Abstract

Introduction: Acute hypercapnic respiratory failure (AHRF) in patients with obesity hypoventilation syndrome (OHS) presents significant challenges in the intensive care unit (ICU), particularly when complicated by comorbidities such as community-acquired pneumonia and heart failure. Effective management requires a tailored approach addressing altered lung mechanics, infection control, and fluid balance. This case report highlights the multidisciplinary management of AHRF in a complex clinical scenario. Case Description: A 35-year-old female with OHS presented with progressive dyspnea for one month, worsening over the last two days. Initial assessment revealed type II respiratory failure with a pCO2 of 89 mmHg. Management included intubation and mechanical ventilation, initially with pressure control-assist control (PC-AC) mode, gradually transitioned to pressure support ventilation (PSV). Empirical antibiotics were administered, later adjusted based on sputum culture results. Fluid management involved furosemide to address concurrent heart failure. Adequate positive end-expiratory pressure (PEEP) was crucial to optimize lung mechanics. Despite an initial failed extubation, the patient was successfully extubated on day 7 with high-flow nasal cannula support and subsequently transferred to a step-down unit with nasal cannula oxygen therapy. Conclusion: This case underscores the importance of a multidisciplinary approach and dynamic therapy adjustments based on clinical response in managing AHRF in OHS patients with complex comorbidities. Early broad-spectrum antibiotics, careful fluid management, and gradual weaning from mechanical ventilation are critical for successful outcomes.

Abstract

According to UNESCO, around 258 million children and adolescents worldwide lacked access to education in 2020, most of whom are in developing countries. In Indonesia, while cities like Bogor have good access to education, villages such as Benteng in Bogor Regency still face challenges in educational development, especially in subjects such as English, Mathematics, Social Studies (IPS), and Digitalization. This community service program aims to enhance education quality through interactive learning approaches, including four main programs: ELEVEN (English Learning and Vocabulary ENhancement), Back2Math (Back to Basic Math), Jelajah Duniaku, and DigiKidz. The program employs hands-on training methods, educational games, and game card method to enrich vocabulary, mathematical concepts, social insights, and digital skills. The results show significant improvement in students' academic abilities across all subjects taught. This program also produces learning modules that can be used sustainably by local teachers and communities.

Abstract

Micro, Small, and Medium Enterprises (MSMEs) in Indonesia continue to encounter various challenges such as limited capital, marketing barriers, and management issues, particularly in human resource management. Compensation serves as a crucial factor influencing employee performance and motivation. It is mandatory for every business in Indonesia to formulate a Pay Structure. This research aims to evaluate the Pay Structure of XYZ Catering MSME in Denpasar, Bali based on the 3P principle (Pay for Position, Person, and Performance). The data utilized comprises primary data obtained through in-depth interviews with business managers, supplemented by secondary data sourced from business websites and relevant literature. Job classification is conducted through job value analysis employing pairwise comparison based on 10 compensation factors assessed by business managers. Salary mapping is executed using a simple ranking method with adhered and overlapping double scales. Salary mapping with the adhered method yields a total salary increase of Rp10,218,890 from the initial total salary, whereas overlapping method result in a total salary increase of Rp5,789,746. Based on the analysis, the rectification of pay structure utilizing the overlapping simple ranking method is chosen due to its greater flexibility and offers pay structure improvement that aligns with the company's capabilities.

Abstract

Latar Belakang dan Tujuan: Operasi tumor otak berhubungan erat dengan risiko perdarahan dalam jumlah besar yang dapat menyebabkan anemia. Efek klinis anemia dapat diperbaiki dengan pemberian transfusi darah. Transfusi diberikan dengan target level Hemoglobin (Hb) antara 9 sampai 10 gr/dL. Tujuan penelitian ini adalah untuk mengetahui gambaran kadar Hb dan hematokrit prabedah dan pascabedah, jumlah perdarahan serta pemberian transfusi darah pada pasien yang menjalani operasi tumor otak di RSUP Dr. Hasan Sadikin Bandung periode Juni 2015 sampai dengan Juni 2016.Subjek dan Metode: Penelitian ini merupakan penelitian deskriptif observasional yang dilakukan secara retrospektif terhadap 126 objek penelitian yang diambil di bagian rekam medis.Hasil dan Simpulan: Penelitian ini memperoleh hasil kadar Hb prabedah rata-rata sebesar 13,231,35 gr/dL dan hematokrit prabedah rata-rata sebesar 39,193,54%. Kadar Hb pascabedah 9 gr/dL sebanyak 15 pasien, Hb 910 gr/dL sebanyak 6 pasien dan Hb 10 gr/dL sebanyak 105 pasien. Hematokrit pascabedah rata-rata sebesar 34,036,03%. Jumlah perdarahan rata-rata sebesar 11591032,66cc. Transfusi yang diberikan pada 56 pasien terdiri atas PRC dengan jumlah rata-rata sebesar 365,81258,70cc, FFP rata-rata sebesar 425,45274,78cc dan WB 250cc.Hemoglobin Levels, Blood Loss and Transfusionin Patients Underwent Brain Tumor Surgery atDr. Hasan Sadikin Bandung General Hospital During 20152016Background and Objective: Brain tumor surgery is closely related to the risk of numerous bleeding that can cause the patient to be in an anemic condition. The clinical effects of anemia can be improved by administered blood transfusions. Transfusion can be administered with target Hemoglobin (Hb) level between 9 to 10 gr/dL.The purpose of this study was to describe of preoperative and postoperative levels of Hb and hematocrit, blood loss and how blood transfusion administered in patients undergoing brain tumor surgery at Dr. Hasan Sadikin Bandung during June 2015 to June 2016.Subject and Method: This is a descriptive observational study with retrospective approach to 126 objects taken at medical records.Result and Conclusion: The average of preoperative Hb level was 13,231,350 gr/dL and the average of preoperative hematocrit level was 39,193,54%. Number of patients with postoperative Hb level 9 gr/dL were 15 patients, Hb 9-10 gr/dL were 6 patients and Hb10 gr/dL were 105 patients. The average of postoperative Ht were 34,036,032%. The rate of blood loss was 11591032,66cc. The rate of transfusions administered to 56 patients was pack red cell 365,81258,70cc, fresh frozen plasma 425,45274,78cc and whole blood 250cc.
