Abstract

LATAR BELAKANG: Salah satu komplikasi makrovaskuler penderita diabetes melitus tipe 2 (DMT2) yaitu penyakit jantung koroner (PJK). Kematian utama pada DMT2 disebabkan karena PJK. Rasio trigliserida (TG)/ high density lipoprotein (HDL) dapat memprediksi perkembangan PJK dan berkorelasi dengan indeks massa tubuh (IMT) dan tingkat tekanan darah. TUJUAN: Penelitian ini bertujuan membuktikan korelasi IMT dan tekanan darah dengan rasio TG/HDL penderita DMT2 Etnis Jawa. METODE : Sampel penelitian sejumlah 107 subjek DMT2 etnis Jawa. Data sekunder berupa rekam medis pasien DMT2 di Puskesmas, Semarang. Uji korelasi Spearman dipakai untuk menganalisis korelasi IMT dan tekanan darah dengan rasio TG/HDL dengan p<0,05. HASIL: Pada DMT2 etnis Jawa ditemukan rerata IMT adalah 24,3 + 3,3 kg/m2 dengan mayoritas IMT obesitas (42,1%), rerata tekanan darah sistolik (138,4+19 mmHg) dan tekanan darah diastolik (80,5+8,9 mmHg) dengan mayoritas mengalami prehipertensi (46,7%). Terdapat korelasi positif lemah yang signifikan antara IMT terhadap rasio TG/HDL pada penderita DMT2 etnis Jawa (r= 0,262, p= 0,006). Tekanan darah tidak terdapat korelasi signifikan terhadap rasio TG/HDL pada penderita DMT2 etnis Jawa (r= -0,114, p=0,243,). KESIMPULAN: Terdapat korelasi signifikan antara indeks massa tubuh (IMT) dengan rasio TG/HDL pada penderita DMT2 etnis Jawa. Sedangkan tingkat tekanan darah tidak berkontribusi terhadap rasio TG/HDL pada penderita DMT2 etnis Jawa. KATA KUNCI: Indeks massa tubuh, tekanan darah, rasio TG/HDL, DMT2

Abstract

Background: Obesity can affect severity of knee osteoarthritis sufferers. Several studies have examined relationship between obesity and osteoarthritis, but no one has examined the different types of obesity on osteoarthritis, therefore, researchers wanted to know differences of central and peripheral obesity on severity osteoarthritis.Methods: A retrospective studi, with cross-sectional, simple random sampling method, conducted between 1 August 2014 and 30 September 2014 in Semarang City. Samples people > 50 years old. Data were analyzed by rank Spearman and Anova correlation.Results: In this study, 45.7% (n = 32) reported severe osteoarthritis. The results of the statistical test obtained p1 = 0,000 (reject Ho). The correlation coefficient (r) is 0.857 (strong), and the linear pattern is positive. The coefficient of determination (r2) is obtained (0.857) 2 = 0.73 = 73%. And 37.1% (N = 26) reported being obese. p2 = 0.043 (reject Ho). The mean deviation (MD) was 0.048 (not significant) between central obesity and peripheral obesity.Conclusion: Obesity affects severity of knee osteoarthritis. The greater body mass index, greater severity of knee osteoarthritis. 73% of severity of knee osteoarthritis is influenced by obesity, but there is no significant effect between central and peripheral obesity on the occurrence of knee osteoarthritis.

Abstract

ABSTRACT Treatment and management of blood clotting disorders in Covid-19 patients when they have just been admitted to the hospital must be considered even if no bad manifestations are found due to increased D-dimer levels. Initial indications obtained from the hemostasis laboratory, D-dimer levels are very important to use to calculate the degree of risk and survival of Covid-19 patients. Therefore, this research was carried out with the aim of assessing the relationship between D-dimer levels and Neutrophil lymphocyte ratio with the severity level in Covid-19 patients at Tugurejo Hospital, Semarang. The type of research used in this research is analytical observational with a cross sectional design. This approach was carried out to determine the relationship between D-dimer levels and Neutrophil lymphocyte ratio with the severity of Covid-19 patients at Tugurejo Hospital, Semarang, Central Java. The majority of Covid-19 patients at Tugurejo Hospital Semarang had increased D-Dimer levels, which could indicate a potential disturbance in the blood clotting system. the majority of patients, 95.5%, showed a risky NLR (≥3.3). Most of the patients treated at Tugurejo Hospital Semarang experienced severe levels of severity. There is a significant pattern between the severity of Covid-19 patients and D-Dimer levels, where higher levels of severity tend to be associated with higher D-Dimer levels. There is a significant relationship between NLR and patient severity, with a significance value of 0.000. Research findings show that the majority of Covid-19 patients at Tugurejo Hospital Semarang experienced increased D-Dimer levels, reaching 93.2%. Elevated NLR reflects high levels of inflammation and an active immune system response. serious impact of this disease on the hospital. There is a significant relationship between D-Dimer levels and the severity of Covid-19 patients at Tugurejo Hospital Semarang with a significance of 0.000 or P<0.05 and there is a significant relationship between NLR and the severity of Covid-19 patients with a significance of 0.000 or P< 0.05

