Abstract

This study aims to categorize various early mobilization therapies in heart disease patients. The research method used is a systematic review by searching for articles using the Science Direct, PubMed, and Google Scholar databases. Key phrases included "early mobilization," "exercise training," and "cardiovascular disease and heart failure." The results showed that the interventions had 6MWT, CPET, progressive mobilization I, muscle breathing/inhalation training on day I, leg and limb exercises on the second and third days, ambulation, limb exercises, sitting, and walking. Around the bed, low-intensity resistance exercises in bed, and standing and tiptoe exercises on the floor beside the bed. Patients with acute heart failure often receive 20 to 40 minutes of therapy daily, including bedside exercises and a 50-step walk. In conclusion, there are still differences in time, scope, and implementation of early mobilization initiatives. However, mobilization therapy can be given to cardiac patients of all ages and is safe and practical. They help reduce re-hospitalization risk and improve a patient's VO2 and quality of life (QOL). Abstract

Introduction: Nosocomial infections, including those caused by Methicillin-Resistant Staphylococcus aureus (MRSA), pose a significant challenge for the global health system. MRSA, a resistant bacterium that increases morbidity and mortality, is often found in healthcare facilities and is associated with inappropriate antibiotic use and suboptimal patient management practices. Methods: This research employs an analytical observational design with a cross-sectional approach. The sample consists of 70 inpatient medical records that meet the inclusion criteria, with data collected from positive MRSA microbiological cultures. Analysis was conducted using SPSS. Results: The results indicate a significant relationship between the type of laboratory examination (p=0.049), use of medical therapeutic devices (p=0.023), type of antibiotics (p=0.015), and duration of treatment (p=0.033) with MRSA incidence. Patients in the ICU and PICU had a higher prevalence, as did those receiving antibiotic treatment such as penicillin (p=0.015). A treatment duration of more than 7 days also increased the risk of MRSA infection (p=0.033). Additionally, devices such as catheters showed a significant association with infection (p=0.023). Conclusion: This study highlights the importance of strict infection surveillance and control in high-risk care units, appropriate antibiotic use, and monitoring of medical therapeutic devices. The findings can serve as a foundation for more effective infection prevention policies to minimize the risks and impacts of MRSA in healthcare facilities.

Abstract

Fall incidence among healthcare facility patients has recently become a patient safety issue. To date, instruments to measure nurses’ performance in preventing patient fall incidence have been lacking. This paper aims to report the first and the second phases of an instrument to measure the implementation of patient fall prevention by nurses working in hospitals’ inpatient units or wards. The instrument, Self-Reported Questionnaire: Implementation of Nurses in Inpatient Wards in Preventing Fall among Patients (INI-PFP), was developed using the scale development method. In the first phase, domain and item generation was identified, followed by content validity (panel of 11 experts from universities and hospitals) and readability test/ face validity of 14 inpatient nurses). The study initially generated 46 items from the instrument development process. The initial instrument was sent to 11 experts, assessing the items’ relevance, clarity, simplicity, and ambiguity. This stage reduced three items, with the remaining items’ S-CVI = 0.99 for the four categories. The 43 items were subsequently provided to 14 inpatient nurses from four hospitals in the readability test and face validity with 98,17% of the items’ clear instructions, ranging from 57%-100% of question agreement. From this stage, 13 items were re-written and brought to a second round of face validity involving eight nurses from two hospitals. The end of the face validity test considered the 43-Items INI-PFP were adequate to enter the final phase, including Construct Validity and the Psychometric Evaluation for its final validity and reliability.

Abstract

Diabetic Foot Ulcer (DFU) is one of the complications that cause big problems in diabetics. One of the management of DFU management is by utilizing hyperbaric oxygen therapy (HBOT) which is an additional therapy that has been proven to be used in the treatment of DFU. However, the results of its benefits and use are still being debated, so the purpose of this review is to evaluate the effectiveness of HBOT on healing diabetic foot wounds. The method of this research is to use a literature review design. By using three international databases, namely PubMed, Science Direct, and Ebscohost, six articles were included. The results of the review explained that from six articles conducted by the literature review, it was reported that hyperbaric oxygen therapy (HBOT) can accelerate the healing process of diabetic foot wounds (DFU), by reducing wound size, lowering levels of C-reactive protein (CRP) and white blood cells (WBC)), induces angiogenesis of EGF, VEGF, PDGF, FGF-2 and CXCL10 and changes serum interleukin 6 (IL-6) and VEGF levels to be greater. Conclusion (HBOT) is an appropriate adjunct therapy, which can provide effectiveness in the treatment of DFU by accelerating the healing of DFU and preventing further infection in DFU, so as to reduce the risk of amputation.

Abstract

Labor problems that often occur are the inability of the uterus to contract effectively, but by doing prenatal yoga and using a birth ball, the uterine muscles can be more relaxed so that the labor process becomes smoother. The study aimed to analyze the effect of prenatal yoga, birth ball and prenatal yoga with birth ball on the duration of active phase I labor. Quasi-experimental method, post-test comparative group design. Sampling using purposive sampling technique. The sample of this study amounted to 60 respondents, divided into three groups. Data were collected with observation sheets and analyzed using the Kruskal Wallis test to see the effect of independent variables, namely prenatal yoga, birth ball, and prenatal yoga with birth ball on the dependent variable of the duration of active phase I labor. The findings showed, the prenatal yoga group in the active phase I was normal 18 (90%) respondents, p-value 0.002, mean rank 90.28. Birth ball group of active phase I normal labor 18 (90%) respondents, p-value 0.000, mean rank 90.50. Prenatal yoga group with birth ball active phase I normal labor 16 (80%) respondents, p-value 0.005, mean rank 87.70.The results of the analysis of the three groups simultaneously obtained prenatal yoga r=81.916, birth ball r=93.412, prenatal yoga with birth ball r=87.714. There is difference between the three groups with p value = 0.001. In conclusion, both in groups and simultaneously, the three groups, have a significant effect on the duration of active phase I labor.
