Abstract

Type 2 diabetes mellitus (T2DM) is a complex metabolic disease characterized by high blood glucose levels due to impaired insulin secretion or insulin resistance. Polymorphisms in the ABCC8 gene rs1799854 are widely found to have an association with T2DM, where the ABCC8 gene encodes the SUR1 protein from the K-ATP channel that plays a role in insulin secretion in cells β pancreas. Mutations in the ABCC8 gene can cause potassium channels and insulin secretions problems that possibly decrease the effectiveness of the drug. Polymorphisms in some populations have been reported, but similar research on Solo's population has never been conducted. The purpose of this study is to identify the genotype of the ABCC8 gene in rs1799854 and the therapeutic outcome of blood glucose levels in T2DM patients. This research is cross-sectional research conducted prospectively at Moewardi Hospital Surakarta, Solo. Blood samples were collected from 10 T2DM patients who received sulfonylurea monotherapy taken through veins for genotype examination by DNA isolation, PCR amplification, and sequencing. Parameters of fasting blood glucose (FBG), 2-hour postprandial blood glucose, and HbA1c are measured as therapeutic outcomes. We found the dominant results in a mutant homozygote (TT) 40%; while wild type (CC); and a mutant heterozygote (CT) was 30%. Single Nucleotide Polymorphisms (SNP) of ABCC8 gene rs1799854, found in T2DM patients at Moewardi Hospital Surakarta, Solo, who received sulfonylurea therapy.

Abstract

Resistensi antibiotik adalah salah satu masalah kesehatan yang utama, dan membutuhkan dukungan serta usaha menyeluruh untuk mencegahnya. Perkembangan penemuan antibiotik baru tidak secepat dengan resistensinya, sehingga penemuan antibiotik baru akan sia-sia jika tidak disertai dengan tindakan pencegahannya. Di Indonesia, resistensi antibiotik belum dapat diselesaikan secara menyeluruh, hal tersebut dipengaruhi oleh perilaku penggunaan antibiotik yang tidak rasional. Beberapa faktor yang mempengaruhi penggunaan antibiotik yang tidak rasional antara lain tata kelola yang buruk, lemahnya implementasi penegakan hukum, kurangnya pengetahuan, dan mudahnya akses untuk mendapatkannya. Kesalahan konsep dan pemahaman terhadap penggunaan antibiotik sangat mempengaruhi perilaku dalam penggunaannya. Adanya penyuluhan terkait penggunaan antibiotik yang rasional kepada masyarakat diharapkan dapat meningkatkan pengetahuan dan pemahaman masyarakat sebagai upaya pencegahan resistensi antibiotik. Kegiatan penyuluhan dilakukan dalam lima tahapan yaitu persiapan survei lokasi, persiapan dan penyusunan proposal, pelaksanaan, evaluasi dan pelaporan. Tahapan persiapan survei lokasi terdiri dari observasi lapangan. Tahap persiapan dan penyusunan proposal terdiri dari pengumpulan materi, penyusunan proposal, dan persiapan materi. Tahap pelaksanaan kegiatan yaitu pretest, materi penyuluhan, dan diskusi. Tahapan evaluasi meliputi postest dan penyusunan laporan. Sasaran kegiatan ini adalah ibu-ibu PKK warga dukuh Pangkah Sumberagung, Jetis, Bantul, Yogyakarta. Hasil tes pengetahuan peserta tentang pencegahan resistensi antibiotik sebelum dan sesudah kegiatan dilaksanakan, meningkat dari 82,30% menjadi 93,64%. Kegiatan pengabdian kepada masyarakat ini mampu meningkatkan pengetahuan terkait pencegahan resistensi antibiotik kepada ibu-ibu PKK warga dukuh Pangkah Sumberagung, Jetis, Bantul, Yogyakarta.

Abstract

Background: Hypertension is a chronic disease that needs to be treated properly and continuously. One of the critical successes of therapy is medication adherence by patients.Objective: Determine the correlation between medication adherence and therapeutic outcome in hypertension patients at the Mlati II health center.Methods: This is a non-experimental study with a cross sectional design analysis, by purposive sampling technique. Adherence was obtained in the form of adherence levels measured using the MMAS-8 questionnaire, therapeutic outcomes assessed by measuring a patient's blood pressure. Then the data were analyzed using the Chi-square test to determine the relationship between the level of adherence and therapeutic outcomes.Results: From 76 hypertension patients, the majority aged 46-55 years (44.7%), female gender (69.7%), senior high school (39.5%), employment status did not work (55.3%), long suffering from hypertension <5 years (76.3%), received single therapy (61.8%) with the type of antihypertension drug amlodipine (59.2%), low adherence category rate (39.5%), outcome of therapy with uncontrolled blood pressure (60,5%), and significancy level p = 0,997>0,05Conclusion: There is no relationship between the level of adherence to the use of antihypertension drugs and the outcome therapy of controlled blood pressure at the Mlati Il health center

Abstract

Diabetes mellitus (DM) is a disease that is a global health problem whose numbers continue to increase. DM has the highest prevalence and causes more severe disease complications such as heart disease and stroke, which often cause death throughout the world. Proper prevention and treatment will reduce the impact or risk of unwanted complications. This health education activity was carried out to increase public knowledge and understanding about DM disease and how to screen to detect DM disease among residents of Pangkah Hamlet. The extension activity method begins with giving a pretest, then continues with the delivery of material and discussion, then ends with a posttest. Next, a health examination is carried out, namely measuring blood glucose levels. Extension activities are carried out in five stages: preparation of location surveys, preparation and preparation of proposals, implementation, evaluation, and reporting. The preparation stage for a location survey consists of field observations. The preparation and proposal preparation stage consists of collecting material, preparing the proposal, and preparing the material. The stages of implementing the activities are pretest, counseling material, and discussion. The evaluation stages include posttest and report preparation. The target of this activity is women residents of Pangkah hamlet Sumberagung, Jetis, Bantul, and Yogyakarta. The results of the participants knowledge test about DM before and after the activity were carried out, with good results increasing from 83.33% to 100%, with an average pretest score of 81.67 and posttest score of 93.61. The results of the blood sugar level screening (GDS) showed that the majority were below 200 mg/dl, namely 34 participants (94.44%). From these results, it can be concluded that this community service activity was able to increase knowledge about DM among women residents of Pangkah hamlet and the results of the initial DM screening were in the normal range.

