Abstract

Fluency in communication cannot be separated from the influence of presuppositions. The presupposition is something that the speaker assumes as an event before the utterance. This study aims to identify and describe the types of presuppositions and common ground in a question-and-answer session between journalists and President Joe Biden at a press conference of Joe Biden's speech denouncing Russia's invasion of Ukraine. To meet this purpose, the researchers applied George Yule's theory (1996) and utilized the descriptive method with a notetaking technique as a data collection technique where the data source is in the form of a video. The results showed 5 types of presuppositions in 26 presuppositions data from 17 speeches data analyzed.

Abstract

Introduction: The SARS-CoV-2 epidemic is sweeping the world with several waves of infections continuing. In some COVID-19 patients, this condition can develop into ARDS (acute respiratory distress syndrome) which requires ICU care with a mortality rate of 50-65 and the need for mechanical ventilation reaches 97%. The aim of this research was to know characteristics of COVID-19 patients to predict mortality in the Negative Pressure Isolation Room COVID 19 ICU at H. Adam Malik General Hospital Method: Retrospective research method with secondary data sources of COVID-19 Negative Pressure Isolation Room ICU (RITN) patients at RSUP H. Adam Malik Medan for the period May 2021 – July 2021.The data collected was in the form of patient data including name, gender, age, medical record number, comorbidities, NLR, D-Dimer, Fibrinogen, PF ratio, use of oxygen supplementation, and length of ICU stay. Chi Square statistical analysis was used for categorical data, while the T-test or Mann Whitney was used for numerical data. Then between the variables an ANOVA test was carried out to assess the comparison of clinical characteristics with mortality. Results: The results of this study indicate a higher mortality rate in women41 people (57.7%), 56-65 years age group 26 people (36.6%), patients using ventilators 51 people (71.8%), comorbid hypertension 33 people (46.5%) ), Moderate PF Ratio were 44 people (62%) and Length of stay <10 days were 55 people (77.5%). The mean fibrinogen value in COVID-19 patients who died was 658.96 ± 674.98, as well as the D-dimer value of 658.96 ± 674.98 and NLR of 11.06 ± 3.23 which showed a significant increase compared to patients Survivors of COVID-19 (p<0.05). Conclusion: A significant relationship was found between gender, age, breathing apparatus, D-dimer, Fibrinogen, NLR and length of stay with mortality of COVID-19 patients treated in the COVID Negative Pressure Room (RITN) ICU

Abstract

This research aims to analyze the type, word formation, function, lexical meaning and contextual meaning of the use British and Jamaican English slangs as identity markers in the film Blue Story (2019) by Andrew Onwubolu. The method used in this research is descriptive qualitative method. This research focuses on British and Jamaican English slangs in the dialog of the central character and supporting characters in the film. This research finds out forty data of British and Jamaican English slangs in the movie, but in this discussion the data will be explain ten data of slangs. The type of imitative or word formation dominantly appears in the film. The function of slang language in the movie is mostly used as an identity marker that the speakers are group of Jamaican and British descent. In addition, slang is used as a form of self-expression in negative terms and mocking others because of the conflict between gangs.

Abstract

Background: A prognostic predictive value in patients with sepsis and septic shock is needed to determine the aggressiveness of the therapy we will choose. Various methods are used to predict patient mortality. Based on this, a marker is needed to assess tissue perfusion disorders.Objectives: To determine the relationship between the P(v-a)CO2/C(a-v)O2 ratio and lactate clearance as a marker of the success of the one-hour bundle in sepsis patients.Methods: This study is a descriptive analytic study of 40 patients who assessed the description of lactate, ScvO2, and PvaCO2 gap values in septic patients. The study was conducted at the Emergency Department (IGD) and Intensive Care Unit (ICU) of H. Adam Malik Hospital Medan, during the period of April to May 2023. The collected data were then analyzed using descriptive statistical methods to calculate the mean, standard deviation, and range of the variables studied. The differences between groups were analyzed using the t-test to compare the mean values of the parameters studied. Correlation analysis was conducted using Pearson’s correlation test to assess the relationship between lactate, ScvO2, and PvaCO2 gap.Results: At T0, the mean lactate level was 22,06 mmol/L (median 16,85 mmol/L), the mean P(v-a)CO2 was 9,28 mmHg (median 8,50 mmHg), and the mean P(v-a)CO2/C(a-v)O2 ratio was 3,66 (median 3,56). At T1, the mean lactate level decreased to 12,52 mmol/L (median 9,00 mmol/L), the mean P(v-a)CO2 was 8,50 mmHg (median 7,50 mmHg), and the mean P(v-a)CO2/C(a-v)O2 ratio was 3,45 (median 2,57). The average lactate clearance was 48,29%. Bivariate analysis revealed a significant relationship between lactate clearance and the P(v-a)CO2/C(a-v)O2 ratio, with a P-value of 0,009 and a Spearman correlation of -0,521, indicating a strong negative correlation between the two variables.Conclusion: There is a significant relationship between the P(v-a)CO2/C(a-v)O2 ratio and lactate clearance in the success of the one-hour bundle for sepsis patients.
