Abstract

Tujuan penelitian dan pengembangan ini adalah: (1) Merancang dan membuat media pembelajaran digital sistem distribusi energi listrik berbasis pemahaman konsep pada pembelajaran energi baru terbarukan di TE UM; (2) Merancang dan membuat simulasi sistem distribusi energi listrik berbasis pemahaman konsep pada pembelajaran energi baru terbarukan di TE UM; (3) Menguji kelayakan media pembelajaran digital sistem distribusi energi listrik berbasis pemahaman konsep pada pembelajaran energi baru terbarukan di TE UM. Pengembangan ini menggunakan model penelitian ADDIE dengan lima tahap penelitian dan pengembangan. Uji kelayakan bahan ajar dilakukan oleh 2 (dua) dosen ahli Teknik Elektro, mahasiswa S1 TE angkatan 2015 dan mahasiswa S1 TE angkatan 2016 untuk megevaluasi kelayakan media pembelajaran digital dan simulasi menggunakan instrumen berupa angket. Hasil penelitian menunjukkan bahwa tingkat kelayakan media pembelajaran digital energi terbarukan tentang sistem distribusi tenaga listrik pembangkit energi terbarukan memperoleh persentase sebesar 83,4% dari 2 (dua) dosen ahli yang artinya produk layak digunakan dengan sedikit revisi, 86% dari 10 (sepuluh) mahasiswa S1 TE 2015 yang artinya produk sangat layak untuk digunakan, sedangkan dari mahasiswa S1 TE 2016 memperoleh 88% yang artinya produk sangat layak digunakan tanpa ada revisi lagi.

Abstract

Background: Preeclampsia is a pregnancy complication with a triad of symptoms including high blood pressure, proteinuria, and tissue swelling as well as convulsions and coma. Preeclampsia in pregnant women can cause disruption in the supply of nutrients and oxygen to the fetus which can inhibit fetal growth and development (intrauterine growth restriction). In several similar previous studies, there were different variations in results which could be caused by multifactorial factors. This research was conducted to determine the relationship between preeclampsia in pregnant women and low birth weight babies by including confounding factors into the research. Subjects and Method: This was a retrospective case control study at Dr. Moewardi Hospital, Surakarta, Central Java, Indonesia. A total of 120 live birth infants was selected using purposive sampling. The dependent variable was low birth weight (LBW). The independent variables were pre­eclampsia, maternal age, gestational age, parity, gemelli, anemia, and asthma. Data were collected from medical records of Dr. Moewardi Hospital from January to December 2022. The data were analyzed using a multiple logistic regression. Results: Preeclampsia increased LBW by 7.75 times compared to without preeclampsia, and this was a statistically significant (OR= 7.75; 95% CI= 2.77 to 21.71; p<0.001). In addition, there was a confounding variable of gestational age (OR= 18.47; 95% CI= 6.46 to 52.79; p<0.001). Conclusion: Preeclampsia increases LBW, and this is a statistically significant.

Abstract

Coronavirus disease 2019 (COVID-19) has spread worldwide rapidly. COVID-19 in children is relatively mild compared to adults. However, several studies showed high mortality rates in children with underlying diseases. This study aimed to describe the clinical characteristics and outcomes of pediatric COVID-19 cases in regional referral hospital. This study was retrospective observational study using data from Medical Record Dr. Moewardi General Hospital between October 1st, 2020 and October 30th, 2022. The epidemiological and clinical data were extracted from the database and were compared with outcomes of patients. All statistical analyses were performed by SPSS software version 27. This study enrolled 371 (19,9%) children with confirmed RT-PCR SARS CoV-2 from 1856 hospitalized suspected children whereas 55% were males, and 63% aged under 5 years old. Moderate-severe patients accounted for 18% of the cases. Cough (74,4%), fever (66,6%) and shortness of breath (22,6) are the most common symptoms while 99 (26,7%) patients who had chronic diseases with the most common diseases being congenital heart disease, hematological malignancy and neurological disorders. Patient outcomes were significantly related to age, degree of severity and chronic disease with p=0.004, p< 0.001, and p< 0.001 respectively. After performing multivariate regression analysis, degree of severity [p<0.001; adjusted OR = 28.03, 95% CI (10.73–73.21)] was found to be significant predictors for mortality. The clinical and epidemiological data revealed that the majority symptom were cough, fever and shortness of breath. Mortality rate during hospitalization was higher in children with moderate-to-severe COVID-19.

