Abstract

Background: tissue integrity disorders are the most common symptoms found and complained about by people with Diabetes Mellitus; this occurs due to macroangiopathy that can trigger vascular insufficiency and nerve damage so that if there is pressure/friction, it is easier to cause open wounds on the surface of the skin The purpose of this case study is to find out more specifically significant minor signs and symptoms and interventions based on SDKI and SIKI. Method: This study is a study that uses descriptive analysis from secondary data from the Case Report of the D3 Nursing Study Program, University of Jember, Lumajang Campus, for the period 2018 – 2021. Univariate data analysis was performed to assess major and minor data and establish diagnoses based on SDKI and SIKI. Results: There were a total of 29 Diabetes Mellitus patients. There are major signs of 100% tissue/layer damage to the skin, and two minor signs, namely 79% pain and 72% redness. Eight interventions are often carried out. Namely 100% wound care, 48% health education, collaboration with other medical personnel 41%, infection protection 27%, other activities 20%, and there are additional interventions, namely modern dressing, 27%, and metronidazole compresses 24%. Conclusion: Enforcement of nursing diagnoses and interventions of skin/tissue integrity disorders will be easier to do with the support of the results of clinical evidence-based practices related to major and minor signs and the most effective, efficient and successful interventions based on SDKI and SIKI.

Abstract

Background: The trend of hypertension is increasing every year. It is estimated that 1.13 billion people worldwide have hypertension 2/3 of low-middle income countries. Hypertension is the highest comorbid group in COVID-19 patients, which is 50.5%. This causes an increased risk of dangerous infections that can lead to death. The fact that the SARS-Cov-2 virus uses the Angiotensin Converting Enzym-2 receptor as an entry point to colonize in the body can cause vasoconstriction and inflammation that can worsen the incidence of hypertension and cardiovascular disease. Purpose: This study aims to determine the characteristics of hypertensive patients with confirmed COVID-19 in the inpatient installation of Panembahan Hospital, Senopati, Bantul in 2020. Methods: This study is a descriptive observational study using secondary data in the form of medical records conducted at Panembahan Hospital, Senopati, Bantul. A total of 21 medical record samples were taken that met the inclusion criteria. Samples were taken from December to October 2020 using the total sampling technique. Results: The results of data analysis using the SPSS program obtained a total of 21 samples that met the criteria. There were 10 male patients (47.6%), female 11 patients (52.4%), age >54 years totaling 15 patients (71.4%). The dominant symptoms felt were cough, fever, headache, shortness of breath, malaise, abdominal pain and sore throat totaling 19 patients (90.4%). Conclusion: COVID-19 disease with the most comorbidities is Hypertension, Hypertension with Diabetes Mellitus, followed by Hypertension with Chronic Kidney Failure. The therapy patterns that are often used in Panembahan Hospital, Senopati, Bantul for hypertension patients with Covid-19 are Calcium Channel Blocker (Amlodipine), Angiotensin Receptor Blocker (Valsartan and Candesartan), and Loop Diuretics (Furosemide and Spironolactone).

Abstract

Background: Complications of Hypertension due to hypertension can be suppressed with an appropriate, effective, and efficient nursing process approach, by knowing more specifically the signs and interventions. However, research on this matter is still very limited. Objectives: Identify signs and symptoms of major/minor acute pain nursing problems in hypertensive patients and identify the implementation of independent nursing actions to overcome these problems. Methods: This study uses a descriptive research design with a secondary data analysis approach sourced from nursing care documentation of 20 respondents in project reports of the D3 Nursing Study Program UNEJ for the period 2018 to 2020. Data on major/minor signs and symptoms, as well as the implementation of actions independent nursing care (based on SDKI and SIKI) identified, are presented in the distribution table for further discussion. Results: There are 6 major and minor signs of acute pain problems according to the SDKI that most often appear, namely complaining of pain, grimacing, restlessness, unable to complete activities, increased blood pressure, and changes in appetite and there are also 9 interventions that most often appear, namely: Identification Location, duration, frequency, quality, the intensity of pain Identify pain scales Identify knowledge and beliefs about pain Provide non-pharmacological techniques to reduce pain Facilitate rest and sleep Explain causes, periods and triggers of pain Explain strategies pain reliever, Teach non-pharmacological techniques to reduce pain. Conclusion: Knowing the most dominant symptoms and signs, complaints of increased pain, and blood pressure can be done with the right intervention, effective and efficient.

Abstract

Background: Diabetes is a non-communicable disease (NCD) and requires adherence to self-care to maintain health and prevent the disease from worsening. Several factors affect self-care in diabetic patients, including uncertainty in illness. Uncertainty is a major source of stress in patients with chronic diseases and also reduces patient compliance with self-care behavior. This study aims to determine the correlation between uncertainty in illness and self-care in type 2 diabetes mellitus patients at the Internal Medicine Polyclinic, Bandung City Hospital. Methods: This study aims to determine the correlation between uncertainty in illness and self-care in type 2 diabetes mellitus patients at the Internal Medicine Polyclinic, Bandung City Hospital. Result: This research results show a high level of uncertainty of 56.63%, a self-care level is good and a poor of 50%, p-value = 0.033 (α<0.05). Conclusion: There is a relationship between uncertainty in illness and self-care in type 2 diabetes mellitus patients at the Internal Medicine Polyclinic at the Bandung City Hospital. This study recommends health workers improve self-care for patients with management uncertainty for type 2 diabetes mellitus patients.

Abstract

Diabetic neuropathy is a heterogeneous entity, consisting of peripheral sensorimotor and autonomic nerve dysfunction conditions. Diabetic peripheral neuropathy has a broad impact on patients, including recurrent infections, ulcers that do not heal and amputations of fingers and toes. This study aims to be able to overcome these risks, screening for neuropathy can be carried out including autonomic, sensory, and motor examinations. The research method uses a cross-sectional exploratory quantitative descriptive. The sample is 100 people using probability sampling method, namely multistage random sampling. The research locations are in 5 working areas of the Jember District Health Center (Bangsalsari, Umbulsari, Kalisat, Arjasa, and Patrang). Inclusion criteria for Type 2 Diabetes Mellitus (T2DM) patients aged 18 - 65 years, diagnosed with T2DM, did not have diabetic ulcers, did not have amputation of both legs, and suffered > 1 year. The exclusion criteria were no hospitalization and no communication disorders. The research instrument used the Michigan Diabetic Neuropathy Score (MDNS), Michigan Neuropthy Screening Instrument (MNSI), screening with the Ipswich Touch Test (IpTT), screening with the Monofilament Test.T2DM respondents experienced multiple autonomic damage to the right leg (52%; 53%). Sensory damage in T2DM patients on the right and left legs showed normal results (63%). Motor damage is dominated by normal conditions on the right and left legs as much as (49%; 52%) and there is a decrease in muscle strength on the right and left (50%; 47%).Based on the results, there is still a moderate risk of T2DM patients experiencing peripheral neuropathy. The provision of nursing care in screening early conditions is an action to reduce risk and improve the patient's quality of life.