Abstract

Physical activity is any body movement caused by the work of skeletal muscles and increases the expenditure of energy and energy. Central obesity is the accumulation of excessive fat that can interfere with health. One of the chronic complications of diabetes mellitus, namely the problem of lesions on the feet called diabetik ulcers is a condition where open wounds are found on the skin surface on the feet in DM patients due to nerve abnormalities and peripheral arterial vascular disorders which can be caused by lack of physical activity and obesity. The purpose of this study was to analyze the relationship between physical activity and central obesity with the incidence of diabetik ulcer complications in type 2 DM patients. Analytical observational research with a case control approach. The research subjects were type 2 DM patients at the Rowosari II Health Center. The research instrument used was the GPAQ questionnaire to measure physical activity, midline to measure central obesity. Samples were taken according to the exclusion and inclusion criteria. In the bivariate analysis using the chi-square test with a significant value if p <0.05. Type 2 DM patients with mild moderate physical activity will have an OR = 22,8 (95% CI 2,7-193,7) of having diabetik ulcers than those with strenuous activity (p=0,005). Meanwhile, type 2 DM patients with central obesity will have an OR =11 (95% CI 0,59-47,3) having diabetik ulcers than type 2 DM patients who do not have central obesity(p=0,025). Light-moderate physical activity and the presence of central obesity have a significant relationship with the occurrence of diabetik ulcers in patients with type 2 diabetes.

Abstract

Diabetic retinopathy is the common complication of T2DM, which leads to vision loss. The severity of retinopathy can be affected by HbA1c levels. Controlling the glycemic such as HbA1c, may prevent the severity of diabetic retinopathy. Objective: To identified the association between HbA1c levels and severity of retinopathy in Type 2 DM patients at Cicendo Eye Hospital, Bandung. Methods: This research was an analytic observational study and cross-sectional approach. Samples were taken using purposive sampling and consisted of 70 people. The secondary data were from the outpatient's medical records at the Cicendo Eye Hospital in Bandung for the period September-October 2022. The Chi-square test was used for data analysis. Results: According to HbA1c levels, the uncontrolled HbA1C levels were found in 52 patients (74,3%) and the controlled were found in 18 patients (25,7%). Most of them were diagnosed with PDR were found 62% and the rest of it were diagnosed with NPDR. A significant association is found between HbA1c levels and the degree of diabetic retinopathy with p-value = 0.018 (p <0.05). Conclusion: HbA1c levels in Type 2 Diabetes Mellitus patients have a significant association with the severity of diabetic retinopathy, which can be used as a parameter for controlling diabetes complications, particularly retinopathy.Keywords: diabetic retinopathy, T2DM, HbA1c, glycemic control

Abstract

Polip gaster merupakan suatu kelainan yang bisa ditimbulkan oleh berbagai faktor prediktor atau multifaktorial. Insidensi polip sangat bervariasi. Meskipun jumlah kasusnya terhitung rendah, ada beberapa jenis polip gaster yang bisa mengarah kekeganasan. Tujuan dari penelitian ini untuk melakukan analisis faktor prediktor kejadian polip gaster. Metode yang digunakan selama meneliti ialah metode kuantitatif berupa pendekatan cross sectional. Kasus yang diteliti berupa pasien yang didiagnosis polip gaster tahun 2018-2022. Pengambilan sampel menggunakan teknik total sampling. Temuan yang dihasilkan berupa analisis univariat  didapatkan hasil bahwa mayoritas responden memiliki usia > 45 tahun sebanyak 23 responden (85,2%),  jenis kelamin perempuan sebanyak 25 responden (92,6%), tidak mengonsumsi obat PPI jumlahnya 25 responden (92,6%), serta IMT < 25 sebanyak 19 responden (70,4%), Analisis Chi spuare  tiap variabel yaitu  usia (p = 0,827), jenis kelamin (p = 0,530), konsumsi obat PPI (p = 0,115), dan IMT (p = 0,433). Sehingga tidak terdapat hubungan yang signifikan antara usia, jenis kelamin, konsumsi obat PPI, dan IMT dengan kejadian polip gaster.
