Abstract

Breast cancer is the most common malignant tumor and the leading cause of death among females. Most women with breast cancer will undergo breast-conserving surgery (BCS) or lumpectomy, simple or total mastectomy, modified radical mastectomy or bilateral mastectomy. Breast reconstruction solves many of the problems. The goal is to return the patient to a near-normal state so that she is not handicapped in her daily living. Breast reconstruction is also intended to offer psychological benefits to women treated by mastectomy following a diagnosis of possibly terminal cancer. In these case reports, we reported two successful cosmetic reconstruction cases. The first case was a 39-years-old female with non-recurrent breast cancer consulted and was referred by the oncology surgeon to the Plastic Reconstructive, and Aesthetic Surgery Division, Department of Surgery, Dr. Sardjito General Hospital. She wanted to undergo breast reconstruction. We planned to operate three years following mastectomy. She was diagnosed with 3B-stage of right breast cancer before. After the surgery, the patient received six series of chemotherapy. She also underwent radiotherapy 25 times and hormonal therapy. We decided to perform a transverse rectus abdominis myocutaneous (TRAM) flap surgery on her with 22 months follow up. The procedure provided good results, and the patient was satisfied. The second case was a 32-year-old woman who after excision of a phyllodes tumor and wanted breast reconstruction. Reconstruction was performed four years after tumor removal using latissimus dorsi myocutaneous (LDM) flap. The surgery provided good results, and the patient was satisfied. We reported two successful cosmetic reconstruction cases of non-recurrent 3B-stage of right breast cancer post-mastectomy after pedicled TRAM flap surgery and left breast post excision of phyllodes tumor after LDM flap surgery.

Abstract

Pariwisata adalah fenomena kemasyarakatan yang menyangkut manusia, masyarakat, kelompok, organisasi kebudayaan. Menurut pasal 1 angka 3 Undang-Undang Republik Indonesia No 10 Tahun 2010 Tentang Kepariwisataan yang menyatakan bahwa” pariwisata adalah berbagai macam  kegiatan wisata yang didukung berbagai fasilitas serta layanan yang disediakan oleh masyarakat, pengusaha, pemerintah, dan pemerintah daerah”. Sektor partiwisata merupakan bidang industri yang cukup diandalkan oleh negara-negara di dunia terutama dalam pembahasan ini adalah di Indonesia dengan begitu banyak keberagamaan serta keindahan alam nya maupun kelautannya. Tujuan Penulisan ini adalah Untuk mengetahui bagaimana perlindungan serta pertanggungjawaban terhadap konsumen jika mengalami kecelakaan atau cidera saat mengendarai jetski, untuk mengetahui bagaimana bentuk perlindungan yang seharusnya dilakukan pihak pelaku usaha ekstrem sport yang ada di Sea-Doo Safari Balikpapan, dan untuk mengkaji dan meneliti bahwa pentingnya perlindungan wisatawan melalui pemberian asuransi sesuai dengan ketentuan Undang-Undang Kepariwisataan Nomor 10 Tahun 2009. Hasil Penelitian yang diperoleh menunjukan ini dapat memberikan gambaran Tentang bagaimana seharusnya implementasi Pasal 26 Huruf D Undang-Undang Nomor 10 Tahun 2009 Tentang Kepariwisataan, Undang-Undang Nomor 8 Tahun 1999 Tentang Perlindungan Konsumen, KUHD (Kitab Undang-Undang Hukum Dagang) dan beberapa aturan terkait seharusnya dijalankan oleh pelaku usaha agar dapat memberikan pelayanan serta keselamatan bagi kosnumennya, serta pengaturan  hukum yang dapat menegakan peraturan Undang-Undang yang sudah ada dan juga tindakan yang harus dilakukan oleh pihak terkait/pengelola serta pihak yang berwenang untuk memberikan sosialisasi atau tindakan tegas kepada para pelaku usaha yang bersangkutan dalam permasalahan yang diangkat pada penelitian ini, dan peneliti melakukan penyusunan hasil penelitian yang bersifat empiris dan normatif, yaitu menggunakan peraturan perundang-undangan yang berlaku sebagai landasan hukum, karena penelitian bertitik tolak dari peraturan - peraturan yang ada sebagai norma hukum yang berlaku di Indonesia.

Abstract

Introduction: We present a case of adult onset Radiation-Induced Meningioma (RIM) following resection of astrocytoma with short latency period of only 18 months. Case Report: A 44-year old man who had high dose irradiation following astrocytoma resection came to our institution due to the appearance of intracranial tumor in the field of radiation in the follow-up MRI. Observation was done and a head CT scan was done 6 months later, where intracranial tumor was doubled in size. Surgical resection was done and histopathological result of high-grade meningioma confirming the diagnosis of RIM. Most RIM are usually presenting in children receiving high dose radiotherapy with mean latency period of 26.2 ± 9.3 years. Result: Our patient is older at presentation; he underwent astrocytoma resection previously and was receiving high dose irradiation of 46 Gy. Conclusion: In astrocytoma patients receiving adjuvant radiotherapy, physician must be aware of RIM; in addition to the recurrence of the high-grade gliomas.

Abstract

Background: Advanced-stage lymphedema is characterized by lymphatic dysfunction and fibroadipose overgrowth. Single-modality treatments, such as lymphaticovenous bypass (LVB) or liposuction alone, are often inadequate. This case series evaluates the safety and outcomes of simultaneous LVB and lymph-sparing liposuction in advanced lower-limb disease.Method: We conducted a case series involving four patients (n=4) with secondary lower limb lymphedema (ISL stage II–III; LDB stage IV–V). All underwent simultaneous LVB and lymph-sparing liposuction. Limb volume was calculated using the truncated cone formula at baseline, 1 week, and 12 months postoperatively. Quality of life was measured using the LYMPH-Q questionnaire. One patient with bilateral lymphedema received LVB on both limbs but underwent liposuction on only one, allowing for direct internal comparison.Results: In all four patients patients demonstrated immediate postoperative limb volume reductions ranging from 12.13% to 37.68%, with further improvements at 12 months (up to 42.1%). Quality of life scores significantly increased, with improvements of up to 37.29%. In the bilateral case, the limb treated with both LVB and liposuction showed superior outcomes compared to the limb treated with LVB alone. No major complications occurred.Conclusions: Simultaneous LVB and lymph-sparing liposuction is a safe and effective treatment for advanced lymphedema. This dual-modality approach addresses both fluid accumulation and fibrotic hypertrophy, resulting in better functional and aesthetic outcomes than either technique alone. Larger studies with longer follow-up are needed to confirm these findings and optimize patient selection.
