Abstract

A B S T R A C TTechnetium-99m ( 99m Tc) has been applied in nuclear medicine as a radiopharmacyfor both diagnosis and therapy. 99m Tc is obtained from a 99 Mo/ 99m Tc (half-life 66 h)generator in the form of sodium pertechnetate (Na[ 99m TcO 4 ]) by decaying to 99 Tc for 6hours and emitting gamma energy rays (Eɤ = 140 keV). This radionuclide has anelectron configuration of 4d 5 5s 2 , which will form complexes with different ligandsand have oxidation rates from +1 to +7. The coordinated complex of technetium-99mhas been utilized in nuclear medicine in tissues and organs (thyroid, red and whiteblood cells, kidneys, brain, myocardial, and bone). The resulting kit production musthave based on Good Manufacturing Practice, which consists of batch planning,washing, sterilization of glassware and stopper, starting material, preparation oflarge quantities of the solution, sterile filtration, dispensing, crimping, a summary ofprocess control, quarantine, packaging and leaving the production premises.

Abstract

Buah Merah, a typical fruit from Papua, Indonesia which is used empirically in cancer therapy is rich in carotenoids and flavonoids. However, the mechanisms by which Buah Merah ameliorates cancer remained unknown. Natural antioxidant enzymes and pro-oxidant enzymes modulation significantly suppressed ROS production and cancer growth. Therefore, the determination of target enzymes of Buah Merah contents was studied through an in silico approach. Carotenoid and flavonoid compounds from Buah Merah were docked to 7 ROS modulating enzymes using Autodock Vina and the interaction stability was studied using the CABS Flex 2.0 server. The crucial amino acids of each enzyme were determined using DockFlin and prediction of acute oral toxicity of each test ligand was studied using ProTox-II. Based on the molecular docking results, quercetin 3'-glucoside is the most potent compound in binding to CAT, GR, GPx, SOD, LOX, and NOX with binding energy values of -11.2, -9.7, -8.6, -10.2, -10.7, and -12.8 kcal/mol, respectively. Meanwhile, taxifolin 3-O-α-arabinopyranose produced the highest binding affinity of -10.0 kcal/mol at the XO. Each test ligand formed stable interactions with ROS modulating enzymes and formed bonds with crucial amino acids resulting in strong adhesion compared to native and reference ligands. The glucoside group of quercetin 3'-glucoside plays an essential role in determining the proper position in the attachment and supports the formation of hydrogen bonds with receptors. With low acute oral toxicity, it can be concluded that quercetin 3'-glucoside from Buah Merah is a potent oxidative stress modulator in cancer prevention and therapy.

