Abstract

Background: Movement disorders following posterior fossa tumor resection are rare complications. Case Report: A 25-year-old male with obstructive hydrocephalus secondary to a fourth ventricular mass initially underwent ventriculoperitoneal shunting. Three months later, he presented with impaired consciousness and focal impaired awareness seizures manifesting as jerking in the left arm. Brain MRI showed a solid cystic extra-axial mass within the fourth ventricle, compressing the pons and cerebellum. He subsequently underwent a suboccipital decompressive craniectomy with transvermian tumor removal. Histopathology confirmed a Desmoplastic/Nodular Medulloblastoma, CNS WHO Grade 4. The post-operative course was complicated by persistent involuntary movements of the left shoulder that were initially considered refractory focal seizures and did not respond to oral antiseizure medications. Discussion: Detailed clinical evaluation revealed that the abnormal movements represented a mixed movement disorder rather than epileptic seizures. The movement in the left shoulder was consistent with Holmes tremor, a rhythmic tremor with a frequency of approximately 6 to 8 Hz, suggesting involvement of the red nucleus and nigrostriatal pathways. In addition, the left arm showed cerebellar intention tremor with dysmetria, which had previously been interpreted as seizure related jerking. This finding indicated disruption of the cerebello thalamo cortical pathway. These hyperkinetic movements improved after treatment with propranolol and topiramate alongside levetiracetam. Conclusion: This case illustrates a rare presentation of mixed Holmes and cerebellar tremors after medulloblastoma resection. Careful clinical evaluation is essential to differentiate these movements from seizures and guide appropriate treatment.

Abstract

Background: Background: Systems in healthcare have become more complicated. Advanced leadership skills need to include the cognitive, emotional, and social aspects. An emerging cross-disciplinary field, neuroleadership, combines neuroscience and leadership, but its use in hospitals is still sparse and further underexplored. Objective: The purpose of this paper is to provide a systematic overview to identify the mechanisms, applications, and research gaps of neuroleadership in hospitals. Methods: A systematic review of the literature was conducted following the PRISMA 2020 guidelines. Searches were conducted in PubMed, Scopus, ProQuest, EBSCO, and ScienceDirect by using a set of predetermined keywords. Studies that were eligible and published in the last 10 years were screened, and the methodological quality was assessed using the Joanna Briggs Institute (JBI) critical appraisal tool. Results: Nine studies were included in the final review. Findings showed that neuroleadership improves leadership effectiveness via emotional control, cognitive mobility, and social impact. The SCARF (Status, Certainty, Autonomy, Relatedness, Fairness) model was the most used framework in addressing interpersonal relations, engagement of employees, and change in the organization. Neuroleadership is also viewed to enhance transformational leadership by the neurocognitive rationale of motivation and judgment. However, there is a preponderance of conceptual studies over empirical studies. Conclusion: Neuroleadership is a highly encouraging viable alternative to current leadership frameworks in the field of healthcare. This review articulates an integrated framework that connects neurocognitive mechanisms to leadership in hospitals. Forthcoming studies should more empirical evidence with better study design and more measurable outcomes in an organization.
