Abstract

Fraktur ankle bimalleolar merupakan jenis fraktur yang menyebabkan ketidakstabilan pada struktur anatomis sendi ankleatau sendi talokrural. Fraktur ini berpotensi menimbulkan gejala sisa pada pasien sehingga diperlukan manajemen intervensi adekuat untuk mengembalikan kondisi struktur maupun fungsi optimal. Systematic review ini bertujuan untuk mengetahui hasil evaluasi status struktural, status fungsional, dan komplikasi pasca intervensi fraktur ankle bimalleolar. Protokol PRISMA digunakan untuk menentukan langkah sistematis dan kriteria eligibility (kelayakan) sebagai pedoman menyeleksi artikelpenelitian. Rentangan tahun publikasi artikel penelitian adalah 2015-2020. Penilaian kualitas dan risiko bias dilakukan dengan kriteria OCEBM dan Cochrane. Sejumlah 7 artikel penelitian dari 249 artikel yang berpotensi diinklusi. Total subjek 496 (14-91 tahun) melaporkan hasil evaluasi intervensi operatif dan 1 studi melakukan perbandingan konservatif. Sebanyak 99,6% (n=494) subjek mencapai union, sedangkan non-union ditemukan pada 2 subjek dengan tindakan konservatif. Instabilitas struktural ditemukan pada 6 subjek dengan medial clear space > 4mm. Status fungsional dengan AOFAS, OMA, dan Biard and Jackson didapatkan 98,9% pada rentang sedang-baik, 5 subjek melaporkan skor buruk, dan tidak ada subjek melaporkan restriksi fungsional. Untuk komplikasi, kondisi yang ditemukan paling umum yaitu nyeri sedang 43,1%, osteoarhtitis ankle grade 1, serta infeksi superfisial dan dalam yang sembuh dengan intervensi antibiotik. Capaian status struktural union sangat tinggi (99,6%), namun 2 subjek mengalami non-union pada kelompok intervensi konservatif. Status fungsional pasien tercapai pada kategori sedang-sangat baik (98,9%) dan tanpa adanya restriksi fungsional meskipun terdapat 5 subjek dengan skor buruk. Komplikasi yang muncul pada pasien umumnya adalah nyeri sedang (43,1%), osteoarthritis ankle grade 1, serta infeksi superfisial dan dalam. Abstract

Background: Tuberculous infection is one of the most common pulmonary infection in developing countries, including Indonesia. Spinal tuberculosis is one of the most common extra¬pulmonary tuberculosis manifestation, accounting for 5% of all extrapulmonary spread, and 50% of all osteoarticular involvement. The aim of this study is to evaluate and how diagnosis and treatment to prevent over or under-diagnosis and explain about excellent management without any late complication further.Case Presentation: A 20-year-old male with a history of weakness of both of his leg for 20 days prior to admission. From these examinations, patient was then diagnosed with pathological fracture 2nd thoracic vertebral body suspected due to atypical spinal tuberculosis. Patient was then undergone surgical debridement, decompression, stabilization and fusion.Results: Patient was then undergone surgical debridement, decompression stabilization fusion. During the exposure at level C6 through Th4, no abscess was found. Transpedicular debridement at level Th2 was then done after the insertion of pedicle screws at level C6-Th1 and Th3-Th4, and again no abscess was present, however, a granulomatous mass was present on the location of 2nd thoracic vertebral body, located on the anterior right side of the spinal cord outside the dura mater.Conclusion: Atypical presentation of spinal tuberculosis with the sole clinical manifestation of neurological deficit below affected level. Abstract

Manually selecting MRI slices is a significant bottleneck in clinical workflows. This issue is worsened by inconsistent naming conventions and variable acquisition protocols across institutions and radiologists, often leading to redundant efforts and potential oversights during medical image data preprocessing. This study introduces a fully automated, four-level hierarchical classification system specifically designed to intelligently filter and select clinically relevant spinal MRI slices directly from raw DICOM series. Our primary objective is to streamline the initial stages of radiological assessment, ensuring that only pertinent images are presented for subsequent analysis and review. We thoroughly evaluated the performance of modern, efficient deep learning architectures, including EfficientViT, MobileNetV4, and RepViT, benchmarking them against a robust ResNet-18 baseline. The proposed pipeline systematically refines its analysis through a structured hierarchy: it first broadly identifies the anatomical region, then precisely classifies the spine location and specific view (axial, sagittal, or coronal). Subsequently, it categorizes the imaging contrast, and finally, confirms the presence of the spinal cord. Our comprehensive experimental results reveal that the EfficientViT-based model achieved the highest end-to-end F1-score of 0.8357, demonstrating robust accuracy across all classification levels. Furthermore, its average inference speed of 9.17 ms per image highlights its computational efficiency. This automated pipeline offers an effective and computationally efficient solution for speeding up initial medical image preprocessing, ensuring subsequent analytical tasks are performed on accurately selected, clinically relevant data.
