Abstract

Background: Prolonged preoperative fasting can trigger surgical stress. Preoperative oral carbohydrate loading may reduce insulin resistance, improve patient comfort, and help preserve postoperative muscle function. Objective: This study aimed to compare changes in cortisol and blood glucose levels between patients receiving oral glucose 2 hours before surgery and those receiving placebo during laparoscopic cholecystectomy. Methods: This study used a single-blind randomized clinical trial involving patients undergoing elective laparoscopic cholecystectomy at Wahidin Sudirohusodo Hospital and its network hospitals in Makassar. Results: Mean cortisol levels at T2 were lower in the glucose group than in the placebo group, at 4.51 mcg/dl and 10.19 mcg/dl, respectively (p < 0.05). Blood glucose levels at T1 and T2 were also lower in the glucose group than in the placebo group (p < 0.05). A significant decrease in cortisol levels from T0 to T2 was also observed (p < 0.05). Conclusion: Oral glucose administration 2 hours before surgery was effective in reducing cortisol levels and maintaining blood glucose stability up to 24 hours postoperatively compared with placebo.
