Abstract

Latar belakang pemilihan tema proyek studi adalah menciptakan karya seni gambar yang terinspirasi oleh ketertarikan pada keberadaan flora dan fauna yang dijadikan ide atau gagasan dengan mengolah unsur visual garis, bentuk dan warna sesuai pencitraan diri sebagai ungkapan ekspresi kreativitas penulis dari perubahan bentuk tumbuhan dan hewan. Penulis membuat karya yang bertema “storge” bermaksud mempresentasikan cinta kasih antara orang tua dan anak, manifestasi saya selaku kreator menggambarkan cinta kasih flora dan fauna dalam menciptakan karya seni gambar. Tujuan Proyek Studi ini adalah untuk menciptakan karya seni gambar yang mengambil sumber inspirasi dari transformasi flora dan fauna. Metode yang digunakan dalam berkarya meliputi pemilihan media, teknik berkarya, dan proses penciptaan karya. Media yang digunakan berupa bahan (kertas), alat (pensil warna, penghapus, dan bolpoin), dan teknik (goresan atau arsiran pensil warna yang bertumpuk-tumpuk). Proses penciptaan karya meliputi pencarian ide dan referensi gambar, dan visualisasi gagasan (sket kasar pada kertas, memberi warna dasar subjek gambar, pemberian warna pada subjek gambar secara detail, sentuhan akhir dan penyajian karya. Proyek studi ini menghasilkan dua belas karya dengan ukuran karya bervariatif, yang dibuat pada tahun 2018. Seluruh karya ini menampilkan subjek perubahan dari hewan dan tumbuhan. Pengalaman subjektif penulis berhasil penulis ungkapkan dengan pendekatan realistik. Bagi penulis flora dan fauna merupakan bukti ciptaan Tuhan yang sangat indah Penulis memahami bahwa proses penciptaan karya seni yang penulis lakukan dapat memberikan pengalaman yang mampu meningkatkan kemampuan penulis dalam mengeksplorasi ide, media, dan teknik ketika membentuk subjek gambar.

Abstract

Introduction: Cerebral oxygen saturation reflects tissue perfusion in the cerebrum. Decreases in cerebral oxygen saturation are linked to longer hospital stays and cognitive impairment. Midazolam and propofol can decrease cerebral blood flow through decreasing the cerebral oxygen metabolic rate. The purpose of this research is to analyze the comparison of changes in cerebral oxygen saturation after midazolam and propofol administration in post-craniotomy patients in the ICU of Haji Adam Malik General Hospital Medan.Subject and Method: This is a randomized control trial study. Patients were divided into, Midazolam group, that given an initial dose of 0.05 mg/kg followed by a maintenance dose of 0.02-0.10 mg/kg/hour and Propofol group that given sedation with a dose of 0.3-3mg/kg/hour, with the target of the 2 groups being a Richmond Agitation-Sedation Scale (RASS) value of 0 to -2. Data analysis using unpaired T test.Results: The results for cerebral regional oxygen saturation and RASS between groups showed significant differences in right and left value (p 0.001), but there was no significant difference in RASS (p0.05) between each group at each measurement time. The results of the analysis of cerebral regional oxygen saturation and RASS between times, there was no significant difference in right and left value (p0.05), but there was a significant difference in RASS (p0.001) at each measurement time. Based on the results of the analysis carried out, it is known that there is no statistically significant difference in changes in cerebral regional oxygen saturation both right and left in changes in RASS because it is found that all data have p0.05.Conclusion: There is no change in right and left for cerebral regional oxygen saturation after administration of propofol and midazolam groups with RASS value 0 to -2 in post-craniotomy patients in the ICU of Haji Adam Malik General Hospital Medan

Abstract

Background: Currently, there is no standardized approach to managing critically ill COVID-19 patients with acute kidney injury and ST-elevation myocardial infarction. Continuous renal replacement therapy is a routinely used technique in managing critical patients in the intensive care unit. This procedure is applicable in patients with unstable hemodynamic, renal, or non-renal indications, such as removing the excess urea and creatinine from patients with acute kidney injury or clearing the tumor necrosis factor from patients with systemic inflammations. Method: This was a retrospective case report, after analysis of patient clinical data. The patient provided written informed consent to publish their case details and any accompanying images. The study protocol complies with the requirements of the institute’s committee of Haji Adam Malik Hospital, Medan, Indonesia. Results: This report presents a case of ST-Elevation myocardial infarction with COVID-19 infection and acute kidney injury who successfully managed by percutaneous coronary intervention and continuous renal replacement therapy. This patient was prepared for percutaneous coronary intervention and intubated with consideration of strict infection control. To improve the outcomes, we performed continuous renal replacement therapy with continuous venovenous hemodiafiltration mode. The patient improved with a stable hemodynamic and better renal function after 24 hours of continuous renal replacement therapy. Conclusion: Early continuous renal replacement therapy might be beneficial in treating COVID-19 patients with AKI, who previously underwent percutaneous coronary intervention for ST-elevation myocardial infarction.

Abstract

Background: Airway management is the responsibility of anesthesiologists, but clinical assessment documentation is often incomplete. Difficult laryngoscopy and intubation occur in 1% to 15% of cases. Intubation is considered difficult if it takes more than ten minutes for a trained anesthesiologist to achieve successful endotracheal intubation. In Indonesia, difficult laryngeal visualization occurs in 10.1% of Malay patients. Common predictors include Mallampati classification, thyromental distance (TMD), obstructive sleep apnea (OSA), male gender, and age.Objective: This study aims to compare neck circumference (NC) to TMD as a predictor of difficulty visualizing the larynx.Methods: This diagnostic study aimed to evaluate the sensitivity and specificity of NC and TMD as predictors of difficult laryngeal visualization in patients undergoing elective surgery at Haji Adam Malik General Hospital, Medan. The sample consisted of 85 patients, aged 18 years or older, with ASA classifications of 1-2, who were undergoing general anesthesia. Exclusion criteria included limited mouth opening, acute burns, airway tumors, or anatomical abnormalities.Result: The mean age of participants was 54.97 ± 14.41 years, with a mean body mass index (BMI) of 26.63 ± 3.12 kg/m², and an average NC/TMD ratio of 6.37 ± 0.87. Patients with an NC/TMD ratio > 5 had difficult intubation in 87.5% of cases, while those with an NC/TMD ratio < 5 had easy intubation in 94.6% of cases. Fisher's Exact test showed a significant association (p < 0.001). The NC/TMD ratio had a sensitivity of 95.45%, specificity of 85.36%, positive predictive value (PPV) of 87.5%, negative predictive value (NPV) of 94%, and an area under the curve (AUC) of 90%.Conclusion: The NC/TMD ratio is a significant and accurate predictor of difficult intubation, supporting its use as a simple, non-invasive preoperative tool.
