Abstract

Female breast cancer is the most common diagnosed cancer worldwide in 2020, with an estimated 2.3 million new cases or 11.7% from total 18,1 million cases, followed by lung, colorectal, prostate, and stomach cancers. In 2040, the breast cancer cases are predicted will be increased into 3 million cases per year or grow 40% from 2020 data. It is also estimated the number of deaths due to breast cancer will be one million death per year in 2040. In Indonesia, the number of new breast cancers reached 68,858 cases (16.6%) of a total of 396,914 new cases of cancer in 2020. ​ The incidence and mortality rates of breast cancer are 44 and 15.3 per 100,000 population, respectively in Indonesia. The most common breast cancer treatments used currently are surgery, chemotherapy, and radiation. These three treatments involve a rather invasive approach and often causes side effects. Serious side effects can reduce a patient's quality of life, lead to non-compliance to chemotherapy, and lead to drug resistance.​ Some cancer patients use alternative therapy such as complementary medicine including herbal medicine to treat the cancer. Patients try to discover and utilize natural-derived anti-cancer drugs because it believes has fewer side effects and more affordable. One of Indonesia plant which is therapeutically beneficial is Ficus deltoidea. Ficus deltoidea leaves had been used in traditional medicine to treat diabetes, inflammation, diarrhoea, cancer and infections. F. deltoidea leaves contained flavonoids, saponins, tannins, steroids and terpenes which known for its anticancer effect.

Abstract

Cardiac remodeling is a phenotype of heart failure characterized by a molecular, cellular, and interstitial change in the heart, which manifests in the change of size and function of the heart after specific insults with multiple mechanisms. Centella asiatica (CA) and its main bioactive triterpenoid, asiatic acid (AA), pose antioxidant and anti-inflammatory effects. Still, no adequate clinical trials support the potency of CA and AA as anti-cardiac remodeling. Hence, this systematic review aims to provide an in-depth analysis of CA extract and AA in animal studies' prevention or therapy of cardiac remodeling. The search strategies were conducted based on preferred reporting Items for systematic reviews and meta-analysis (PRISMA) protocol through Pubmed, EMBASE, Scopus, and Web of Science using keywords as follows: “Centella asiatica” OR “Asiatic Acid” AND “Cardiac Remodeling” OR “Cardiac Hypertrophy” OR “Cardiac Fibrosis” along with their synonym. The data collected included hemodynamic parameters based on echocardiography, biomolecular tests such as quantitative polymerase chain reaction (qPCR), Western blotting, or biochemistry procedures. The paper quality was assessed using Systematic Review Centre for Laboratory Animal Experimentation (SYRCLE) risk of bias (RoB). The previous selected study has shown that CA and AA might prevent and cure cardiac remodeling by inhibiting various pathways and protein expressions through AMPKα, NOX2/4, PI3K/Akt/mTOR, p70S6K, YAP/TAZ, and IL-1β, IL-6, and IL-18 cytokines. CA and AA, thus, exhibit cardioprotective effects in the animal model, which need to be confirmed in the clinical trials on humans. Keywords: cardiac remodeling, cardiac hypertrophy, cardiac fibrosis, Centella asiatica, asiatic acid

Abstract

Berdasarkan data dari Badan Pusat Statistik Tahun 2021, lebih dari 10% penduduk di Indonesia merupakan lansia atau berjumlah 29,3 Juta Jiwa. Jumlah tersebut diprediksi akan mengalami peningkatan. Diketahui sejak tahun 2000, persentase peningkatan populasi lansia di Indonesia melebih 7% sehingga Indonesia dikatakan masuk dalam negara kelompok berstruktur tua (aging populations). Diproyeksikan pada tahun 2045, satu per lima dari total populasi di Indonesia adalah lansia. Di Dunia pun pada tahun 2030 diprediksi akan ada 1 dari 6 orang yang merupakan lansia. Pada lansia, terjadi akumulasi kerusakan molekuler dan seluler yang dilanjutkan dengan penurunan fungsi organ sehingga terjadi peningkatan morbiditas dan mortalitas. Beberapa penyakit yang dialami lansia di antaranya adalah hipertensi, jantung, penyakit sendi, stroke, kanker, alzheimer, hingga penyakit langka seperti Myasthenia gravis (MG). Berdasarkan studi epidemiologi, diketahui terjadi peningkatan insiden Myasthenia gravis (MG) pada lansia.
