Abstract

Stroke iskemik merupakan salah satu penyebab kecacatan dan kematian yang dapat dicegah. Kejadian stroke iskemik dapat diprediksi berdasarkan skor risiko stroke Framingham, serta faktor risiko lain seperti obesitas, dislipidemia, dan hiperurisemia. Penelitian ini bertujuan untuk mengetahui profil risiko stroke iskemik, obesitas, dislipidemia, dan hiperurisemia pada penduduk di Kecamatan Sekarbela Mataram. Sebanyak 115 subjek dilibatkan dalam penelitian potong lintang yang mengukur derajat risiko stroke iskemik dalam 10 tahun menurut Framingham, indeks massa tubuh (IMT), lingkar pinggang, serta rasio lingkar pinggang-panggul. Faktor risiko dislipidemia pada 84 subjek ditentukan berdasarkan kadar kolesterol total dan kolesterol HDL. Faktor risiko hiperurisemia pada 77 subjek ditentukan berdasarkan serta kadar asam urat serum. Hasil penelitian menunjukkan derajat risiko stroke iskemik tinggi dan sedang masing-masing sebesar 12,2% dan 13%. Subjek obese berdasarkan IMT, lingkar pinggang, dan rasio lingkar pinggang-panggul masing-masing sebesar 14,78%, 37,39%, dan 9,57%. Subjek dengan kadar kolesterol total serum tinggi dan kolesterol HDL serum rendah didapatkan masing-masing sebesar 10,72%, dan 51,19%. Hiperurisemia didapatkan pada 46,75% subjek. Sebagai kesimpulan, penduduk di Kecamatan Sekarbela Mataram memiliki risiko stroke iskemik 10 tahun Framingham dan proporsi obesitas yang rendah, disertai dislipidemia dan hiperurisemia pada separuh penduduk. Abstract

Executive dysfunction is a consequence of type 2 diabetes mellitus decreasing the quality of life of the patients. Currently, its prevalence is still scarce. Early detection of this executive dysfunction is beneficial in improving the clinical outcomes of the patients. This study aims to investigate the prevalence of executive dysfunction among type 2 diabetes mellitus patients in Mataram. This case-control study involving 106 subjects divided into two groups, type 2 diabetes subjects (n=53) and healthy subjects (n=53) in outpatient setting. The data collected from both groups were age, gender, years of education, TMT-B score, and executive function status. The data collected only from type 2 diabetes mellitus group were age, gender, years of education, duration of diabetes, treatments, smoking, hypertension, coronary heart disease, body mass index, and dyslipidemia. There was significant difference in the frequency of executive dysfunction between type 2 diabetes mellitus and healthy subject groups (p0.05). The prevalence of type 2 diabetes mellitus-associated executive dysfunction was 49.1%. Gender was the only characteristics associated with the prevalence of type 2 diabetes mellitus-associated executive dysfunction (OR=0.25; 95%CI=0.07-0.88, p0.05). There was a high prevalence of type 2 diabetes mellitus-associated cognitive impairment and it was associated with female gender.

Abstract

Cognitive dysfunction is an important consequence of ischemic stroke, which can progress in the first few years and is primarily determined by clinical factors. This study aimed to investigate the clinical determinants of cognitive dysfunction in stroke survivors in West Nusa Tenggara Province, Indonesia. This cross-sectional study assessed 255 ischemic stroke survivors with a mean age of 57.1±9.3 years old and 29–79 years old, recruited consecutively in three main hospitals in West Nusa Tenggara Province between March 2019 and October 2021. Categorical data collected included age, sex, education level, clinical determinants of ischemic stroke, and cognitive status of the patients. The association between the clinical determinants of ischemic stroke and the risk of cognitive dysfunction in patients was analyzed using logistic regression after adjusting for age, sex, and level of education. The final multiple logistic regression analysis models revealed infarct diameter as the only clinical determinant significantly associated with an increased risk of cognitive dysfunction (OR = 3.14;95% CI = 1.20–8.23). Thus, a larger infarct diameter is the only clinical determinant of cognitive dysfunction in ischemic stroke survivors in West Nusa Tenggara Province, Indonesia.

Abstract

Cognitive impairment is a medical condition commonly found in elderly populations, which can be due to vascular risk factors in patients. There remains limited data on risk factors for cognitive impairment among coastal region populations. This study aimed to investigate risk factors for cognitive impairment in the adult population of Maringkik Island, West Nusa Tenggara Province, Indonesia. Data collected were age, sex, education level, hypertension, antihypertensive treatment, diabetes mellitus, cigarette smoking, and body mass index status. A total of 114 participants were recruited using a consecutive sampling method. The participants’ cognitive function assessment used the Mini-Cog instrument. The cognitive impairment frequency in the island’s adult population was approximately 48.2%. The final model of multiple regression analysis showed that hypertension (OR: 2.3; 95%CI: 1.0 – 5.0; p-value: 0.045) was a characteristic associated with the cognitive impairment frequency. Thus, the high frequency of cognitive impairment and hypertension frequency as primary risk factors for cognitive impairment in the island’s adult population implies the need to develop strategies for detecting and managing hypertension and hypertension-related cognitive impairment in the population by local health authorities.
