Abstract

The case study focuses on a multi-product company in Indonesia that aims to develop a proactive manufacturing strategy to tackle production disruptions caused by process uncertainty. Data from 30 periods are analyzed to evaluate the company's weekly production plan performance, revealing consistent quantity fulfillment but significant discrepancies in the types of products produced compared to the plan. Process uncertainty, especially regarding quality, leads to disruptions such as uneven and inadequate material supplies. Statistical Process Control (SPC) tools, like Pareto analysis, are used to identify priority quality issues, forming the basis for Fault Tree Analysis (FTA) to trace root causes and determine intervention points. Each machine's limited set of valves and pipes poses challenges for thorough cleaning during downtime and quality management outside working hours. Introducing backup valves and pipes per machine can mitigate these issues and improve production efficiency. Companies can implement procurement strategies for pipes and valves to prevent dirty prints in the primary process, ensuring smooth material supply and minimizing disruptions. Although actual production plan achievement hasn't reached 100%, this strategy has the potential to increase product sales by 3.31% by reducing the rejection ratio of dirty prints. Ultimately, proactive procurement of spare pipes and valves is expected to increase production efficiency by 4%, indicating enhanced production schedule stability.

Abstract

Lean Six Sigma is a method that is quite effective in solving waste problems in the production process, but it is often not optimal because of the tools used in the DMAIC process. Therefore, we developed a proposed model by combining the Waste Assessment Model and Fuzzy-FMEA in Lean Six Sigma to reduce waste in the production process. The Waste Assessment Model was used at the defined stage to identify waste, while the Fuzzy-FMEA was used for priority waste risk analysis at the analysis stage. Based on the results of the Waste Assessment Model case study, defects were the most influential waste, with a percentage of 22.38%. At the analysis stage using Pareto diagrams, tearing was the dominant defect in the Nike Extreme. Based on calculations using the Fuzzy-FMEA method, the highest cause of defects was errors in processes or missed processes that caused the failure. Improvement was carried out at the improvement stage, and we found it necessary to carry out intensive and periodic control or supervision of the Nike Extreme production process. Based on these results, the proposed Lean Six Sigma model can be used to identify and reduce waste in the production process.

Abstract

Construction and material shops involve various high-risk activities, making Occupational Safety and Health (OSH) practices essential. This study aims to build a clear picture of existing safety practices and key risks in building material retail requiring attention through structured safety initiatives. Based on the findings, this study provides recommendations to improve OSH practices for workers in these environments. HIRA (Hazard Identification and Risk Assessment) and JSA (Job Safety Analysis) methods were employed to identify potential occupational hazards by defining the characteristics of possible hazards and evaluating associated risks. The findings reveal several unsafe acts and conditions from various working process and activity leading to potential hazards. A total of eight potential hazards were identified with HIRA uncovering one extreme risk, three very high risks, three high risks, and one medium risk. Among these potential hazards, manual handling posing the highest risk, followed by ergonomic issues, dust exposure, and contact with sharp objects where all classified as very high risks. To mitigate these risks, by using JSA, a combination of engineering controls, administrative measures, and PPE use was recommended in order to improve material handling, workstation conditions, and safety equipment usage. These recommendations serve as the foundation for implementing a proper OSH management system.

Abstract

Clean water is one of the basic needs for human life. The toughest challenge related to clean water problems is in the procurement and distribution of clean water. Instability in distribution and supply chain issues has now become one of the company's focuses of attention. The success of the company depends on the structure and alignment of the supply chain design, this can be done if the company is able to minimize the negative effects of the risk of critical assets. Understanding the concept of asset management is important. This study aims to design and implement an asset management policy model in the case of critical assets in Company X, especially in the K water house. After an analysis was carried out using the House of Risk phase 1 and the Pareto diagram, the results were 14 risk agents (Risk Agent) which are a description of 6 risk events (risk events) that are included in the vulnerable and high-risk categories. After an analysis was carried out using the House of Risk phase 2 and an assessment of the Effectiveness to Difficulty Ratio (ETD) value, 22 priority risk management strategies (PA) were obtained. One of these risk management strategies is the policy of installing a soft starter for the pump panel.

Abstract

The aviation industry, a pillar of global transportation, is under constant pressure to increase productivity and efficiency while maintaining strict quality requirements.  Airctraft defects in production can result in significant financial losses, lead to costly rework, delays, and even safety risks. This study proposes a framework to improve productivity and efficiency in aircraft manufacturing and analyze quality control using machine learning, Six Sigma, and the QCDSME (Quality-Cost-Delivery-Safety-Morale) method. The DMAIC (Define-Measure-Analyze-Improve-Control) stage is a reference in the implementation steps of the Six Sigma method of the Airbus A320. The sigma value in this study was obtained on average for 40 periods of 4.61 sigma and a DPMO of 1225.69. At the analyze stage, a fishbone diagram is used to find the root cause of the problem.  Furthermore, a machine learning analysis was performed using the text mining method to identify the most common product components that frequently have defects in Airbus A320 and identify the main factors causing defects, by the human factor.  The enhance stage suggests a rise in overcoming challenges with the QCDSME method. Overall, it was discovered that the number of defects fell while the sigma improved and this method can enhance industry performance.