Abstract

Background The purpose of the pediatric residency program is for aspiring pediatricians to develop the core competencies needed to care for the children’s health. Upon graduation, pediatricians encounter competency challenges related to diverse populations, cultures, and medical facilities across the nation. Enhancing the efficacy and efficiency of medical education in pediatric residency programs requires identifying and addressing gaps in the post-learning outcomes and intra-learning processes. Objective To investigate competency gaps between the core competencies taught in the pediatric residency training program and the actual necessitates in the professional experience of pediatricians. Methods This qualitative study was conducted by a focus group discussion using semi-constructed questions. Twelve pediatricians who graduated from the Pediatric Residency Program of Universitas Sebelas Maret and had professional work experience of 3 to 18 months were included. Focus group discussions were held online and moderated by the research team using guided questions. Two FGD sessions of 5 and 7 subjects each were conducted. The discussions were recorded and transcribed for analysis in a stepwise manner involving data grouping, information labeling, and data coding to identify the main themes. Results All subjects were working in type B, C, and D hospitals in six different provinces at the time of the study. The competency gaps in intra-residency learning domain covered two topics: curriculum content (core competencies, medicolegal education, effective communication, vaccination, and longitudinal case management) and training (case variations, patient complexity, and affiliated hospital rotations). The smallest competency gaps was core competency; and there was a need for improvement in other topics, particularly in effective communication and vaccination. The competency gaps in the post-learning domain included 4 topics: daily practices (demographic differences, popular topics, and effective communication), health facilities (the gap between individual competency and availability of health facilities), professional relationships with senior/other center colleagues, and strategic plans (continuing medical education). The cultural social background of the population and limited health resources were the main issues in the post-learning are-sxa that need be acknowledged. Effective communication was the theme that emerged and should be to be taught in both ares of learning and post-learning. Conclusion Pediatric residency graduates should address some gaps in learning after completing the pediatric residency program. Effective communication was identified as a learning gap during and after training. Based on our findings, we recommend for the program to provide additional steps to prepare pediatric residents before their graduation.

Abstract

Indonesia sebagai negara berkembang terus menghadapi tantangan yang signifikan dalam mengurangi kasus dan kematian akibat pneumonia pada anak-anak. Rumah Sakit Umum Dr. Moewardi, rumah sakit rujukan di Surakarta, menangani banyak kasus pneumonia anak. Namun, munculnya pandemi COVID-19 membawa tantangan baru, termasuk kebutuhan untuk menyesuaikan prosedur perawatan kesehatan dan prioritas layanan, yang kemungkinan mempengaruhi penanganan kondisi non-COVID seperti pneumonia. Penelitian ini bertujuan untuk membandingkan efektivitas pengobatan pneumonia pada anak sebelum dan selama pandemi, untuk mengevaluasi apakah pandemi tersebut berdampak positif atau negatif terhadap kualitas perawatan. Metode kuantitatif deskriptif dengan pendekatan cross-sectional digunakan untuk menganalisis perbedaan kasus pneumonia anak di RS Dr. Moewardi antara tahun 2019 dan 2021. Hasilnya mengungkapkan pergeseran penting dalam karakteristik dan pola insiden pneumonia anak. Pada tahun 2019, kasus lebih sering, terutama pada anak-anak di bawah lima tahun. Sebaliknya, tahun 2021 melihat lebih sedikit kasus tetapi proporsi anak yang lebih tua yang terkena dampak lebih tinggi. Selanjutnya, jumlah pasien yang dirawat di PICU menurun, sementara lebih banyak anak dirawat di bangsal umum, HCU, dan NICU. Perubahan signifikan juga diamati dalam kultur bakteri. Pada tahun 2019, Acinetobacter baumannii dan Klebsiella pneumoniae dominan, tetapi pada tahun 2021, prevalensinya menurun dan Pseudomonas aeruginosa muncul sebagai patogen baru. Temuan ini menunjukkan bahwa pandemi COVID-19 tidak hanya memengaruhi pola pengobatan tetapi juga etiologi pneumonia pada anak-anak, kemungkinan karena perubahan protokol kesehatan, penggunaan antibiotik, dan tindakan pengendalian infeksi.
