Abstract

Abstrak. Coronavirus Disease 2019 (COVID-19) telah menyebabkan masalah kesehatan di seluruh dunia dengan morbiditas dan mortalitas yang terus meningkat. Pada beberapa kasus COVID-19 diketahui terjadi manifestasi trombotik. Adanya kelainan trombotik ini tentunya mempengaruhi tatalaksana dan prognosis pasien COVID-19. Oleh karena itu penting untuk mengetahui patofisiologi dan manajemen trombosis pada COVID-19. Tujuan dari artikel ini adalah mengulas literatur terbaru terkait prevalensi, patofisiologi, penilaian risiko, serta manajemen trombosis pada COVID-19.Kata kunci: COVID-19, trombosis.Abstract. Coronavirus Disease 2019 (COVID-19) has become a global health problem with ever increasing morbidity and mortality. In some cases, thrombotic manifestation has been reported in COVID-19 patients. This thrombotic manifestation certainly affects the management and prognosis of COVID-19 patients. Therefore it is crucial to understand the pathophysiology and management of thrombosis in COVID-19. The aim of this article is to review the recent literature regarding prevalence, pathophysiology, risk assessment, and management of thrombosis in COVID-19.Keywords:COVID-19, thrombosis

Abstract

Background: Learning environment is one parameter to evaluate the quality of learning. The learning approach reflects how a student learns in various situations that affect learning. This study aims to know the relationship between the perceptions of the learning environment and the learning approaches in the clerkship students of Medical Faculty.Methods: This research used an analytical observational method with cross-sectional technique. The sample selection technique used cluster sampling. Respondents consisted of 178 students from all clinical rotations. Each respondent was given two questionnaires to assess perceptions of the learning environment and learning approaches. Data were analyzed using contingency coefficient test.Results: Student perceptions of the learning environment show similar results between the number of clinical rotations that have the category of 'good but need improvement' (9 clinical rotations) and the 'many problems' category (9 clinical rotations). Most of the students (84.27%) used the in-depth learning approach, the remaining 10.67% used the strategic approach, and 5.06% used the surface approach. There was relationship between learning environment perception and learning approach with contingency coefficient value C = 0.312, and value p = 0,001 (p <0.05).Conclusion: There is a weak relationship between perception of clinical learning environments and learning approach on clerkship students of the medical faculty, University Sebelas Maret..

Abstract

Introduction. The neutrophil-to-lymphocyte ratio (NLR) is a simple parameter to assess the inflammatory status, especially in chronic kidney disease (CKD) patients experiencing capillary damage due to increased glomerular capillary pressure. In a state of vascular endothelial inflammation, there is a higher tendency to form atherosclerotic plaques and calcification that can induce cardiovascular events such as heart failure, coronary heart disease, and stroke. This study aimed to determine the relationship between the NLR and cardiovascular complications in CKD patients undergoing hemodialysis. Methods. An analytical observational study with cross-sectional approach was conducted among 119 CKD patients undergoing hemodialysis at Dr Moewardi Regional Hospital who met the inclusion and exclusion criteria. The relationship between the two variables was analyzed using Fisher exact test. Results. The majority of subjects in this study were male (51.3%) and were aged over 50 years old (63.9%). Most subjects experienced complications of heart failure (54.6%), followed by coronary heart disease (19.3%), and stroke (17.7%). Analysis of complete blood count data showed a mean neutrophil count of 77.20% (SD 11.49), lymphocytes 13.64% (SD 8.23), and NLR 10.7 (SD 10.3). Notably, a majority of participants exhibited an NLR value ≥3.5, observed in 95 individuals (79.8%). Fisher exact test results indicated a significant relationship between elevated NLR and cardiovascular complications in hemodialysis CKD patients (p-value= 0.028). Conclusion. There is a significant relationship between NLR and cardiovascular complications in CKD patients undergoing hemodialysis.

Abstract

Human Immunodeficiency Virus (HIV) infection remains a global health concern characterized by the reduction of CD4 lymphocyte cells and weakened immune systems. Knowing the CD4 count and the factors affecting it is crucial for assessing the immune status of HIV patients. Hematological markers, including neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR), have been recognized as prognostic tools that were associated with CD4 Count. The goal of this study was to determine the impact of NLR and PLR on CD4 count among people living with HIV (PLHIV). This study used analytic observational method with a cross-sectional on HIV-positive individuals who were treated at Moewardi Hospital, Surakarta, Indonesia. The Chi-Square and Pearson correlation tests were performed to identify the correlation between variables and the linear regression test was done to investigate the association between NLR and PLR with CD4 count. A total of 80 PLHIV were identified for this study, with the median CD4 count of 103 cells/mm3. NLR and PLR were found to be 3.06 and 181.03, respectively. This study found that opportunistic infection, duration on ARV treatment, body mass index, total lymphocyte count, and hemoglobin were significantly associated with CD4 count. The Pearson correlation test revealed a strong correlation between NLR and PLR to CD4 count. Linear regression analyses showed that NLR and PLR could predict the CD4 count. These findings indicate that NLR and PLR could serve as alternative prognostic parameters for monitoring treatment outcomes in PLHIV, particularly in health facilities where access to CD4 count testing is limited.
