Abstract

Seizures are a serious complication of tetanus, contributing to high morbidity and mortality rates. Various therapeutic approaches have been employed to manage seizures, including diazepam and magnesium sulfate. This literature review aims to compare the effectiveness of these two agents in reducing both the frequency and severity of seizures in tetanus patients. A systematic search was conducted using scientific publications from PubMed, Google Scholar, and ScienceDirect over the past ten years. The review indicates that magnesium sulfate shows potential as an effective and relatively safe alternative to diazepam in certain clinical settings. However, variations in study design and limited available data highlight the need for further clinical trials to strengthen existing evidence. This review is expected to serve as a reference for clinicians in selecting optimal anticonvulsant therapy for tetanus cases. Furthermore, the study emphasizes the importance of understanding the pharmacological mechanisms, clinical indications, and monitoring of adverse effects to ensure both safety and efficacy of the therapy. Overall, this literature review contributes to the development of evidence-based clinical guidelines for managing seizures in tetanus patients.

Abstract

Dysphagia in extremely elderly patients represents a significant clinical challenge because it is frequently associated with frailty, malnutrition, and an increased risk of aspiration. Multifactorial etiologies and atypical clinical manifestations may complicate diagnosis and delay appropriate management. This study aimed to describe the diagnostic evaluation and clinical findings of dysphagia in an extremely elderly patient through a case report supported by a scoping review. A 105-year-old woman with chronic progressive dysphagia underwent comprehensive assessment, including physical examination, upper gastrointestinal endoscopy, histopathological evaluation, and chest radiography. Relevant literature on dysphagia in geriatric populations was reviewed to provide contextual understanding of diagnostic challenges and underlying etiologies. The evaluation revealed chronic active gastritis and a hamartomatous polyp in the gastroesophageal region without evidence of malignancy. Comprehensive assessment successfully excluded malignant and acute obstructive causes, emphasizing the complexity of dysphagia diagnosis in extremely elderly individuals with frailty and aspiration risk. These findings underscore the importance of endoscopic and histopathological examinations in distinguishing benign from malignant etiologies and support early recognition of multifactorial non-malignant causes to optimize clinical management and reduce the risk of adverse outcomes in this vulnerable population

Abstract

Background: Multiple facial fractures are complex injuries commonly caused by high-energy trauma, particularly traffic accidents. These injuries may involve multiple facial skeletal structures, resulting in impaired mastication, occlusion, respiration, vision, and facial aesthetics. Appropriate management requires anatomical reconstruction based on the principles of open reduction and internal fixation (ORIF), with an appropriate fixation sequence to achieve optimal functional and cosmetic outcomes. Case Report: A 20-year-old male presented to the emergency department following a traffic accident, complaining of severe facial pain accompanied by bilateral epistaxis. Physical examination revealed edema, deformity, crepitus, and tenderness in the nasal, naso-orbito-ethmoid (NOE), zygomatic, bilateral maxillary, and mandibular regions. Laboratory examination revealed anemia due to acute blood loss, leukocytosis with neutrophilia, an increased neutrophil-to-lymphocyte ratio (NLR), and abnormalities in coagulation parameters. The patient was diagnosed with multiple facial fractures involving the nasal bone, NOE complex, left zygomatic bone, bilateral maxilla, and mandibular symphysis. ORIF was performed using a reconstruction sequence beginning with the mandible to restore occlusion, followed by reconstruction of the zygomaticomaxillary, nasomaxillary, and NOE buttresses using plates and screws. Discussion: Beginning reconstruction with the mandible provides a stable occlusal reference, thereby facilitating the repositioning of other facial structures. This approach is consistent with the principles of facial buttress reconstruction, which aim to restore bony continuity, masticatory function, facial stability, and aesthetic outcomes. Changes in laboratory parameters reflected the post-traumatic inflammatory response and the process of erythrocyte regeneration following blood loss. Conclusion: The management of multiple facial fractures requires comprehensive clinical evaluation, adequate supporting examinations, and reconstruction based on the principles of facial buttress restoration. ORIF performed with an appropriate fixation sequence can optimally restore facial function and anatomy while reducing the risk of long-term complications.

Abstract

Angina ludwig adalah bentuk selulitis difus, akut, dan menyebar cepat. Kematian tersering akibat gangguan jalan napas, sampai dengan 50% pada pasien tidak diobati, dan 8% yang menerima pengobatan yang memadai. Diagnosis dini dan perencanaan perawatan segera menjadi prosedur yang menyelamatkan jiwa. Tujuan: memprediksi level keparahan dan progresivitas penyakit melalui analisis keluhan dan tanda untuk penatalaksanaan yang tepat. Laporan kasus: kami laporkan satu kasus laki laki 27 tahun, keluhan nyeri tenggorok, bengkak leher, trismus, stridor dan gangguan pernapasan dengan diagnosis angina ludwig. Metode: analisa mekanisme gejala dan tanda pasien berdasarkan tinjauan pustaka, menentukan level keberatan dan progresivitas penyakit. Hasil dari analisa sebagai pedoman penatalaksanaannya, yaitu tentang penanganan gangguan pernapasan, pemberian antibiotik dan antiperadangan, incisi drainase, dan ekstraksi gigi. Hasil: analisa gejala dan tanda menetapkan tindakan invasif gangguan jalan napas, memberikan antibiotik untuk bakteri aerob dan anaerob, dan kortikosteroid. Kemudian incisi drainase dan ekstraksi gigi. Pasien sembuh dan dapat rawat jalan. Kesimpulan: analisa keluhan dan tanda pasien angina ludwig memberikan masukan untuk menetapkan penatalaksanaan penyakit. Keberhasilan penyelamatan jiwa pasien angina ludwig menjadi tantangan dokter dan petugas kesehatan, karena sampai saat ini standar baku penatalaksanaan angina ludwig belum ada.
