Abstract

Kinerja perkerasan jalan mengalami penurunan sepanjang masa pelayanan. Karena itu perlu upaya untuk menjaga kestabilan kondisi jalan dengan melakukan pemeliharaan. Campuran bubur aspal emulsi (slurry seal) saat ini banyak digunakan untuk pemeliharaan jalan. Sebagai bahan campuran, digunakan agregat, bahan pengisi (filler), aspal emulsi dan air. Penggunaan mastic asbuton (hasil ekstraksi) dan fly ash sebagai campuran slurry seal dengan metode campuran dingin, sebagai upaya pemanfaatan sumber daya alam (asbuton) dan limbah (fly ash). Kinerja campuran slurry seal dapat dilihat dari nilai konsistensi dan waktu pengerasan (setting time), sehingga tujuan penelitian adalah menentukan konsistensi optimum dan nilai setting time. Sampel dibuat dengan 5 variasi persentase mastic asbuton (MA) dan fly ash (FA), yaitu 0%MA : 100%FA; 25%MA : 75%FA; 50%MA : 50%FA; 75%MA : 25%FA dan 100%MA : 0%FA. Hasil pengujian menunjukkan bahwa nilai konsistensi semua variasi campuran slurry seal memenuhi persyaratan. Kadar aspal emulsi optimum 13% dengan pra-pembasahan 5% dan penambahan air 10% terhadap berat kering total agregat. Semakin tinggi kandungan filler mastic asbuton (penurunan kadar filler fly ash) maka nilai setting time semakin besar karena filler fly ash mempunyai nilai penyerapan air yang lebih besar dibandingkan dengan filler mastic asbuton.

Abstract

It has become a common issue that the Global Fund (GF) as one of the largest international donors to AIDS, Tuberculosis, and Malaria Program will immediately stop the funding. Data shows that in 2009 GF ATM support reached 88,8% while APBN funding just cover 11,2% of the total budget needed. However, APBN budget for ATM programs was significantly increased in 2012 which covered almost 30% of the total budget. Eventhough the increasing trend of ATM budget seemed at the central government level, how ever the local governments will hold the key to the sustainability of the post- termination GF ATM Funding

Abstract

Cost-containment is one of several strategies to ensure the financial sustainability of the National Health Insurance scheme. Several cost-containment models were commonly globally, such as cost-sharing, capping, and others. This review aims to determine the costs and impacts of implemented policy schemes as cost-containment instruments in various countries. We performed a systematic review from several primary databases (Proquest, Pubmed, and Cochrane Library) with the primary intervention are the cost-sharing methods. The results of our review focused on the cost containment scheme from the government perspective, in which the context of social insurance can be a modification of payment systems, cost-sharing, capping/quota, and waiting period. From one of the studies in Canada, we can see that the result has a significant impact on the health system, reducing the expenditure and the use of drugs that are not essential, and also indirectly improve the technical efficiency of the drug market through the care of participants in drug utilisation. In this research, it can be concluded that the implementation of cost containment schemes can reduce the moral hazard risk from the perspective of participants with additional contributions to the utilisation of healthcare services.

Abstract

Private hospitals must monitor their performance using indicators commonly used in DRG payment systems: casemix, casemix index and hospital base rate. This study aims to analyze the performance of private hospitals including casemix, casemix index and hospital base rate. This cross sectional study was conducted with a sample of 7 hospitals selected purposively representing private hospital type B, C, and D in 6 provinces. Casemix and CMI RS are strongly influenced by the hospital capacity, which the hospitals class B have higher casemix and CMI than class C and D hospitals. There is a phenomenon, CMI in class C hospitals smaller than class D which is probably caused by coding accuracy. The completeness and accuracy of diagnosis and procedures coding predominating affect the casemix and CMI. Almost all hospitals have HBR which are higher than National HBR except RSC3. It definitely will trigger profit from the implementation of INA-CBGs. Hospitals should monitor casemix, CMI and HBR regularly. The Ministry of Health is expected to publish the Cost Weight and National HBR as well as the average of casemix and CMI according to class of hospital, for a benchmark by hospitals.

Abstract

Almost 80% of the source of funds is spent on hospital. This situation indicates the failure of the gatekeeper function in primary health care. Performance-based capitation is implemented to ensure the cost and quality of primary care, with non-specialist referral rate as one of the particular parameters. The peer review policy is a discussion between BPJS Health, District/City Health Services, IDI, provincial and branch Team of KMKB and other relevant stakeholders to determine which diagnoses must be handled at FKTP also to accommodate and improve the capacity of FKTP to handle patients and minimize referral cases. This study aims to evaluate the implementation of peer review discussions and analyze the obstacles, challenges and potential of peer review policies to optimize the FKTP gatekeeper function. The research method is qualitative, and it involves conducting in-depth interviews, FGDs, and literature reviews. In general, the peer review policy is quite good in controlling referral cases, especially RNS (Non-Specialist Referrals). Evaluation of the "peer review discussion" policy on the communication aspect is the lack of socialization regarding the management of tiered referrals. Resources, especially human resources, health equipment and drugs, still need to be increased in FKTP. Payment mechanisms such as capitation are essential to adjust the risk and capacity of FKTP or incentive mechanisms—determination of PPK on FKTP, clear boundaries and medical authority for FKTP and FKRTL. As well as, the role and coordination among stakeholders need to be improved to strengthen the prerequisites for FKTP infrastructure.

