Abstract

Diabetes Mellitus (DM) dapat menyebabkan komplikasi mikrovaskuler berupa retinopati. Prospek nanopartikel emas (nanogold) dalam mencegah komplikasi retinopati diabetikum dianggap baik karena mampu melewati penghalang darah-retina dan berasosiasi dengan sel epitel retina. Nanogold (AuNP) menekan berbagai ekspresi mediator inflamasi dan mengurangi radikal bebas yang meningkat akibat hiperglikemia. Berbagai studi membuktikan AuNP memiliki daya inhibisi terhadap TGF ß, VEGF, antiglikasi, dan antioksidan Penelitian ini menggunakan hewan coba tikus putih jantan (Rattus norvegicus) sebanyak 25 ekor yang dipisahkan secara acak menjadi lima kelompok, diinduksi oleh aloksan dan diberikan AuNP, inulin, dan keduanya untuk melihat gambaran vaskularisasi mata. Hasil pengukuran menggunakan uji Kruskall Wallis dan Uji ANOVA menunjukkan perbedaan kadar gula darah sewaktu antar kelompok tidak signifikan. Sementara, analisis gambaran vaskularisasi menggunakan uji pearson chi square menghasilkan pemberian kombinasi inulin dan nanogold dapat mencegah terjadinya vasodilatasi (Nilai p = 0,001) dan edema makula (Nilai p = 0,001). Namun tidak terdapat perbedaan proporsi yang signifikan pada kejadian perdarahan retina (Nilai p = 0,308). Stres oksidatif mendukung patogenesis komplikasi diabetik. Kombinasi inulin dan nanogold secara bersamaan memberikan efek yang tidak efektif terhadap menurunkan gula darah. Meskipun tidak dapat disimpulkan kemungkinan adanya interaksi antara inulin dan AuNP memberikan efek inhibisi terhadap kadar gula darah, kombinasi nanogold dan inulin memberikan efek protektif yang paling baik terhadap endotel retina mata karena dapat mencegah vasodilatasi, edema makula, dan perdarahan. Abstract

Inteoduction : Pupillary block occurs when aqueous humor flow from posterior chamber to anterior chamber is obstructed by functional block between pupillary portion of iris and lens. It is relatively common when pupil is occluded by anterior chamber intraocular lens (IOL). This condition can lead to iris bombe, iridocorneal adhesion formation, increasing intraocular pressure (IOP), and optic nerve damage. Case Illustration : A 65-year-old woman came to local hospital with complaints of blurred left eye five days prior presentation. She had history of bilateral cataract surgery in 2013. On physical examination, BCVA was 6/6 on right eye and 1/300 on left eye. The IOP was 15. 3 on right eye and 31.5 on left eye. Slit lamp examination showed anterior chamber IOL on left eye, shallow camera oculi anterior (COA), no flare/cell, pupillary block was present, and no surgical iridectomy was found. She was previously given timolol 0.5% eye drop and added latanoprost 50 mcg eye drop, sodium chloride 4.4 mg- potassium chloride 0.8 mg eye drop for current therapy. Discussion : Pupillary block is a frequent complication of anterior chamber IOL implantation after cataract surgery. The anterior chamber in this patient was shallow, which indicates partial/total blocked aqueous flow from posterior chamber, causing an elevated IOP, and may disrupt optic nerve that cause decreased vision. Surgical iridectomy is suggested in high-risk procedures such as implantation of an anterior chamber IOL. Conclusion : Elevated IOP may cause visual dysfunction and damage to optic nerve. In cases of high-risk postoperative pupillary block, surgical iridectomy is suggested to prevent further complications

Abstract

This study critically analyzes the epistemological foundations of hadith criticism, examining the paradigmatic conflict between the textualist Ahl al-Hadith and the rationalist Ahl al-Ra’yi/Kalam traditions. This topic is vital as it addresses the core tension between tradition and reason in Islamic thought, a debate that remains highly relevant in contemporary contexts, especially regarding hadith’s authenticity and its compatibility with modern knowledge. The research uses a qualitative, library-based comparative analysis to scrutinize the methodologies of both traditions, focusing on their respective primary sources of authority, criticism criteria, and validity standards. The research findings indicate that both paradigms possess complementary strengths and weaknesses: the textual paradigm excels in historical authenticity but lacks rational coherence, while the rational paradigm emphasizes theological consistency and universal Islamic values yet tends toward subjectivity. Both represent distinct but mutually corrective approaches to understanding prophetic truth. This study's key contribution is a proposal for a comprehensive critical epistemology that integrates the historical rigor of sanad criticism with the rational scrutiny of matn. This hybrid framework offers a balanced and accountable approach to hadith criticism, bridging the historical gap and ensuring its continued relevance for modern scholarship and believers.
