Abstract

AbstractThe Potential hazards that are poorly understood by workers but have a large risk of work accidents are the hazards of working in confined space. Based on the findings of researchers that work in confined spaces found unsafe conditions on the access stairs leading to the groundtank, the access ladder was not tied or not held, causing the stairs to move and workers who went down the stairs fell from a height. PT. X is a contractor (vendor) who usually does groundtank maintenance work. This study aims to identify the overall potential hazards of each stage or sequence in the work process carried out by workers during the maintenance of PT's A1 groundtank confined space. X in 2023 in order to prevent work accidents and occupational diseases (PAK). The method used in this study is the Job Safety Analysis (JSA) method. the results of identifying potential hazards related to occupational health and safety using the Job Safety Analysis (JSA) method on groundtank maintenance which includes the confined space area at Plant A1 PT. X, obtained 8 stages of the work process and 18 potential hazards including chemical hazards exposed to toxic gases and lack of oxygen, electrical hazards of electric shock, biological hazards from animal hazards, physical hazards of falling and slipping. From the potential hazards that exist, then recommendations for control efforts are carried out such as the provision and use of Personal Protective Equipment (PPE), the use of appropriate equipment and always check equipment before use, blower the groundtank area within 1 work shift, which is for 8 hours, atmospheric measurements in the groundtank to remove toxic gases.AbstrakPotensi bahaya yang kurang dipahami oleh pekerja namun memiliki risiko kecelakaan kerja yang besar adalah bahaya bekerja di ruang terbatas (confined space). Berdasarkan hasil temuan peneliti bahwa pekerjaan di confined space ditemukan adanya kondisi tidak aman atau unsafe condition pada tangga akses menuju dalam groundtank, tangga akses tersebut tidak diikat maupun tidak dipegangi sehingga menyebabkan tangga bergerak dan pekerja yang menuruni tangga tersebut terjatuh dari ketinggian. PT. X adalah sebuah kontraktor (vendor) yang biasa melakukan pekerjaan perawatan groundtank. penelitian ini bertujuan untuk mengidentifikasi potensi bahaya secara keseluruhan dari setiap tahapan atau urutan pada proses pekerjaan yang dilakukan oleh pekerja saat perawatan confined space groundtank A1 PT. X tahun 2023 agar dapat mencegah kecelakan kerja maupun penyakit akibat kerja (PAK). Metode yang digunakan pada penelitian ini adalah metode Job Safety Analysis (JSA). hasil identifikasi potensi bahaya yang terkait dengan Kesehatan dan keselamatan kerja dengan menggunakan metode Job Safety Analysis (JSA) pada perawatan groundtank yang termasuk area confined space di Plant A1 PT.X, didapatkan 8 tahapan proses pekerjaan dan 18 potensi bahaya diantaranya bahaya kimia terpapar gas beracun dan kekurangan oksigen, bahaya electrical tersengat arus listrik, bahaya biologi dari animal hazard, bahaya fisik terjatuh dan terpleset. Dari potensi bahaya yang ada, berikutnya dilakukan rekomendasi upaya pengendalian seperti penyediaan dan penggunaan Alat Pelindung Diri (APD), penggunaan peralatan yang layak dan selalu dilakukan pemeriksaan peralatan sebelum digunakan, melakukan pembloweran pada area groundtank dalam jangka waktu 1 shift kerja yaitu selama 8 jam, pengukuran atmosfer dalam groundtank untuk menghilangkan gas beracun.

Abstract

At PT. Berdikari furniture Nusantara which is a manufacturing company that produces furniture. In the implementation of the production process has 11 stages of the process and the woodworking process into a process with the most reject data of 15.2% for the reject process in each process. In the woodworking process found damaged wood (broken, chunk, damaged) as much as 22% which became the most common type of defect found in the woodworking process. From the existing data, the use of FTA (fault tree analysis) method aims to analyze the factors that cause defects in products. FMEA (failure mode & effect analysis) method aims to identify and determine the proportion and priority of factors causing product defects that must be repaired immediately. From these methods obtained the results of the analysis for the FTA is that there are 4 basic root causes of 2 causal factors, namely the absence of job desc and responsibility, no understanding of maintaining product quality, no work instructions, and no habit of continuous improvement. The results of FMEA analysis that has been conducted interviews at the head of the quality control department that has not understood about maintaining the quality is meant by having a value of RPN (risk priority number) of 252. Available for 3 employees who actually have RPN values each are not yet familiar with ongoing habits with a value of 175, not to mention work instructions with a value of 144, and the most rare are job desc and responsibilities with a value of 48. In the end, the root cause of the improvement will be discussed again so that it is hoped that it can have a positive effect on the company.
