains limited. This study aimed to assess whether milk consumption frequency is associated with BMD Z-scores in women aged 20–35 years. Methods:  An analytical observational study with a cross-sectional design was conducted among 60 premenopausal women aged 20–35 years. Milk consumption frequency was categorized as non-consumers, 1–3 times/week, and >3 times/week. Lumbar spine (L1–L4) and femoral neck BMD were measured using Dual-Energy X-ray Absorptiometry (DXA) and expressed as Z-scores. Normality was assessed prior to analysis, and One-way ANOVA was used to compare mean Z-scores across groups (α = 0.05), with additional sensitivity analysis using non-parametric testing due to imbalance in group sizes Results:  Mean (SD) Z-scores were 0.068 (0.927) for lumbar spine and −0.528 (0.797) for femoral neck. There was no significant difference in lumbar spine Z-scores across milk consumption groups F (2.57) = 0.479; p = 0.622). Similarly, femoral neck Z-scores were not significantly different F (2.57) = 2.673; p = 0.078), although a trend toward higher Z-scores was observed among participants with more frequent milk consumption. Conclusions:  In this sample of women aged 20–35 years, milk consumption frequency was not significantly associated with lumbar spine or femoral neck BMD Z-scores. These findings suggest that milk consumption alone may be insufficient to influence BMD in this population without considering other determinants such as total calcium intake, vitamin D status, and physical activity. Further studies with larger, more balanced samples and improved control of potential confounders are warranted.

Abstract

Background: Magnetic Resonance Imaging (MRI) is a diagnostic imaging modality that does not use ionizing radiation and offers high spatial resolution and excellent soft-tissue contrast, making it the primary modality for the evaluation of metastatic bone disease. In musculoskeletal examinations, particularly knee joint MRI, the sagittal T2 Turbo Spin Echo (TSE) sequence plays a crucial role in depicting bone involvement, bone marrow infiltration, and surrounding soft tissues in metastatic cases. This information is essential for assessing lesion extent and supporting accurate clinical diagnosis. In patients with metastatic bone disease, MRI examinations are often limited by severe pain, restricted mobility, and reduced patient tolerance to prolonged scan times. These clinical challenges necessitate the use of faster acquisition techniques without compromising diagnostic image quality. One technique developed to address this need is parallel imaging using Generalized Autocalibrating Partially Parallel Acquisitions (GRAPPA), which operates by undersampling k-space data to reduce acquisition time. However, the application of GRAPPA may influence image quality, thereby requiring objective and clinical evaluation. Methods: This study aimed to analyze differences in image quality, including anatomical structure information, image contrast and sharpness, and artifacts, in sagittal knee joint MRI using T2 Turbo Spin Echo sequences with and without GRAPPA in patients with metastatic bone disease. The study employed a quantitative analytic design with an experimental approach by comparing two acquisition techniques: sagittal T2 TSE imaging with GRAPPA and sagittal T2 TSE imaging without GRAPPA. Image quality assessment was based on contrast and image sharpness parameters, which were evaluated by professional respondents using a structured assessment instrument. The collected data were then statistically analyzed using the Wilcoxon test to determine differences in image quality between the two techniques. Results: the results demonstrated that the contrast and sharpness of knee joint MRI images acquired with GRAPPA are much better than those obtained without GRAPPA. This finding was supported by the Wilcoxon test (Z = −2.388; p = 0.017 < 0.05), indicating a statistically significant difference in image quality in the patients with metastatic bone disease. In addition, the use of GRAPPA in sagittal T2 TSE knee MRI also provided clear visualization of anatomical structures, with artifacts remaining within diagnostically acceptable limits, indicating that this technique is effective and comparable to non-GRAPPA imaging. Conclusions: The use of GRAPPA in sagittal T2 Turbo Spin Echo MRI of the knee joint provides significantly better image sharpness and contrast compared to non-GRAPPA imaging (Z = −2.388; p = 0.017). GRAPPA also enables clear visualization of knee joint anatomical structures with diagnostically acceptable artifact levels. Therefore, GRAPPA is an effective technique for improving anatomical image quality in knee MRI of metastatic bone disease.

