Abstract

Background: Fluid management can reduce mortality, severe comorbidities, and debilitating symptoms in patients on hemodialysis. Therefore, a restricted fluid intake plan is crucial for patients with chronic kidney disease (CKD). Little evidence has been found to date on exploring the experience in fluid management of CKD patients in Indonesia.Purpose: This study aimed to explore the fluid management experience of adults with chronic kidney disease participating in hemodialysis treatment in Indonesia. Methods: A qualitative study with a phenomenological approach was conducted. Purposive sampling was used to recruit 14 adults patients with chronic kidney disease undergoing hemodialysis in a tertiary hospital in Padang between July and September 2020. Manual content analysis using the Colaizzi approach was used to identify themes. Results: Data analysis revealed four themes with 12 sub-themes. The four major themes include the challenge of thirst control, fluid/diet restriction management, inadequate information, and the support system. Conclusions: The result showed the severe suffering and sadness experienced by CKD patients who conform to fluid restrictions. It is important to provide clear information on the fluid prescription or the exact consumable amount per day. Structured education with a personal approach is recommended to ensure detailed information regarding "fluid prescribing". The support obtained from family, friends, and dialysis staff is a significant factor in promoting acceptance and adherence.

Abstract

This study aims to determine the effect of implementing discharge planning on self-care for ischemic stroke clients at Advent Hospital and Al Ihsan Hospital, Bandung. The method used is quasi-experiment with pretest-posttest with control group design. The sampling technique uses purposive sampling. The results of the research show that the description of Self Care Behavior before and after discharge planning at the Adventist Hospital is that there is an influence of Discharge Planning intervention using flip sheets (audio visual video) and using ROM training tools, with a p-value of 0.01 < 0.05, and Self Description Care Behavior before and after discharge planning at Al Hospital. Ihsan Bandung, There is an effect of Discharge Planning intervention using leaflets with a p-value of 0.01 < 0.05. Furthermore, the description of Self Care Management before and after discharge planning at the Adventist Hospital shows the influence of the Discharge Planning intervention at the Hospital. Advent used a flip sheet (audio-visual video) and used a ROM training tool with a p-value of 0.01 < 0.05 and there was no effect of Discharge planning intervention in the hospital. Al Ihsan used Leaflet with a p-value of 0.71 > 0.05. The conclusion in this research is that there is an influence before and after discharge planning on self-care behavior in hospitals. Adventist and RS. Al Ihsan, and There is an influence before and after discharge planning is implemented on self-care management in hospitals. Abstract

Background: Fatigue is a common and distressing symptom among patients with chronic kidney disease (CKD) undergoing hemodialysis, significantly impacting their daily functioning and quality of life. Despite its prevalence, the contributing factors to fatigue remain unclear and underexplored in specific populations.Purpose: This study aimed to identify factors associated with fatigue among CKD patients undergoing hemodialysis. Methods: A cross-sectional study was conducted in two hemodialysis centers. A total of 164 patients were recruited using convenience sampling, with sample size determined through power analysis. Data were collected using the FACIT-Fatigue Scale, a health literacy questionnaire, and an observation sheet for hemoglobin (Hb), blood pressure, and interdialytic weight gain (IDWG). Patient characteristics were also recorded. T-tests and ANOVA were used to examine differences in fatigue levels across groups, while Spearman correlation and multiple linear regression were employed to analyze associations between fatigue and clinical variables. Results: The mean fatigue score was 28.23(SD=7.46). Fatigue was significantly associated with age, education level, employment status, comorbidities, duration of hemodialysis, systolic blood pressure, HB, IDWG, and health literacy (p<0.05). Multiple linear regression identified employment status and hemoglobin levels as the strongest predictors of fatigue. Conclusion: Fatigue is a prevalent issue among CKD patients undergoing hemodialysis and is influenced by a range of demographic, clinical, and behavioral factors. Employment status and hemoglobin levels were the most significant contributors. Interventions promoting physical activity, a healthy diet, and fluid adherence, alongside routine monitoring of hemoglobin levels, may help reduce fatigue and improve quality of life in this population.

Abstract

Adolescence is a critical developmental period marked by identity exploration and vulnerability to negative influences, with parenting playing a significant role in shaping behavior. This study investigates the relationship between parenting styles and violent behavior among adolescent inmates, focusing on two correctional institutions. The research employed a cross-sectional quantitative design, utilizing questionnaires to collect data from adolescent inmates in two prisons. The analysis focused on identifying the predominant parenting styles experienced by the inmates and the prevalence of violent behavior within this population. The findings revealed that authoritarian parenting was the most common style reported by respondents, and a high level of violent behavior was observed among the inmates. Statistical analysis (p-value=0.001) demonstrated a strong and significant correlation between authoritarian parenting and the occurrence of violent behavior, indicating that strict and inflexible parenting approaches contribute to the development of aggression in adolescents. These results underscore the importance of fostering positive parenting practices and providing targeted interventions in correctional settings to reduce violence among adolescents. Low educational attainment and external factors, such as peer influence and family instability, further exacerbate violent tendencies. Understanding the impact of parenting styles is essential for developing effective prevention and rehabilitation strategies for young offenders.

Abstract

Introduction: Individuals with heart failure anticipate adjustment to self-care behaviors following hospital discharge. Self-care in heart failure includes medication adherence, dietary modifications, symptom monitoring, and activity adjustment. Adequate self-care behavior may contribute to enhanced outcomes, decreased hospitalization, and improved quality of life in patients with heart failure. This study aimed to determine the level of self-care behavior, quality of life, and its relationships among patients with heart failure. Methods: A cross-sectional study using purposive sampling was conducted among 200 heart failure patients at a teaching hospital in Selangor, Malaysia. The sample size was determined using G*Power to ensure adequate statistical power. Data were collected using the Self-Care of Heart Failure Index (SCHFI) and the Minnesota Living with Heart Failure Questionnaire (MLHFQ). Descriptive statistics, Pearson’s correlation, and multiple linear regression were used in this study. Results: The results indicated that the total mean score of self-care behavior was 145.42 and SD=16.23. Most patients demonstrated good quality of life (n=73, 36.5%), moderate quality of life (n=62, 31%), and poor quality of life (n=65, 32.5 %). Pearson's correlation revealed a significant (p<0.001) correlation (r = 0.651) between self-care behavior and quality of life. Conclusions: In conclusion, this study demonstrated that higher levels of self-care are associated with better quality of life in patients with heart failure. It is recommended that nurses provide individualized self-care education at discharge, focusing on medication adherence, symptom monitoring, and lifestyle changes to improve quality of life.
