Abstract

Hemodialysis has a therapeutic effect on patients with chronic renal failure. However, on the other hand, it can also cause acute complications that can negatively affect the quality of life. Acute complications can be prevented by changing the lifestyle of hemodialysis patients, one of which is by using audio-visual aids. This study aims to determine the effect of audio-visual aids on the patient's level of knowledge about dealing with acute complications of hemodialysis. This study used a quasi-experimental design with pre and post-tests without a control group with 30 hemodialysis patients who met the inclusion criteria as respondents. This study indicates a significant effect between the use of audio-visual aids and the level of knowledge related to the management of acute complications in hemodialysis patients (p-value= 0,000). Therefore, nurses can use audio-visual aids as a means of education to deal with acute complications of hemodialysis.

Abstract

Chronic Renal Failure (CRF) is a decrease in irreversible kidney function characterized by uremia which affects patients' biological, psychological, social and spiritual aspects. This change certainly requires good adaptation of CRF patients. Nurses can help patients adapt to changes in their level of health. The role of nurses is needed in the provision of nursing care using Roy's adaptation theory approach which is applied in the case of Mr. F, 32 years old with CRF, diabetes mellitus, diabetic retinopathy. The purpose of writing this case is to gain real experience in providing nursing care to CRF patients using Roy's adaptation theory approach. Nursing diagnoses that appear in cases of excess fluid volume are associated with decreased kidney function, fluid and sodium retention; decreased cardiac output is associated with increased cardiac workload; nutritional imbalances less than body requirements related to insufficient insulin; constipation is associated with a decrease in physical activity. The results achieved after carrying out nursing care with Roy's adaptation theory approach are edema has diminished, excess fluid volume is partially overcome; decreased cardiac output has not been resolved; blood sugar is still unstable with the help of insulin, partial deficit nutrition; partial constipation. Conclusion: Nursing care with a theory approach Roy's adaptation model is quite effective to be applied to patients with urinary system disorders, especially in cases of chronic renal failure where patients must adapt to changes that occur both biologically, psychologically, socially and culturally.

Abstract

Diabetes mellitus (DM) is a group of metabolic disorders characterized by increased blood glucose levels (hyperglycemia) due to damage to insulin secretion, insulin action or both. Diabetes mellitus can cause complications in hypoglycemia and diabetic ketoacidosis, while chronic complications can include coronary heart disease, cerebrovascular disease, peripheral vascular disease, diabetic retinopathy, diabetic neuropathy and diabetic nephropathy. The client is named Mr. R, male, aged 25 years with a medical diagnosis of type 2 diabetes mellitus. After the assessment, a nursing diagnosis was obtained, namely ineffective airway clearance associated with retained secretion, the risk of fluid volume deficiency associated with excess gastric loss: diarrhea, risk of changes in nutrition less than the body's needs associated with nausea, insufficiency of insulin, acute pain associated with hyperperistaltic and high risk of spread of infection to sepsis associated with high glucose levels, effects of invasive procedures: infusion installation, pre-existing respiratory infections. The results achieved after performing nursing care are three nursing diagnoses that have been overcome ie ineffective airway clearance associated with retained secretions, the risk of lack of fluid volume associated with excess gastric loss: diarrhea and acute pain associated with hyperperistaltic. In addition, there is 1 (one) nursing diagnosis that is partially resolved, namely the risk of changes in nutrition less than the body's needs related to nausea, insufficiency of insulin and there is 1 (one) nursing diagnosis that has not been resolved, namely the high risk of spread of sepsis associated with high glucose levels, effects invasive procedure: infusion installation, respiratory infections that existed before.

Abstract

Chronic Kidney Failure (CRF) is a progressive and irreversible renal function disorder in which the body fails to maintain metabolism, fluid and electrolyte balance, causing uremia. Hemodialysis is an attempt or an act of cleansing the blood of toxic substances that cannot be excreted by the kidneys from the body. The impact of chronic kidney failure if not treated immediately is hypertension, anemia, pulmonary edema, cardiac tamponade, proteinuria, hematuria, decrease in white blood cells, bleeding disorders, fetor uremic, renal osteodystrophy, psychotic symptoms of convulsions and coma, bruising, excoriation, irregular menstruation . The purpose of writing is expected that students can gain real experience in providing nursing care to clients with chronic kidney failure on HD. The research method is descriptive method and library study. The results of this scientific paper are students gain real experience about providing nursing care to clients with chronic kidney failure on HD. Nursing problems in the form of ineffective airway cleaning, ineffective breathing patterns, risk of damage to gas exchange, high risk of decreased cardiac output, excess fluid volume, risk of nutritional disorders less than body requirements, risk of infection.

Abstract

Aims: Chronic kidney diseases effects make the pruritus prevalence in hemodialysis patients relatively high. Several studies have been conducted to determine the effect of acupressure on complaints of uremic pruritus. However, it is still debatable how acupressure relief pruritus complains regarding the duration of acupressure. This study aims to identify the effectiveness of acupressure against pruritus based on previous studies. Materials and Methods: An electronic literature search was conducted to identify studies that matched the inclusion criteria. The electronic databases used in this research are Wiley online library, Clinical key, ProQuest, EBSCO, and Google Scholar. The keyword used was “acupressure”, “pruritus”, “itch”, and “hemodialysis”. Results: We found 1,983 articles based on keywords, and five articles met the inclusion criteria after conducting the article selection process. Three of the four experimental studies measured pruritus using a visual analog scale, and one used a numerical scale. One study applied auricular acupressure, and four studies used acupressure. Four studies have shown that acupressure effectively reduces the scale of pruritus. One study showed no significant difference between acupressure, sham control, and negative control groups in two weeks of intervention in the severity of pruritus (p=0.66). Conclusion: Acupressure is effective in treating uremic pruritus and nurse can applied acupressure as a non-pharmacological intervention for pruritus in hemodialysis patients.