Abstract

Background: Epidemiologically, fatty liver disease is one of the most common liver disorders worldwide. The severity of fatty liver is classified based on the proportion of hepatocytes containing fat, with categories ranging from normal to mild, moderate, and severe. In relation to which, Magnetic Resonance Imaging (MRI) is a reliable modality for the detection and quantification of fatty liver and demonstrates higher diagnostic accuracy compared with CT and ultrasound. The T1 dual-echo sequence is used as a qualitative method for assessing fatty liver based on radiologist interpretation, whereas the IDEAL IQ sequence serves as a quantitative method by providing proton density fat fraction (PDFF) values. Methods: This research was undertaken at Radiology Department of Soedirman National Defence Central Hospital starting from September up to November 2025. This research applied a descriptive quantitative study design which was employed using observation and questionnaire methods. A total of 20 samples were accordingly included, and all participants underwent the same imaging protocol, consisting of axial T1 dual-echo and IDEAL IQ sequences during upper abdominal MRI examinations. The agreement in fatty liver severity assessment between the two sequences was subsequently analysed using Cohen’s Kappa statistical test. Results: Using the axial T1 dual-echo sequence as a qualitative method and the IDEAL-IQ sequence as a quantitative method, fatty liver assessment was obtained across all severity categories. Conclusions: This research demonstrates that there is a significant agreement between the assessment of fatty liver severity using the axial T1 dual- echo sequence and the IDEAL IQ sequence. Therefore, it can be concluded that the both methods produce highly consistent and reliable evaluations and are justifiable for clinical use.

Abstract

Background: Magnetic Resonance Elastography (MRE) is a non-invasive liver imaging technique used to assess tissue stiffness by measuring liver stiffness values expressed in kilopascals (kPa). In relation to which, this research was undertaken to describe the technical aspects of hepatic MRE examination and to analyse the distribution of fibrosis stages based on kPa values obtained from the upper, middle, and lower regions of the liver. Methods: This research employed a qualitative method with descriptive design. Risearch under taken at Radiology Medistra Hospital Jakarta. The data collection methods employed were observation, interviews, and document review. The study participants consisted of two radiographers and one radiologist, each with more than three years of experience in MRE examinations. Results: ROI measurement results for the liver are clearly displayed, including the conversion values ​​to stiffness (Table 1). The MRE technique produces a color-coded elastogram designed to assist physicians in their interpretation (Table 4). The contouring confidence map method was shown to improve the reliability of ROI selection by ensuring that measurement areas were located within regions of good elastogram quality. This approach yielded reliable mean liver stiffness values, which were subsequently classified into fibrosis stages and analysed as the percentages. Conclusions: Magnetic Resonance Elastography (MRE) using the contouring confidence map method provides accurate and representative evaluation of hepatic tissue stiffness.

Abstract

Background: Primary breast lymphoma (PBL) is a rare malignancy, accounting for approximately 0.4% of all breast neoplasms. Clinically and radiologically, PBL may mimic other breast tumors, particularly breast carcinoma or phyllodes tumors. This study aims to report a rare case of giant unilateral primary breast lymphoma that initially mimicked a phyllodes tumour on imaging examination. Methods: This study was conducted as a case report of a 53-year-old woman presenting with a rapidly enlarging breast mass. Clinical examination and breast ultrasonography were performed to evaluate the characteristics of the lesion. Ultrasound imaging assessed the size, internal structure, and associated lymph node involvement. A definitive diagnosis was established through histopathological and immunohistochemical examinations. The diagnosis of PBL was confirmed based on established criteria, including the absence of prior extramammary lymphoma and no evidence of systemic disease. Results: The patient presented with a large, rapidly growing, and painful mass in the right breast. Ultrasonography demonstrated a diffuse, inhomogeneous solid mass without spiculations, architectural distortion, or microcalcifications, and no evidence of axillary lymph node enlargement. Based on these findings, the lesion was initially suspected to be a phyllodes tumour. However, histopathological and immunohistochemical analyses confirmed B-cell Non-Hodgkin lymphoma. The tumor exhibited several imaging features similar to phyllodes tumour, including giant size, unilateral presentation, and a diffuse solid structure without spiculation, calcification, or axillary lymphadenopathy. Nevertheless, hypervascularity and the absence of a cleft cyst suggested lymphoma. Conclusions: Primary breast lymphoma should be considered in the differential diagnosis of large breast masses, particularly when imaging findings show hypervascular lesions without spiculation, calcification, or axillary lymph node involvement. Accurate diagnosis requires histopathological confirmation to differentiate PBL from other breast tumors such as phyllodes tumour and breast carcinoma.

