Abstract

Digital transformation in the healthcare sector has become an urgent priority to improve service quality and operational efficiency. A key component of this transformation is the implementation of Electronic Medical Records (EMRs), as mandated by the Indonesian Minister of Health Regulation No. 24 of 2022 and supported by the 2024 Health Digital Transformation Blueprint. Clinic X began implementing EMRs in late 2023 but continues to face challenges, including limited system features, synchronization issues with the Satu Sehat platform, and server disruptions. This study evaluated the clinic’s digital maturity using the Digital Maturity Index (DMI) to formulate strategic recommendations. A mixed-methods approach with a concurrent triangulation strategy was employed through observations, in-depth interviews, and questionnaires based on the official DMI instrument developed by the Indonesian Ministry of Health. The results showed that Clinic X achieved an average maturity level of 3 in Primary Care Readiness, Information System Capability and Infrastructure, and Data Security. The clinic demonstrated strengths in staff digital literacy and infrastructure but weaknesses in governance guidelines and EMR feature development. Overall, the clinic is at the developing stage, indicating the need to strengthen system integration, infrastructure, and information governance to accelerate digital transformation.

Abstract

Background: The Waruroyom Community Health Center has implemented the E-Puskesmas application as an information system to support recording, service data management, and health reporting. Initial observations revealed constraints such as network disruptions, suboptimal data input processes, and differences in users' ability to operate the system, which affected health workers' satisfaction. Methods: This research used a quantitative descriptive approach with a population of 47 health workers at Waruroyom Community Health Center, all of whom were included as samples (total sampling). Data were collected using a Likert scale questionnaire based on the six dimensions of the PIECES method. Results: All six PIECES dimensions fell within the satisfied category, with mean scores ranging from 3.83 to 4.10. The Information dimension obtained the highest mean (4.10), while the Performance dimension obtained the lowest mean (3.83). The highest satisfaction indicator across all dimensions was information accuracy (89.3%), while the lowest was the indicator of systems rarely experiencing errors (44.6%). Discussion: The strength of E-Puskesmas lies in its information quality, while technical stability remains a primary challenge. Conclusions: Health workers at Waruroyom Community Health Center are satisfied with E-Puskesmas in all six PIECES dimensions. Improvements are needed particularly in the Performance dimension, especially in minimizing system errors and strengthening technical support services.

Abstract

Pending BPJS Kesehatan claims remain a significant obstacle to hospital financing as they can disrupt cash flow and service continuity. This study aims to analyze the factors causing pending BPJS Kesehatan claims for inpatient services at Waled Regional Hospital in the third quarter of 2025. The study used a descriptive quantitative design with a retrospective cross-sectional approach. The population and sample consisted of 908 pending claim files using a total sampling technique. The research instrument was a checklist, while data analysis was conducted descriptively using frequency distribution and percentages. The results showed that the most common cause of pending claims was the coding aspect at 41.9%, followed by the medical aspect at 35.0% and the administrative aspect at 23.1%. In the administrative aspect, the most dominant problem was the mismatch of care classes. In the medical aspect, the main cause was neonatal asphyxia that did not meet the TKMKB criteria. In the coding aspect, the most common problem was inaccurate diagnosis codes or those that needed to be adjusted to clinical data based on verification BPJS. In conclusion, pending claims at Waled Regional Hospital are multidimensional and interrelated, thus requiring strengthening clinical documentation, coding accuracy, and integrated administrative verification.

Abstract

Abstract

The efficiency of inpatient care, particularly in bed utilization, is a key factor in improving the quality of hospital services. Efficiency is measured using the Barber Johnson method, which includes four main parameters: BOR (Bed Occupancy Rate), AVLOS (Average Length of Stay), TOI (Turn Over Interval), and BTO (Bed Turn Over). Based on medical records from Sumber Waras Hospital in 2025, inpatient care indicators show results that do not yet meet the ideal standards set by the Ministry of Health, where the AVLOS value of 2,8 days and the BOR of 55.35% remain below the standard, while the BTO value of 70,81 times actually exceeds the ideal limit. This study aims to describe the efficiency of inpatient services at Sumber Waras Hospital for the 2022–2026 period using the Barber-Johnson method. Methods: This study employs a quantitative method with a descriptive design and utilizes secondary data from inpatient medical records for the 2022–2026 period. Results: The BOR for 2022–2026 ranges from 51.95% to 55.61%, AvLOS ranges from 2.67 to 3.16 days, TOI ranges from 2.30 to 2.77 days, and BTO ranges from 18.06 to 70.81 times. Conclusion: The utilization of inpatient beds at Sumber Waras Hospital for the 2022–2026 period is not yet efficient. Of the four indicators, only the TOI meets the ideal standard, while the BOR, AvLOS, and BTO all fall short of the standard. The Barber-Johnson chart shows that the intersection points for 2022–2026 all lie outside the efficiency zone.
