Abstract

Medication errors could occur at any stage, including prescribing, transcribing, dispensing, and administration stages. Errors in the prescribing stage accounted for the most significant percentage, ranging from 70% to 99%. The most mistake at the prescribing stage is the illegible prescription. This situation causes high morbidity and mortality in hospitals. This study aimed to analyze the root of the problems of the incidence of illegible prescriptions and find anticipating strategies. This research is a descriptive study initiated by finding the root of the problems, followed by identifying alternative solutions and preparing priority solutions. From the analysis of the root of the problems, the illegible prescription included incomplete identity, drug name and the preparation, unclear usage, the error in assuming the writing in the prescription, and ineffective design of the prescription. The solution chosen was the improvement of prescription service flow (re-engineering) that added an identity labeling procedure to correct the error on patient's identity due to the repetition of manual writing and the addition of an authorization procedure by double-checking. The second solution was creating patient identity labels, creating appropriate prescription designs, and developing prescription service guidelines to support the implementation of prescription services re-engineering. This research concludes that the root of the problems on the illegible prescription is the absence of prescription service procedures that aim to anticipate medication errors. Hence, a re-engineering of prescription service needs to be prepared as an alternative solution

Abstract

The Emergency Department (ED), as the main entrance of health service access in hospitals, faces the highest infection risk and high service load due to the increasing number of patients during the Covid-19 pandemic. Implementing a pre-triage system offers a solution to deal with a surge in the number of patients who demand staff preparedness. This study aimed to determine the knowledge level on the pre-triage system and its relationship with the preparedness of emergency room staff in implementing the pre-triage system. The study was conducted with a cross-sectional approach in a private type C hospital in Malang District on all 30 ER staff as respondents. The research instrument used were questionnaires. Data analysis was done using descriptive analysis and the Fisher exact test. The results show that although 76.7% of respondents have insufficient knowledge, 86.7% of respondents are ready to implement the pre-triage system. The test results prove that there is no relationship between the knowledge and the preparedness levels of the emergency room staff in implementing the pre-triage system.

Abstract

One of the most important managerial activities for hospitals is an internal control activity for drug supplies, including during the Covid-19 pandemic. A preliminary study at Graha Sehat Medika hospital (GSM hospital) showed that the inaccuracy of stocktaking results in the last six months was 40% -70%. The solution taken is the use of "SO PINTAR RS" application with barcode scanning. The purpose of this study was to determine the effectiveness of "SO PINTAR RS" application with barcode scanning in improving the accuracy of the stocktaking results at GSM hospital. The method used in this research is quasi-experimental. The data used is primary data from stocktaking activities in July-November 2020. A total of 162 drug items were used as research samples taken from the purposive sampling technique. The discrepancy between drug items in the hospital management information system (HIS) and the physical count results before and after using the application was measured in this study. The analysis of the results was carried out through nonparametric statistical tests with the Wilcoxon test. The results showed a significant difference in the results of stocktaking activities before and after using "SO PINTAR RS" application (p <0.001). The difference between the calculation results of the stocktaking with the data in the HIS decreased significantly. In addition, using this application can reduce the number of staff involved and reduce the loss of uncounted medicine items. This application is still not linked to HIS, so it is necessary to manually merge the results into HIS.

Abstract

The number of Covid-19 patients hospitalized continues to increase in line with the increase in Covid-19 cases in Indonesia. The increasing number of Covid-19 patients in hospitals is increasing the workload of paramedics. High workloads will cause saturation and fatigue. In addition, an increase in the number of Covid-19 patients increases anxiety and depression will be infected when treating patients. Paramedics should also use personal protective equipment during shift guards which causes dehydration and discomfort. This situation can degrade the quality of service to patients. The study aims to measure the mental workload of paramedics by NASA-TLX (National Aeronautics and Space Administration Task Load Index) method and find out the factors that affect mental workload levels. This research is a quantitative research with a cross sectional approach. The research was conducted at Hospital X type C in Malang Regency in October 2020. The samples used were all 100 Hospital X paramedics. Research instruments in the form of questionnaires shared through Google Form. Data analysis is done with descriptive analysis and Chi-Square analysis with p value <0.05. The results found paramedics had a low mental workload of 1%, a moderate mental load of 3%, a high mental workload of 47%, and a very high mental workload of 49%. The Chi-Square test results found that age factors had an effect on the mental workload level of paramedics.

Abstract

Coronavirus Disease 2019 (Covid-19) infection spread rapidly to more than 114 countries. Covid-19 spreading has a tremendous effect socially and economically, Indonesia included. Indonesian Government was issuing regulations about Technical Claim Instructions for anticipation. This Technical Claim Instruction was regulated on Keputusan Menteri Kesehatan (KMK) No HK 01.07/Menkes/238/2020, then renewed on KMK No HK.01.07/Menkes/446/2020. The purpose of this research is to analyze budget efficiency related to the change of Covid-19 hospitalization claim regulation. The research was done retrospectively, using secondary data such as claims and patient's medical records. Research conducted in October 2020 on X Hospital. The research sample used was every Covid-19 patient from March to September 2020 totaling 146. The data were analyzed using chi-square with the result showing that there is no correlation between changes of technical claim regulation with budget efficiency.
