Abstract

Community Health Centers (CHCs) take the frontline position of health services in Indonesia. To be able to perform their functions properly, CHCs require human resources (HR) as a driving force. One of the human resources required is a general practitioner. The policy of utilization GP sat CHCs in Jakarta provides flexibility for sub-district health centers as the implementing agency in the management of human resources. One of the variables that influence the process of policy implementation is the capability of the implementing agencies indicated through their management capabilities. This study aims to describe the policy implementation on the utilization of general practitioners at Kemayoran sub- district health centers. The research was conducted using a mixed method combining qualitative and quantitative approach. The results confirm that there has been an imbalance proportion between the workload of general practitioners at village health centers and sub-district health centers, due to the absence of standardized rules regarding the placement of general practitioners, in addition to lower capacity of the human resources available. One of the recommendations proposed is the improvement of management using a simulation model of general practitioners based on balanced workload forecasting.

Abstract

The Cardiovascular disease (CVD) prevalence continues to increase. This study aims to identify the CVD spending and utilization in the Indonesia National Health Insurance (JKN) scheme 2017-2021 using the JKN claims classified by Disease Accounts analyzed descriptively. The CVD spending increased by $125,4 million in 2019 compared to 2017 and mostly for the hospital (91% in 2019) with the highest spending occurring in the age above 60 years. Moreover, the hospital inpatient spending was dominated during these five years. The highest portion of spending by severity level was at mild level, followed by moderate and severe levels. Health facilities’ capacities across the regions remain the issue in providing health services, especially for the CVD that require further treatment using advanced technology. The CVD cost continues to increase annually; fortunately, the JKN plays a key role as financial protection for participants. But, there was a decrease in the number of visits and spending on CVD during the COVID-19 pandemic because of lockdown policies and health systems shifting to provide capacity to overcome COVID-19. The decrease in the number of visits to health facilities raises the potential of being diagnosed later with more severe. As policy recommendations, strengthening primary care screening and hospital capacity across regions through implementing the Health System Transformation are expected to reduce the impact of these problems.

Abstract

Colorectal cancer is fairly common compared to other cancers. The incidence and mortality rates are predicted to increase globally. In some cases, cancer can be potentially spread to another organ or metastatic. One of recent available targeted therapies for metastatic colorectal cancer (mCRC) patient is Cetuximab (Erbitux ®), combined with chemotherapy. Despite clinical effectiveness, there is the importance of the evidence related cost-effectiveness of therapy. This study aims to summary, synthesize, and systematically review the economic evaluation studies of Cetuximab for metastatic colorectal cancer (mCRC). Model based economic evaluation of Cetuximab for metastatic colorectal cancer will be searched and included in the review based on specific eligibility criteria. Several electronic databases that will be used: Medline, Embase, Cochrane, National Institute of Health Research (NIHR) Center for Reviews and Dissemination. Full economic evaluation evidence will be summarized and critically appraised using Drummond as well as (Consolidated Health Economic Evaluation Reporting Standards (CHEERS) checklist. In terms of analysis, we will qualitatively appraise and present the studies that meet our inclusion and exclusion criteria. We are expected to summarize the quality and capture the valuable insights related to health economic evaluation studies of Cetuximab for mCRC patient.