Abstract

Colorectal cancer is fairly common compared to other cancers. The incidence and mortality rates are predicted to increase globally. In some cases, cancer can be potentially spread to another organ or metastatic. One of recent available targeted therapies for metastatic colorectal cancer (mCRC) patient is Cetuximab (Erbitux ®), combined with chemotherapy. Despite clinical effectiveness, there is the importance of the evidence related cost-effectiveness of therapy. This study aims to summary, synthesize, and systematically review the economic evaluation studies of Cetuximab for metastatic colorectal cancer (mCRC). Model based economic evaluation of Cetuximab for metastatic colorectal cancer will be searched and included in the review based on specific eligibility criteria. Several electronic databases that will be used: Medline, Embase, Cochrane, National Institute of Health Research (NIHR) Center for Reviews and Dissemination. Full economic evaluation evidence will be summarized and critically appraised using Drummond as well as (Consolidated Health Economic Evaluation Reporting Standards (CHEERS) checklist. In terms of analysis, we will qualitatively appraise and present the studies that meet our inclusion and exclusion criteria. We are expected to summarize the quality and capture the valuable insights related to health economic evaluation studies of Cetuximab for mCRC patient.

Abstract

East Suwangi Village is one of the villages in Sakra District, has an area of ​​2.17 km2, has hilly geographical conditions and a population of 5,069 people. Based on survey results and interviews with village heads, some areas are prone to flood disasters. The flooding that occurred was caused by overflow from drainage channels, causing it to accumulate on roads and residential areas. One effort to minimize inudation is the implementation of an environmentally friendly drainage system by constructing infiltration wells along flooded channels. The method used in this community service activity is to explain environmentally friendly drainage systems to the community and the practice of making infiltration wells in locations that are often flooded. The result of this activity is an increase in public understanding of the function and benefits of infiltration wells as a medium for environmentally friendly flood management. And the public can see firsthand how to make infiltration wells through practice in locations that are often flooded.

Abstract

Desa Setanggor dilewati oleh sebuah sungai yaitu Sungai Berampes yang dapat dimanfaatkan sebagai penyediaan air di musim kering melalui pengolahan kualitas air. Sebagian besar masyarakat di Desa Setanggor Kecamatan Praya Barat belum memanfaatkan dan terlayani oleh jaringan air bersih PDAM sehingga memanfaatkan air dari sumur bor komunal sebagai penghasil air baku sehari-hari. Namun, karena beberapa keterbatasan teknis dan non-teknis menyebabkan ketidakcukupan untuk pemenuhan kebutuhan sehari-hari masyarakat sekitar. Akibatnya sebagian masyarakat mengalami kekurangan air bersih terutama di masa kemarau panjang. Hasil instalasi dan uji fungsi alat menggunakan sampel air dari sungai Berampes menghasilkan 4 dari 5 nilai parameter kualitas air yang sesuai dengan standar dari Permenkes 32/2017, kecuali temperatur. Oleh karena itu, disarankan lokasi dan waktu pengambilan sampel air serta perlakuan pra-pengujian mendapatkan perhatian dan porsi khusus dalam proses pengujian, selain jenis material gradasi partikel penyaringnya.

Abstract

Selaparang and Mataram Districts are two of the six districts in the Mataram City area that experience changes in land use every year. This change can affect the absorption capacity of groundwater in the area to decrease so that if there is rain with a fairly high intensity, it can cause water to overflow to the surface and inundate the area. This study was conducted to determine the runoff discharge that occurred due to changes in land use that occurred in 2013, 2018, and 2023, and to compare the runoff discharge in 2023 with the runoff discharge based on the Regional Spatial Plan of Selaparang and Mataram Districts in 2011-2031. The initial stage in this study was to collect several secondary data such as rainfall data, land use based on the regional spatial plan, and others that can be obtained from related agencies. After that, the next stage is the delineation of the catchment area and land use analysis with the help of ArcGis and Google Earth software, as well as runoff discharge analysis. The results of the analysis show that the percentage increase in runoff discharge that occurred in Selaparang District in 2013-2018, 2018-2023, and 2013-2023 was 2.21%, 1.40%, and 3.64%, respectively. Meanwhile, the percentage increase in runoff discharge in Mataram District in 2013-2018, 2018-2023, and 2013-2023 was 2.43%, 0.73%, and 3.18%, respectively. Based on this, it can be concluded that the reduction in green land area and the increase in built-up land area each year will cause an increase in the runoff coefficient value, so that the runoff discharge that occurs in Selaparang and Mataram Districts also increases every year.

Abstract

Flood disasters in Kuta Village, Central Lombok Regency, are caused by high rainfall intensity, sedimentation, land-use changes, and low public awareness of river conservation. This study evaluates flood mitigation strategies for the Tabelo River through structural and non-structural approach. Hydrological analysis indicates that the river’s cross section is inadequate to handle flood discharge, leading to frequent overflows. Structural measures include sediment dredging to a depth of 1,45 m and constructing 4 m high embankments. Hydraulic modeling using HEC-RAS support these interventions to reduce flooding. Non-structural measures involve community outreach on riverbank regulations based on Ministerial Regulations No. 28 of 2015. This initiative aims to increase public awareness, prevent illegal riverbank constructions, and promote sustainable river management. The enforcement of riverbank regulations is expected to reduce flood risks, improve water quality, and minimize erosion and sedimentation. Additionally, educational posters have been developed to reinforce awareness of flood control and river construction. Integrating structural and non-structural strategies is crucial to mitigating floods and fostering environmental responsibility in Kuta Village.
