Abstract

Abstrak: Tidur merupakan salah satu kebutuhan dasar manusia yang memiliki fungsi penting bagi tubuh. Kualitas tidur yang buruk sangat mempengaruhi kualitas hidup pada orang dewasa, dan menimbulkan gangguan gastrointestinal fungsional, termasuk dispepsia fungsional. Dispepsia fungsional mengacu pada ketidaknyamanan di perut bagian atas. Kurang tidur juga sangat mendorong perkembangan gejala dispepsia fungsional. Penelitian ini bertujuan untuk mengetahui adanya hubungan antara kualitas tidur dengan kejadian dispepsia fungsional pada mahasiswa tingkat akhir Fakultas Kedokteran Universitas Muhammadiyah Sumatera Utara. Jenis penelitian ini adalah observasional analitik dengan desain cross-sectional. Sampel yang digunakan yaitu mahasiswa tingkat akhir Fakultas Kedokteran Universitas Muhammadiyah Sumatera Utara yaitu stambuk 2019 berjumlah 135 sampel dengan menggunakan teknik purposive sampling. Pengumpulan data dengan mengisi kuesioner dan analisis data menggunakan uji chi-square. Hasil analisis bivariat antara kualitas tidur dengan dispepsia fungsional dengan uji chi square diperoleh bahwa P = 0,01 (p0,05). Terdapat hubungan yang signifikan antara kualitas tidur dan dispepsia fungsional pada mahasiswa tingkat akhir Fakultas Kedokteran Universitas Muhammadiyah Sumatera Utara.

Abstract

ABSTRACTIntroduction: Hypertension is a manifestation of hemodynamic balance disorders in the cardiovascular system. Isolated systolic hypertension was defined as systolic blood pressure ?140 mmHg and diastolic blood pressure ?90 mmHg. Objective: The purpose of this study was to analyze the effect of family history, obesity, smoking, physical activity and consumption of salt and fat on the incidence of isolated systolic hypertension in the elderly at the Geriatrics Clinic of Haji Hospital Medan. Method: The design of this study was an analytic observasional study with a case control approach using a questionnaire from patients at the Geriatrics Polyclinic, Haji Hospital Medan. The sampling technique used was purposive sampling. Results: From 90 samples, it was found that there was an effect of family history (p-value=0,003; OR=3,62; CI 95% = 1,51-8,65), smoking (p-value=0,035; OR=2,47; CI 95% = 1,05-5,76), physical activity (p-value=0,010; OR=3,07; CI 95% = 1,28-7,36), and consumption of salt and fat (p-value=0,033; OR=2,53; CI 95% = 1,07-5,98) on the incidence of isolated systolic hypertension in the elderly and there is no effect of obesity on the incidence of isolated systolic hypertension (p-value=0,827; OR=1,10; CI 95% = 0,46-2,59). The results of multivariate analysis showed that the most dominant variable influencing the incidence of isolated systolic hypertension was family history (p-value = 0,023; Exp ? =0,347). Conclusion: There is an effect of family history, smoking, physical activity, and consumption of salt and fat on the incidence of isolated systolic hypertension in the elderly and there is no influence of obesity with the incidence of isolated systolic hypertension in the elderly. The most dominant variable influencing the incidence of isolated systolic hypertension in the elderly is family history.Keyword : Elderly, Isolated Systolic Hypertension, Risk Factors

Abstract

Pituitary tumors are benign tumors that occur in the pituitary gland with a presentation of 10% of all intracranial tumors. If the size of the tumor is 1 cm, the pituitary tumor is called a macroadenoma, the large size will have a mass effect so that there is an emphasis on the tumor area and its surroundings. This emphasis will manifest in hyposecretion of the hormones produced in the pituitary. We report a case of 57-year-old male with complaints of decreased vision since 3.5 months. The patient had previously been diagnosed with a pituitary tumor 6 years ago and had surgery. After diagnostic steps, the patient is diagnosed with a residual pituitary adenoma tumor with hypocortisol and hypothyroidism. Hormone replacement therapy was performed by administering methilprednisolone tablets 16 mg-0-8 mg and levothyroxine tablets 1 x 50 g. The patient was planned to undergo an Endoscopic Endonasal Excision Tumor surgery with 125 mg/12 hour preoperative injection of methilprednisolon. The provision of preoperative hormone replacement therapy aims to correct hormonal imbalances that can be an intraoperative complication

Abstract

Sugar sweetened-beverages (SSB) is not only found in soft drinks, fruit juice drinks, or energy drinks, but also various drinks that are currently "trending" such as boba drinks with various flavors, coffee or milk coffee, even tea or milk tea with various flavors. The lifestyle of consuming drinks that are high in sugar will lead to a tendency to several diseases such as prediabetes, diabetes mellitus (DM), metabolic syndrome and cardiovascular disease. Prevention and screening of prediabetes and DM at a young age in Indonesia must be increased, especially in high-risk groups because most of the initial conditions of the disease are asymptomatic. The aim of this study is to analyze the association between frequency of sugar sweetened-beverages (SSB) consumption and prediabetes in medical students. This study was an observational analytic study with cross sectional design conducted at Medical Faculty Universitas Muhammadiyah Sumatera Utara. The subjects of this study was 70 subjects. Examination of frequency of SSB consumption was carried out with a validated questionnaire and food frequency questionnaire (FFQ) of nutrient intake for the past 24 hours. There is no significant relationship between gender, family history of DM, smoking history, physical activity and hypertension with prediabetes. However, there is a significant relationship between obesity and prediabetes with p value 0,010. Meanwhile, the relationship between frequency of SSB consumption and prediabetes showed with p 0.05. There is no significant relationship between frequency of SSB consumption and prediabetes, but there is significant relationship between obesity and prediabetes.

