Abstract

Hipertensi atau tekanan darah tinggi merupakan salah satu faktor utama yang berkontribusi terhadap munculnya penyakit kardiovaskular yang dapat menyebabkan kematian, khususnya pada kelompok lansia. Penelitian ini bertujuan untuk mengetahui hubungan antara tingkat pengetahuan, jenis obat antihipertensi yang dikonsumsi, dan tingkat stres terhadap pengendalian hipertensi pada pasien geriatrik. Desain pada penelitian ini menggunakan cross-sectional dengan melibatkan 70 dari populasi sebanyak 76 pasien lanjut usia penderita hipertensi di RSUD Kota Mataram dengan purposive sampling. Kuesioner digunakan untuk mengukur tingkat pengetahuan, jenis antihipertensi, dan tingkat stres, sedangkan pengukuran tekanan darah dilakukan langsung terhadap responden dengan menggunakan sphygmomanometer. Data kemudian dianalisa dengan Chi-Square. Hasil penelitian menunjukkan bahwa terdapat hubungan signifikan antara tingkat pengetahuan dengan nilai p value sebesar 0,008, ketepatan jenis obat antihipertensi nilai p value sebesar 0,001, serta tingkat stres p value sebesar 0,025, dengan pengendalian hipertensi. Pasien dengan pengetahuan rendah, penggunaan obat tidak tepat, dan tingkat stres tinggi memiliki risiko lebih besar terhadap hipertensi yang tidak terkendali. Penelitian ini menyimpulkan bahwa pendekatan komprehensif yang meliputi edukasi, ketepatan pemilihan obat, dan manajemen stres diperlukan dalam pengelolaan hipertensi pada pasien lansia. Disarankan agar program edukasi dan manajemen stres terintegrasi dalam pelayanan kesehatan lanjut usia.

Abstract

In 2019, there were 523 million cases of cardiovascular disease which caused the deaths of 18.6 million people. In this manner, some major issue should be considered, high cardiovascular endurance, for example. Relatively, high cardiovascular endurance can reduce the incidence of cardiovascular disease by 40% to 70%. Objective to nalyzing the relationship between physical activity and smoking habits among farmers in Pandan Wangi Village. The method used is quantitative research with cross-sectional design involving 108 respondents. The respondents were selected by a simple random sampling technique. Data in this study were collected using GPAQ for physical activity variables, Brinkman index questionnaire for smoking variables, and Harvard step test for cardiovascular endurance variables. Meanwhile, Spearman rank test was used in the data analysis. The research shows that the characteristics of respondents were dominated by male (64.8%) within 36-45 years old age range group (52.8%). In addition, most of the respondents were non-smoker category (62.0%), had a high level of physical activity (52.8%), and a very good level of cardiovascular endurance (27.8%). Bivariate analysis shows that there is a significant relationship between physical activity (p-value = 0.005) and smoking behavior (p-value = 0.047) on cardiovascular endurance among farmers in Pandan Wangi Village. There is a significant relationship between physical activity and smoking habits on cardiovascular endurance among farmers in Pandan Wangi Village.

Abstract

Heart failure (HF) stands as a major global health problem where precise and early prediction of patient prognosis is essential for improving clinical management and patient care. A common obstacle for standard machine learning models in this domain is the prevalent issue of class imbalance within clinical datasets. To overcome this challenge, this study introduces a systematically optimized ensemble learning model for the accurate classification of patient survival. The methodology was applied to a publicly accessible clinical dataset of 299 heart failure patients. Its comprehensive framework included logarithmic transformation, stratified data splitting (80:20), SHAP-based selection of eight key features, and hyperparameter tuning with Optuna over 75 trials, with the specific objective of maximizing the F1-score using 10-fold cross-validation. The performance of three ensemble models (Random Forest, XGBoost, and LightGBM) was refined using decision threshold tuning. The results revealed that the fully optimized Random Forest model yielded superior outcomes, attaining an accuracy of 96.67%, an F1-score of 0.9474, and precision and recall values of 0.95, demonstrating high reliability with only a single instance of a False Negative and False Positive. The study concludes that the systematic application of SHAP, SMOTE, and Optuna within an ensemble framework substantially improves classification performance for imbalanced HF data, surpassing existing benchmarks. This work thus provides a replicable and systematic framework for developing reliable machine learning models from complex, imbalanced medical datasets, contributing a valuable methodology to the field of computational science.