Abstract

Kipas laut (Gorgonia mariae) telah digunakan masyarakat Maluku secara turun temurun sebagai obat asma. Kandungan metabolit sekunder yang paling dominan dalam kipas laut adalah sterol, dimana memiliki aktivitas terapi melalui efek sinergisme antara senyawa metabolit dengan polivalent activity. Pengujian kipas laut sebagai anti-asma belum pernah dilaporkan sebelumnya. Oleh karena itu, perlu dilakukan virtual screening menggunakan metode in silico pada komponen sterol kipas laut sebagai tahap awal dalam menentukan efektivitas terapi anti-asma dengan memprediksi nilai ikatan energi bebas (ΔG), konstanta inhibisi (Ki), dan interaksi residu asam amino menggunakan Autodock Tools 4.2 dan Discovery Studio 2016 Client®. Keamanan dan efektivitas kandidat obat dievaluasi menggunakan parameter dari Lipinski Rule of Five dan pre-ADMET. Hasil penelitian menunjukkan bahwa konstanta inhibisi dan ikatan energi bebas (Ki; ΔG) dari komponen senyawa kipas laut dapat diurutkan secara potensial yaitu 4,24-dimetil kolestanol  (0,809; -12,40) > 24-metil-22-dehidrokolesterol (0,864; -12,36) > 23-demetil gorgosterol (1,74; -11,95) > 4,24-dimetil-22-dehidrokolestanol (1,89; -11,90). Residu asam amino yang berperan penting dalam aktivitas inhibisi hCHIT1 adalah 213-ASP. Semua komponen senyawa uji memiliki nilai log P lebih dari 5 yang menunjukkan bahwa kelarutan dan toksisitas perlu diperhatikan. Evaluasi distribusi pre-ADMET berdasarkan nilai dari pengikatan protein plasma menunjukan bahwa senyawa uji dapat berdifusi menembus membran plasma dan berinteraksi sesuai target farmakologi. Selain itu, hasil parameter uji toksisitas menunjukkan bahwa senyawa 23-demetil gorgosterol dan 4,24-dimetil-22-dehidrokolestanol memiliki potensi sebagai anti-asma.Virtual Screening of the Compounds in Gorgonians (Gorgonia mariae) as anti-asthma. The people of Maluku have used Kipas laut (G. mariae) for generations as an asthma medicine. The secondary metabolite that is most dominant in kipas laut is sterols, which have therapeutic activity through the synergistic effect between metabolite compounds and polyvalent activity. Anti-asthma activity of kipas laut has never been reported. Therefore, it is necessary to do virtual screening using the in silico method on the sterol component of kipas laut as a first step in determining the effectiveness of anti-asthma therapy by predicting the value of free energy bonds (ΔG), constant inhibition (Ki), and interactions of amino acid residues using Autodock Tools 4.2 and Discovery Studio 2016 Client®. The effectiveness and safety of prospective drugs are evaluated using the Lipinski Rule of Five and pre-ADMET. The results showed that the value of inhibition constants and free energy bonds (Ki; ΔG) on the compound of kipas laut that was potentially sorted was 4.24-dimethyl cholestanol (0.809; -12.40) > 24-methyl-22-dehydrocholesterol (0.864; -12.36) > 23-demethyl gorgosterol (1.74; -11.95) > 4.24-dimethyl-22-dehidrokolestanol (1.89; -11.90). The crucial residues of amino acids is 213-ASP, which play a significant role in hCHIT1 inhibitory activity. All components of the test compound have a log P value of more than five, which indicates that solubility and toxicity need to be considered. Evaluation of the pre-ADMET based on the value of plasma protein binding shows that the test compound can diffuse through the plasma membrane and interact according to pharmacological targets. In addition, the results of the toxicity test showed that 23-demethyl gorgosterol and 4.24-dimethyl-22-dehidrokolestanol compounds have potential as anti-asthma.

Abstract

Spesies dari genus Caesalpinia sebagian besar berupa spesies berkayu pada zona tropis dan subtropis. Caesalpinia pulcherrima (L.) Sw. adalah salah satu spesies dari genus Caesalpinia yang biasa dikenal dengan bunga kembang merak. Artikel review ini bertujuan untuk menjelaskan profil senyawa dan aktivitas farmakologi dari C. pulcherrima. Metode pencarian data untuk menyusun artikel review ini adalah penelusuran pustaka dari berbagai sumber jurnal serta buku, meliputi profil tanaman, profil senyawa, serta aktivitas farmakologi dari C. pulcherrima. Metabolit sekunder yang terdapat pada genus Caesalpinia diantaranya diterpenoid, fenolik, flavonoid, triterpenoid, dan lakton. C. pulcherrima mengandung senyawa diterpenoid,; pulcherrimin A dan isovouacapenol C (akar), 6-methoxypulcherrimin, peltogynoids bhonducellin, dan homoisoflavonoid (batang), myricetin, lupeol, flavonoid dan β-sitosterol (bunga), tanin, asam hidrosianat, serta asam benzoat (daun). Isolat pada buah C. pulcherrima mengandung senyawa β-lakton. Aktivitas farmakologis Caesalpinia meliputi anti-mikroba, anti-inflamasi, anti-kanker, anti-virus, dan anti-jamur. Aktivitas antibakteri C. pulcherrima dapat diuji dengan cara mengektraksi serbuk kering aerial parts dengan metode perkolasi dingin. Kemudian, dilakukan evaluasi dengan KLT  dan penentuan Minimal Inhibitory Concentration (MIC) dan Minimal Bactericidal Concentration (MBC). Fraksi 9 dan 10 sangat efektif terhadap Klebsiella pneumoniae. Caesalpinia, terutama C. pulcherrima, memiliki beraneka aktivitas farmakologi yang kemungkinan besar terkait dengan dengan adanya gugus metoksi karbonil dan hidroksi pada struktur metabolit sekundernya.

