Abstract

Background: Maternal and child health (MCH) routine data is essential in making a good health-relatedpolicy. However, the quality of MCH routine data in Indonesia is doubted, and thus the Indonesian governmentrelies heavily on the survey data for policymaking. This condition raises questions about where the problemsexist in routine data recording stages. This study aims to explore the barriers and strategies of MCH routinedata recording by the administrators in the primary healthcare center.Method: This study was qualitativeresearch conducted in Buru Regency, Ambon City, Purworejo Regency, and Surakarta City from May toNovember 2020. The data collections were intended to understand administrators’ efforts to deal with the datarecording problems. Data triangulation was performed through in-depth interviews with primary healthcarecenter staff and observations on daily routine data administration practices.Results: The study demonstratedchallenges in the MCH routine data administration context. The first problem is behavioral contexts lead toincorrect input and delay data submission. Second, technical determinant shows the lack of integration thatleads to repetitive data recording and data fragmentation. The third was the organizational problem suchas lack of inter and intra-departmental coordination in data sharing, infrastructure, and human resourceshortage.Conclusion: The findings elucidate the problem of administrative structures in the implementationof routine data policy. A comprehensive response to cope with routine data policy implementation contextis needed. Existing maternal and child healthcare routine data requires structural administration refinementthat provides a context for implementing reliable routine data recording of maternal and child health.

Abstract

Background:The danger of tobacco use to health has been globally known. However, its prevalence especially in developing countries including Indonesia remains high.This study aims to investigate determinants of tobacco use among adolescents and young adults in Indonesia. These age groups are essential since most regular smokers initiated to smoke at these ages. Methods: This study utilized IFLS wave 5 data. A total of 7213 responses on tobacco use from respondents age 15-24 years old were taken into analysis. Association between tobacco use with sociodemographic, religious, and ethnic identity was determined by using binary logistic regression. Results: Prevalence of tobacco use was 25.1%. The fully adjusted model showed factors that associated with tobacco use were age, sex, educational attainment, marital status, having smoking parents, and Madurese ethnicity. Conclusion:Tobacco use was prevalent among adolescents and young adults in Indonesia. Madurese was the only ethnic group associated with tobacco consumption in Indonesia. Appropriate design of interventions, therefore, is needed to target these subgroups.

Abstract

The high rate of maternal (MMR) and infant mortalities (IMR) is a detrimental health development challenge in Indonesia. The use of the Maternal and Child Health (MCH) handbook and MCH service guidelines are the government’s eff orts in reducing the MMR and IMR. However, the reduction of MMR and IMR is still slow. The fi nding of the previous study asserts the need for MCH handbook and MCH service guidelines modifi cations to be suited to the needs of users, both health workers, and expectant mothers. Therefore, this study mainly aims to analyze the needs and use of MCH handbook and service guidelines by doctors and midwives in the primary health centers. The method was qualitative needs analysis with 137 informants from 12 health centers in three regions of Indonesia. The results revealed that the needs of health workers were clustered in three signifi cant aspects; instrumental (language and book-making materials), managerial (procurement, distribution, and dissemination) and operational (ease of access to MCH handbook and service guidelines as well as the availability of SOP). The fulfi llment of these three aspects is expected to improve the MCH program’s achievement, especially in cities where the research took place and other areas with similar characteristics. Abstrak Tingginya Angka Kematian Ibu (AKI) dan Angka Kematian Bayi (AKB) masih menjadi masalah dalam pembangunan kesehatan di Indonesia. Penggunaan buku Kesehatan Ibu dan Anak (KIA) serta pedoman pelayanan KIA merupakan upaya pemerintah dalam penurunan AKI dan AKB. Meskipun demikian, penurunan AKI dan AKB dinilai masih lambat. Hasil penelitian sebelumnya menunjukkan perlunya modifi kasi buku dan pedoman pelayanan KIA disesuaikan dengan kebutuhan pengguna, baik tenaga kesehatan maupun ibu hamil. Oleh sebab itu, penelitian ini bermaksud menganalisis kebutuhan dan pemanfaatan buku dan pedoman pelayanan KIA oleh dokter dan bidan di puskesmas. Metode yang digunakan adalah analisis kebutuhan kualitatif dengan 137 informan dari 12 puskesmas di tiga Kabupaten/kota di Indonesia. Hasil analisis menunjukkan kebutuhan nakes terkelompok pada tiga aspek utama, yaitu instrumental (bahasa dan bahan pembuat buku), manajerial (pengadaan, pemerataan distribusi dan sosialisasi) dan operasional (kemudahan akses pada buku dan pedoman layanan KIA dan ketersediaan SOP). Pemenuhan terhadap tiga aspek tersebut diharapkan mampu memperbaiki capaian program pelayanan KIA khususnya di wilayah tempat penelitian.

Abstract

Kehadiran teknologi informasi membuat perubahan yang cukup berpengaruh dalam interaksi antara dokter-pasien. Teknologi informasi tidak hanya sekedar menyediakan keterbukaan akses informasi atas pengetahuan mengenai dunia kesehatan akan tetapi juga hadir menjadi komplementer dari pelayanan kesehatan kovensional. Perubahan tersebut berimplikasi pada relasi konvensional dalam pelayanan kesehatan yang telah ada. Pertama, adanya pemberdayaan semu pasien atas pemberian sumber daya informasi mengenai kesehatan menjadikan dokter bukan lagi satu-satunya pemilik informasi tentang kesehatan. Kedua, mulainya pergeseran pola mencari sehat masyarakat dari tipe konvensional beralih menggunakan media-media yang disediakan oleh teknologi informasi. Dua perubahan mendasar tersebut menghasilkan pola mediatisasi relasi dokter-pasien yang perlahan melebarkan jarak sosial antara pasien terhadap dokter dalam bidang pelayanan kesehatan. Alih-alih, terbukanya akses informasi dan pengetahuan tentang kesehatan mendorong pada otonomi pasien yang selama ini bergantung pada dokter, pasien beralih tersub-ordinasi oleh mediatisasi dari produk-produk teknologi informasi.

Abstract

Maternal mortality in Indonesia increased but the coverage of antenatal care and birth attendants by health workers is high. The majority of antenatal care was by private midwives and the majority of birth attendants was by midwives. The study aims to determine the maternal health services by private midwives in maternal health services. It was a mixed study, carried out in Surabaya City because of the highest maternal deaths in East Java Province in 2016. The secondary data of maternal health services were collected from Surabaya City Health Office and Indonesian Midwives Association of Branch Surabaya City. The qualitative data were domain on internal and external problems of private practice midwives. There were 10 informants whom selected purposively. The data were collected by round table discussion. The secondary data were analyzed univariately and the qualitative data were analyzed descriptively. From about 250 private midwives in Surabaya City, 133 were ‘Delima’ midwives in which a program of the Indonesia Midwives Association. Maternal health services by private midwives enhance access. The ‘Delima’ midwives have requirements of knowledge, enhancing skill, reporting, infrastructure standards, and development. So, the quality of maternal health services by ‘Delima’ midwives was relatively better. However, private midwives’ services, including the ‘Delima’ midwives in Surabaya city had not analyzed ANC results, pregnant women visiting private midwives should visit health centers for integrated ANC, midwives’ duties beyond educational competencies, and lack of mentoring for apprentice midwives because of many midwives’ duties. It suggests improving the knowledge and skill of private midwives.