Abstract

Hipertensi ialah suatu kondisi saat adanya peningkatan tekanan darah di atas 140/90 mmHg. Hipertensi pada lansia menjadi masalah kesehatan signifikan, terutama dengan meningkatnya jumlah lansia di Indonesia. Faktor risiko hipertensi yang dapat dimodifikasi seperti aktivitas fisik, kualitas tidur, pola makan, dan indeks massa tubuh (IMT).  Penurunan kondisi fisik akibat proses degeneratif pada lansia berkontribusi terhadap peningkatan tekanan darah. Kebaruan penelitian ini yaitu meneliti tentang aktivitas fisik, kualitas tidur, pula makan, dan indeks massa tubuh dengan kejadian Hipertensi. Tujuan penelitian yaitu untuk menganalisa hubungan aktivitas fisik, kualitas tidur, pola makan, serta indeks massa tubuh (IMT) dengan kejadian hipertensi pasien lansia di Puskesmas Selong. Metode Penelitian yaitu kuantitatif observasional analitik dengan desain cross sectional. Teknik pengambilan sampel menggunakan simple random sampling dengan total sampel 103 orang. Hasil penelitian menunjukkan mayoritas responden memiliki aktivitas fisik berat (50,5%), kualitas tidur baik (56,3%), pola makan baik (67%), dan indeks massa tubuh (IMT) normal (46,6%). Adanya hubungan yang signifikan terkait aktivitas fisik dengan kejadian hipertensi (p=0,029), kualitas tidur dengan kejadian hipertensi (p=0,007), pola makan dengan kejadian hipertensi (p=0,003), dan IMT dengan kejadian hipertensi (p=0,024) pasien lansia di Puskesmas Selong. Kesimpulannya bahwa aktivitas fisik, kualitas tidur, pola makan, dan IMT ada hubungan signifikan dengan kejadian hipertensi pasien lansia di Puskesmas Selong.Kata Kunci: Hipertensi; IMT; Aktivitas Fisik; Pola Makan; Kualitas Tidur.

Abstract

This study analyzes the effects of Brand Attachment, Brand Characteristics, Service Quality, Servicescape, and Food and Beverage Quality on Revisit Intention, mediated by Customer Brand Attitude and Customer Satisfaction. The sample consists of 500 Generation Z consumers from five coffee shop brands in Jakarta. Data were analyzed using Structural Equation Modeling (SEM) with AMOS. The findings show that Brand Attachment and Brand Characteristics significantly influence Customer Brand Attitude, while Service Quality and Food and Beverage Quality positively affect Customer Satisfaction. Both mediating variables significantly influence Revisit Intention and mediate the relationships between antecedent variables and Revisit Intention. In contrast, Servicescape does not significantly affect Customer Satisfaction among Generation Z consumers. This study contributes to service marketing literature by integrating branding and quality perspectives. However, the cross-sectional design and geographic focus limit generalizability.

Abstract

ABSTRACT Heart failure with reduced ejection fraction is a complex clinical syndrome that results in a significant decline in patients’ quality of life and functional capacity. Cardiac rehabilitation has been recognized as a comprehensive evidence-based intervention to address these issues. Objective: To systematically analyze the effect of cardiac rehabilitation on quality of life and functional capacity in patients with heart failure with reduced ejection fraction based on the latest literature. Methods: A systematic literature review was conducted using the PRISMA 2020 guidelines with searches performed in five major electronic databases for the publication period 2021–2025. The inclusion criteria comprised randomized controlled trials and quasi-experimental studies that evaluated the effectiveness of cardiac rehabilitation in adult patients with clinically stable HFrEF. Results: Thirty studies met the inclusion criteria with a total population of 84,064 patients. Cardiac rehabilitation showed a significant improvement in the KCCQ score with a mean increase of 18.4 points and a reduction in the MLHFQ score by 15.8 points. Functional capacity improved with an increase in peak VO2 of 3.2 mL/kg/min and an increase in 6MWT distance of 68.4 meters. Comprehensive programs showed a superior effect size compared to exercise-only interventions. Conclusion: Cardiac rehabilitation provides a clinically significant positive impact on the quality of life and functional capacity of patients with HFrEF, supporting its implementation as a standard of care with a comprehensive and individualized approach. Keywords: Cardiac Rehabilitation, Heart Failure, Quality of Life, Functional Capacity, Systematic Review.