Abstract

Systemic lupus erythematosus (SLE) is a chronic autoimmune inflammatory disease that can damage organs and cells due to autoantibodies and immune complexes bound to tissue. Infection is a major source of co-morbid in patients with SLE. The infection has been the second major cause of death in SLE, after disease activity itself. Perianal abscess is part of anorectal abscess which can occur in patients that have a risk factor of trauma on the anorectal region. Management of the patient consists of non-operative and operative procedures. In SLE patients that undergo the operative procedure, it is necessary to do perioperative preparation to prevent post-operative complications such as "flare-up" condition due to SLE. Evaluation of medicines and adjustment of glucocorticoid dosage based on the risk of surgery is important in perioperative preparation.

Abstract

 Diabetes mellitus is a complicated necessitates ongoing medical attention and is a serious health problem that has grown to worrisome proportions. The study aimed to assess the avocado pulp extract’s glycemic control in individuals with type 2 diabetes mellitus (T2DM) in a clinical setting in 2023. From the Klinik IMAN database, patients 18 years with T2DM who had previously been treated with oral hypoglycemic medications were included. The index date—the first day that avocado pulp extract was consumed—was used to collect baseline data. A laboratory measurements analysis was used to evaluate the change in glycated hemoglobin (HbA1c) and fasting blood glucose levels after a month of consuming avocado pulp extract and placebo. At one month, the percentage of patients who achieved their goal HbA1c and fasting blood glucose levels was determined. Overall, 30 patients were included and had baseline HbA1c and fasting blood glucose levels values available. Added avocado flesh extract not associated with improved HbA1c (0.04 [95% CI: -0.22-0.30], p0.05) and fasting blood glucose levels values (0.85 [95% CI: - 24.7-26.4], p0.05) versus baseline at a month. Added avocado flesh extract not associated with improved glycemic control during a month of follow-up in patients with T2DM.

Abstract

Background: Prediabetes merupakan kondisi awal gangguan metabolik yang ditandai dengan peningkatan kadar glukosa darah di atas normal namun belum memenuhi kriteria diabetes melitus. Deteksi dini terhadap prediabetes menjadi langkah penting dalam mencegah progresivitas penyakit menuju diabetes tipe 2. Di sisi lain, konsumsi kopi yang merupakan kebiasaan umum di kalangan masyarakat, termasuk pegawai perguruan tinggi, memiliki pengaruh terhadap metabolisme glukosa dan sensitivitas insulin, baik secara positif maupun negatif tergantung pada pola konsumsinya. Metode: Kegiatan pengabdian masyarakat ini bertujuan untuk mengidentifikasi status prediabetes pada pegawai Universitas Muhammadiyah Sumatera Utara (UMSU) melalui pemeriksaan glukosa darah puasa, serta memberikan edukasi tentang konsumsi kopi yang sehat dan berbasis bukti ilmiah. Metode yang digunakan meliputi penyuluhan interaktif dan pemeriksaan gula darah secara langsung menggunakan teknik spektofotometri kepada 48 orang pegawai UMSU. Hasil: Hasil kegiatan menunjukkan bahwa sebagian peserta memiliki kadar glukosa darah yang berada dalam rentang prediabetes, serta sebagian besar belum mengetahui hubungan antara konsumsi kopi dan kesehatan metabolik. Edukasi yang diberikan meningkatkan pemahaman peserta tentang pentingnya konsumsi kopi secara moderat dan disesuaikan dengan kondisi kesehatan masing-masing. Kesimpulan: Kegiatan ini dapat meningkatkan kesadaran serta mendorong perubahan gaya hidup sehat pada kalangan pegawai perguruan tinggi.

Abstract

Prediabetes is a condition characterized by glucose or HbA1C levels that do not meet the criteria for diabetes but have abnormal carbohydrate metabolism that results in abnormal glucose levels between normoglycemia and diabetes. Early screening in young adults is an effort to prevent progression to diabetes mellitus. On the other hand, coffee consumption habits in young adults, including office workers, affect glucose metabolism and insulin sensitivity. This activity aims to identify prediabetes status in employees of Badan Kependudukan dan Keluarga Berencana Nasional (BKKBN) in North Sumatra through fasting blood glucose level examinations, as well as providing education about prediabetes and healthy coffee. The methods used include interactive counseling and fasting blood glucose level examinations using spectrophotometry techniques. The results of the activity showed that 26.7% of participants suffered from prediabetes, and most of them did not know the relationship between coffee consumption and prediabetes. Conclusion: This activity increased participants' knowledge and awareness of healthy coffee and efforts to prevent prediabetes.