Abstract

Ethnobotanically, sea fan was a group of gorgonian coral that have used the Maluku people as medicinal ingredients with secondary metabolites containing sterols, terpenoids, and alkaloids that have anti-inflammatory, anti-viral, anti-cancer, analgesic, gastroprotective, anti-bacterial, anti-cancer, and anti-fouling agents. However, the effectiveness of sterols as anti-viral SARS-CoV-2 has not been reported, so research was needed. The initial stage of targeting (SwissTargetPrediction), binding affinity (Autodock Tools 4.2), and amino acid interactions (Discovery Studio 2016 Client®). The route of administration, pharmacokinetic properties, and acute oral toxicity (LD50) were predicted by Lipinski's rule of five, pre-ADMET, and ProTox-II. The results of target class obtained probability of 10.6% (4,24-dimethyl cholesta-7,22-dien-3ß-ol and 4,24-dimethyl-22-dehydro-cholestanol) and 11, 8% (dinosterol). Binding affinity (ΔG, kcal/mol and Ki, nM) potentially 4,24-dimethyl cholesta-7,22-dien-3ß-ol (-9.90; 55.13) > dinosterol (-9.77; 68.66) > 4,24-dimethyl-22-dehydro-cholestanol (-9.48; 113.33), respectively with the crucial amino acid, Asp187. The test compound has a log P value > 5, so solubility must be considered. Pre-ADMET showed an excellent disposition as a drug and was not mutagenic and carcinogenic. However, the distribution of plasma proteins and the dose of LD50 need to be considered. Thus, sea fan sterols have potential as MPro protein inhibitors.

Abstract

Pharmaceutical services are considered by some observers to be below standard. Both pharmacists and pharmaceutical technicians as health technicians are required to improve their skills, knowledge and behavior in order to provide quality direct services to patients. The purpose of this study was to determine the implementation of pharmaceutical services at Health Centers in Pandeglang Regency in 2023. This study is an observational study with a case study design. The method used is descriptive using checklist sheets and open statements. Based on the results of this study, there are significant differences in the aspects of facilities and infrastructure and clinical pharmacy services and there are differences but not significant in the aspects of managing pharmaceutical preparations and disposable medical materials. The conclusion of this study is that pharmaceutical services at Health Centers in Pandeglang Regency are in accordance with Permenkes No. 74 of 2016 by obtaining a percentage value of 79%, entering the GOOD category.

Abstract

Over time, advancements in the medical field have led to a decline in public trust in herbal medicine due to the fast-acting effects of chemical drugs. However, herbal medicines generally have fewer side effects. Public awareness of health remains low, particularly in Kamanisan Village, largely due to limited knowledge. A comprehensive strategy is needed to enhance public awareness and understanding of medicinal plants. This program aims to increase public knowledge and awareness of the use of medicinal plants. The activity took place in RT.07/RW.13 Kamanisan, Curug District, involving 30 participants. It included a pre-test questionnaire, material presentation, Q&A session, and post-test. The results showed an increase in participant knowledge from 86.76 percent to 90 percent. The number of participants who strongly agreed with the use of medicinal plants (in Indonesia: TOGA) rose from 12 to 22 people. The Chi-Square test indicated a significant impact on knowledge and attitudes before and after the session. Additionally, the paired sample t-test confirmed differences in pre- and post-knowledge, pre- and post-attitude, and knowledge-to-attitude shifts. The success of this activity strengthens public awareness of TOGA as a complementary and alternative treatment for various diseases.
