Abstract

Electronic Health Records (EHRs) have become an essential component of healthcare digital transformation, but their implementation may also increase healthcare workers’ workload and affect their well-being. This review aimed to systematically analyze the impact of EHR implementation on healthcare workers’ workload and well-being based on recent scientific evidence. A systematic review was conducted following the PRISMA 2020 guidelines. Literature searches were performed in PubMed, Scopus, Google Scholar, and the Cochrane Library to identify studies published between 2020 and 2025 using the PECO framework. The selection process identified 20 studies that met the predefined inclusion criteria and were subsequently synthesized using a narrative approach. The review found that EHR implementation consistently increased documentation burden, prolonged digital working hours, and elevated cognitive workload due to system complexity. Poor EHR usability was also associated with higher levels of work-related stress, burnout, emotional exhaustion, reduced job satisfaction, and impaired work–life balance among healthcare workers. The magnitude of these effects varied according to professional role, healthcare setting, and individual characteristics, including gender. Overall, EHR implementation provides important benefits for improving healthcare quality; however, it may also increase workload and reduce healthcare workers’ well-being when system usability is inadequate and digital tasks extend beyond regular working hours. Therefore, optimizing EHR design, simplifying documentation workflows, and limiting after-hours digital work are essential to support the long-term well-being of the healthcare workforce.
