Abstract

Background problem: Diabetes Mellitus (DM) prevalence in Indonesia based on the results of basic health Research (RISKESDAS) year 2013 is 2.1%, in Central Java  18.33%. Banyumas District has an incidence rate of 1,417 in 2014 and Sumbang Sub-district has the highest incidence rate of DM type 2. Management in DM sufferers is integrated, including eating arrangements, physical activity, education, and medicines. Meal arrangement for patients with type 2 DM includes setting against 3J: number, type and meal schedule in a day. Patients with DM who have already run a diet program has not been able to control blood glucose properly. Food arrangement often fails due to the disobedience of sufferers in running their diet. Many factors affect dietary compliance, among them the level of education, level of knowledge, self-motivation, family support and the role of health workers.Objective: To determine the factors that affect the compliance of diet in diabetes mellitus type 2 patients in Sumbang Sub-district of Banyumas.Methods: The studies used are cross sectional, the level of compliance seen from 3J, i.e. precise amounts, precise types and precise schedules. Samples taken from DM patients who are in Prolanis present during laboratory examination in June 2019. Sampling technique using total sampling. Bound variables are the level of dietary compliance, the free variables are education levels, knowledge levels, self-motivation, family support and the role of health workers.Results: The number of samples was 32 people, who were obedient in a diet of 15 people (46.9%), who did not obey 17 people (53.1%). The P value of the factor variable factors affecting the diet is: age p = 0.411, sex p = 0.035, Job p = 0131, long DM p = 0.261, education level p = 0.035, knowledge level p = 0.036, self motivation p = 0.464, family support p = 0.314, and P Health worker, p = 0.452. Variables that have a meaningful relationship (P < 0.05) with dietary compliance levels are gender, level of education and level of knowledge. As for the variable age, employment, old DM, self-motivation, family support, and the role of health workers have an insignificant relationship (P > 0.05Conclusion: From 32 patients DM most have a level of diet compliance in the category of disobedient 17 people (53.1%), there is a meaningful relationship between gender, education level and level of knowledge with DM dietary compliance,

Abstract

Diabetes Melitus Tipe 2(DMT2) dapat menyebabkan terjadinya berbagai komplikasi, baik komplikasi makrovaskuler maupun mikrovaskuler. Salah satu komplikasi mikrovaskular yang paling umum adalah retinopati diabetik. Metode Penelitian ini merupakan penelitian observasional analitik dengan pendekatan cross sectional. Sampel penelitian ini terdiri dari 43 pasien Prolanis di FKTP Klinik Tanjung yang diambil dengan menggunakan teknik consecutive sampling.. Analisis hipotesis menggunakan uji chi-square. Hasil penelitian analisis univariat menunjukkan 67,4% responden kurang mendapat informasi, 67,4% responden tidak mendapat informasi dari petugas kesehatan, dan 18,6% responden terdiagnosis retinopati diabetik. Analisis bivariat antara pengetahuan pasien dengan prevalensi retinopati diabetik menggunakan uji chi-square menunjukkan p = 0,180. Hasil analisis bivariat terhadap data prevalensi retinopati pada petugas kesehatan dengan menggunakan uji chi-square menunjukkan p = 0,180. Pada penelitian ini, Tidak terdapat hubungan antara pengetahuan pasien dan informasi oleh tenaga kesehatan mengenai penyakit retinopati diabetik dengan kejadian retinopati diabetik pada FKTP Klinik Tanjung Purwokerto. Kesimpulan tidak ada hubungan pengetahuan dan dukungan informasi dari petugas kesehatan mengenai penyakit retinopati diabetik dengan kejadian retinopati diabetika di Fasilitas Kesehatan Tingkat Pertama (FKTP) Tanjung Purwokerto.

Abstract

Diabetic retinopathy is a major microvascular complication due to diabetes mellitus (DM) and prolonged hyperglycemia. One of the risk factors for the development and progression of diabetic retinopathy is lipid profile levels in DM patients. Objectives: To determined the relationship between lipid profile and the incidence of diabetic retinopathy in DM patients who follow the Chronic Disease Management Program at the  first level health facilities Klinik Tanjung Purwokerto. Methods: The number of respondents in this study was 49 people. The independent variables assessed were lipid profiles, including levels of total cholesterol, triglycerides, LDL, and HDL. The dependent variable is the incidence of diabetic retinopathy determined by an ophthalmologist. Statistical analysis to determine the relationship between lipid profile and the incidence of diabetic retinopathy used the Chi-square test and calculated the odds ratio (OR). Results: The results showed that there was no relationship between levels of total cholesterol (p=0.586), triglycerides (p=1), and LDL (0.719) with the incidence of diabetic retinopathy. The statistical analysis results of the relationship between HDL levels and the incidence of diabetic retinopathy were known to have a p-value = 0.046 (OR 4.83; 95% CI 1.23-18.98). Conclusion: There is a significant relationship between HDL levels and the incidence of diabetic retinopathy in Type 2 DM patients at the FKTP Klinik Tanjung Purwokerto. DM patients with abnormal HDL levels are 4.83 times more likely to develop diabetic retinopathy than DM patients whose HDL levels are normal.Keywords: diabetes mellitus,  lipid profile, diabetic retinopathy

Abstract

Background: Indonesia ranks 6th highest with Diabetes mellitus (DM) in the world in 2019. There are complications occur due to DM type 2. To prevent this, fasting blood sugar (GDP) monitoring and patient diet by using pocketbooks as an educational compliance can be carried out. Objective: To identify the effect of using pocketbooks as an educational medium in monitoring dietary compliance and GDP levels of type 2 DM patients in Sumbang. Methodology: Quasi-Experimental method was used with pretest – posttest design on 100 DM Prolanis patients at Puskesmas Sumbang 1 and 2 which were divided into two groups. Both groups were given different education where the treatment group used a pocketbook while the control group was given oral education. Results: In the pretest-posttest data of GDP levels, there were differences in results before and after the intervention in the treatment group (Wilcoxon test, p = 0.000) while the control group gave no difference in results before and after the intervention (Wilcoxon test, p = 0.090). In the pretest – posttest diet adherence data, there were differences in results before and after the intervention in the treatment group (Paired T Test, p = 0.000) while the control group gave no difference in results before and after the intervention (Paired T Test, p = 0.074). The Mann Whitney test and the unpaired T test showed that the two variables had a significant difference (p = 0.000). Conclusion: there is an effect of pocketbook education intervention on GDP levels and dietary compliance of type 2 DM patients in Sumbang District, Banyumas Regency which reduces GDP levels and improves respondents' dietary compliance.