Abstract

Background: Diabetes mellitus (DM) is a metabolic disorder caused by damage or inability of pancreatic β cells to produce insulin. Type 2 DM patients are often accompanied by accompanying diseases that are either a complication of type 2 DM or the presence of other diseases so that the drugs used will be more. More drugs taken at the same time can increase the risk of unwanted drug interactions. Research Objectives: This study aims to evaluate the potential interaction of metformin with other drugs in patients with type 2 DM on the road at Nur Hidayah Bantul Yogyakarta Hospital. Research Methods: Descriptive observational research with retrospective data collection. The data collection technique used purposive sampling method. Samples that met the inclusion criteria were obtained as many as 82 patients. Data analysis used univariate analysis to describe patient characteristics and treatment, as well as identification of drug interactions based on Drugs.com. Research Results: In this study, the majority of patients with type 2 DM were aged 61-80 years 47.6%, female 63.4% and had comorbidities 89.0%. Treatment of type 2 DM patients mostly used combination therapy regimens as much as 78% of combinations, with a combination of 2 drugs, namely metformin and glimepiride 35.4%, getting ≥5 drugs 59.8%. Potential drug interactions of metformin with other drugs as much as 85.4% experienced drug interactions with moderate severity 88.6% and the most common drug interactions were metformin and glimepiride 37.1%. Conclusion: The majority of type 2 DM patients in the outpatient installation of Nur Hidayah Bantul Yogyakarta Hospital experienced potential metformin drug interactions.

Abstract

Hypertension is a chronic disease and a major risk factor for cardiovascular disease, often referred to as a "silent killer" due to its asymptomatic nature. Its prevalence in Indonesia is increasing, reaching 34.1% among individuals aged 18 years and over in 2018. Traditional herbal medicine can treat degenerative diseases, but its use is still limited due to inadequate public knowledge. This community service program aims to provide education about hypertension and the use of traditional herbal medicine in Dusun Beji, Bantul, Yogyakarta. The program includes health education, blood pressure checks, and promotion of herbal medicine for hypertension management. A total of 34 participants (85% attendance) attended the session, with 26 completing pretest and posttest questionnaires to evaluate increased knowledge. The majority of participants were elderly (?61 years, 50%) and female (69.2%), with 69.2% showing abnormal blood pressure levels. Educational materials, including presentations and brochures, were provided along with live demonstrations of herbal preparations. Post-test results showed a slight increase in mean score from 7.73 to 7.88, with increased homogeneity of data and an increase in median from 7 to 8. Although paired t-test showed no statistically significant difference (P=0.885), descriptive analysis showed an increase in understanding and awareness. Active participation during the discussion, especially regarding herbal preparations, reflected the community's interest in adopting alternative approaches to hypertension management. This initiative has succeeded in increasing awareness of hypertension and the benefits of herbal medicines to effectively manage the chronic disease and its complications.

Abstract

Hipertensi merupakan penyakit tidak menular yang ditandai dengan tekanan darah sistolik diatas 140 mmHg atau tekanan darah diastolik diatas 90 mmHg pada pemeriksaan berulang. Penyakit ini termasuk penyakit degeneratif yang dapat mengakibatkan kerusakan organ vital tubuh seiring berjalannya waktu. Penderita hipertensi diperkirakan sebesar 22% dari total populasi di dunia. Hipertensi termasuk masalah kesehatan global yang cukup berbahaya, dan menjadi penyebab utama penyakit jantung koroner dan penyakit ginjal. Hipertensi atau tekanan darah tinggi juga dikenal sebagai silent killer karena pada kebanyakan kasus pasien tidak mengetahui kapan mengalami tekanan darah tinggi dikarenakan pada pemeriksaan fisik seringkali tidak menunjukkan tanda-tanda sampai terjadi komplikasi. Pencegahan dan penanganan yang tepat akan mengurangi dampak atau risiko terhadap terjadinya komplikasi yang tidak di inginkan. Kegiatan penyuluhan kesehatan ini bertujuan untuk meningkatkan pengetahuan dan pemahaman masyarakat Dukuh Beji tentang penyakit hipertensi dan DAGUSIBU obat antihipertensi sebagai upaya promotif dan preventif. Metode kegiatan penyuluhan diawali dengan pemeriksaan kesehatan yaitu pengukuran tekanan darah, selanjutnya dilakukan pretest yang kemudian dilanjutkan dengan penyampaian materi dan diskusi kemudian diakhiri dengan posttest. Hasil kegiatan penyuluhan dari 26 peserta diperoleh karakteristik peserta mayoritas berusia >61 sebanyak 50%, dengan jenis kelamin perempuan 69,2%, dan tekanan darah berada pada rentang tidak normal 69,2%. Pada evaluasi tingkat pengetahuan terjadi peningkatan pengetahuan peserta yang dilihat dari hasil pretest sebelum dilakukan penyuluhan masih ada masyarakat yang memiliki tingkat pengetahuan yang cukup sebanyak 15,4%, setelah diberikan penyuluhan dan diberikan postest diperoleh tingkat pengetahuan seluruh peserta 100% dalam kategori “Baik”.