Abstract

Background:Gallbladder ultrasonography is highly influenced by fasting status. Accordingly, numerous studies have demonstrated that adequate fasting plays a crucial role in optimizing ultrasound image quality and visualization. Some radiology centers require patients to fast for 6 to 8 hours prior to an ultrasound examination. Nevertheless, excessively prolonged fasting may lead to patient discomfort, increase the risk of hypoglycaemia in diabetic patients, and disrupt workflow efficiency within hospital radiology services. The aim of this study is to obtain high-quality ultrasound images by comparing these two fasting strategies. Methods: This research employed a qualitative method with an analytical observational approach. The research was conducted at Health Polytechnic of Ministry of Health Jakarta II over a two-month period, from December 2025 to January 2026. A total of 12 samples were purposely selected, comprising examinations performed after 6-hour fasting and 8-hour fasting. Results: Gallbladder ultrasonography after 6-hour fasting demonstrated generally adequate sonographic patterns, with normal wall thickness and no evidence of gallstones or sludge, although some cases showed less optimal distension. In contrast, 8-hour fasting provided greater gallbladder distension and clearer lumen visualization, facilitating more optimal structural assessment. Overall, both fasting durations produced diagnostically acceptable images, with 8-hour fasting yielding superior visualization, while 6-hour fasting remained sufficiently reliable and more efficient. Conclusions: the findings of this research indicate that a 6-hour fasting duration is both evidently more effective and efficient for gallbladder ultrasonography. This duration was shown to produce sonographic patterns comparable to those obtained after 8 hours of fasting, without compromising diagnostic quality. Moreover, 6-hour fasting period offers additional advantages in terms of the patient comfort and improved efficiency of radiology services.

Abstract

Background: : Neurodegenerative diseases such as dementia are characterized by progressive atrophy of the hippocampal structure. Accurate evaluation of brain morphometry is crucial for early diagnosis and monitoring disease progression. Currently, atrophy assessment is performed using the manual Medial Temporal Atrophy (MTA) Score, but this is subjective and depends on the assessor's experience. Advances in artificial intelligence (AI) technology, such as the AI-Rad Companion (AIRC) Brain MR, enable more objective, automated volumetric measurements. This study to analyze the level of correlation between the brain atrophy measurements using MTA Score method and the results of automatic volumetric measurements using the AIRC Brain MR program, as well as to assess the validity of the use of AI in brain morphometry in patients with dementia. Methods: This research was quasi-experimental. The sample was 32 dementia patients who underwent T1-W MPRAGE brain MRI examination at RSPON Dr. Mahardjono. MRI images were assessed visually using the MTA Score (0–4) and measured automatically with AIRC Brain MR. Data were analyzed using the Spearman Rank correlation test to determine the relationship between the MTA score and AI-derived hippocampal volume. Results: The results showed an average MTA score of 3 on both sides, indicating moderate to severe atrophy. The average hippocampal volume measured by AIRC was 3.0 ml (left) and 2.8 ml (right). The Spearman Rank test showed a very strong negative correlation between the MTA score and hippocampal volume (r = −0.898; p-value = 0.000). This means that the more severe the brain atrophy according to visual assessment, the smaller the hippocampal volume calculated automatically by AI. Conclusions: There was a very strong negative correlation between the manual MTA Score method and the automated volumetric measurements of AI-Rad Companion Brain MR in detecting brain atrophy in dementia patients. These results suggest that AIRC can be used as an objective validation tool in the evaluation of brain morphometry in neurodegenerative diseases. The use of AI technology is expected to improve the efficiency, accuracy, and consistency of brain atrophy diagnosis in MRI examinations.

Abstract

Background: Hernia Nucleus Pulposus (HNP) is commonly evaluated using lumbar MRI, with sagittal T2 TSE sequences serving as a standard imaging protocol. However, reducing scan time with GRAPPA parallel imaging may decrease image quality. Artificial Intelligence (AI) Deep Resolved has been developed to improve image resolution and signal quality without increasing acquisition time. Therefore, this study evaluated the effect of AI Deep Resolved combined with GRAPPA on the diagnostic image quality of lumbar MRI in HNP cases. Methods: This quantitative experimental study compared the image quality of Lumbar MRI T2 TSE sagittal sequences acquired using conventional GRAPPA and GRAPPA combined with AI Deep Resolved in detecting HNP. Sixteen patients at RS Mardi Rahayu Kudus underwent both imaging protocols using identical parameters. Two radiologists evaluated the images using a 3-point scoring system. Data were analyzed using Cohen’s Kappa test for inter-observer agreement and the Wilcoxon test to compare image quality between the two techniques. Results: The results showed that GRAPPA combined with AI Deep Resolved produced significantly better diagnostic image quality than conventional GRAPPA alone (p < 0.05). All evaluated anatomical structures, including HNP, demonstrated higher image quality scores with AI assistance. The mean rank score was higher for the AI-assisted technique (91.50) than for conventional GRAPPA (0.00), indicating superior image quality. Scan time remained unchanged (1 minute 24 seconds), and inter-observer agreement was excellent (Cohen’s Kappa = 0.886). Conclusions: The integration of AI Deep Resolved with the GRAPPA parallel imaging technique improves the quality of Lumbar MRI T2 TSE sagittal images by providing clearer visualization of anatomical structures and HNP lesions. This method produces better and more detailed images than conventional GRAPPA without increasing scan time, thereby supporting more accurate diagnosis.
