Abstract

Selama masa pandemi Coronavirus disease 2019 (COVID-19), apoteker harus tetap memberikan pelayanan farmasi klinik di apotek. Apoteker di apotek merupakan sumber informasi terkait penggunaan obat yang mudah diakses oleh masyarakat. Tetapi pelaksanaan kegiatan pelayanan kefarmasian di apotek tentu mengalami hambatan dan tantangan selama pandemi ini. Tinjauan literatur dilakukan dengan tujuan untuk mengetahui dampak pandemi terhadap pelaksanaan pelayanan farmasi klinik dan perubahan yang dilakukan oleh apoteker di apotek selama situasi pandemi ini. Penelusuran artikel dilakukan melalui search engine PubMed dan ScienceDirect. EndNote X9 digunakan sebagai alat bantu untuk pengorganisasian artikel hasil penelusuran. Hasil penelusuran didapatkan tujuh artikel penelitian observasional atau eksperimental yang memenuhi persyaratan inklusi dan eksklusi lain yang sudah ditetapkan. Hasil tinjauan literatur ini menunjukkan bahwa pengkajian resep, dispensing, pelayanan informasi obat, konseling, home pharmacy care, pemantauan terapi obat, dan monitoring efek samping obat tetap dapat dilakukan selama masa pandemi dengan persyaratan yang ketat. Pelayanan informasi obat dan konseling di apotek dilakukan dalam waktu yang singkat, pasien kemudian diberikan informasi tambahan melalui telepon atau flyer. Pemantauan terapi dan efek samping obat dapat dilakukan melalui telepon. Home pharmacy care hanya dilakukan jka sangat diperlukan. Pelayanan farmasi klinik di apotek dilakukan secara terbatas dengan menerapkan protokol kesehatan dan modifikasi sistem. Pelayanan online menjadi alternatif yang bisa diterapkan untuk mengisi kekurangan pelaksanaan secara tatap muka.

Abstract

Infeksi Saluran Kemih (ISK) merupakan suatu penyakit infeksi yang dapat memperburuk kondisi pasien dengan penyakit komorbid seperti Penyakit Ginjal Kronis (PGK). Pasien ISK dengan PGK yang dirawat inap umumnya mendapatkan terapi antibiotik. Namun, penggunaannya berpotensi menimbulkan Masalah Terkait Obat (MTO) karena adanya risiko nefrotoksisitas pada beberapa antibiotik, gangguan terhadap farmakokinetik obat, dan banyaknya peresepan obat pada pasien PGK. Studi cross-sectional ini bertujuan untuk mengevaluasi kesesuaian dosis dan potensi interaksi obat pada pasien ISK dengan PGK rawat inap pada salah satu RSUD di DKI Jakarta. Sejumlah 78 sampel penelitian diperoleh menggunakan teknik total sampling. Data sekunder pasien meliputi resep, rekam medis, dan hasil laboratorium pasien periode tahun 2018-2022 diambil secara retrospektif. Analisis MTO menunjukkan bahwa 13 pasien (17,1%) menerima dosis antibiotik tidak sesuai dan 34 pasien (43,6%) mengalami potensi interaksi obat. Uji statistik menggunakan Chi-square menunjukkan bahwa ketidaksesuaian dosis antibiotik lebih sering ditemukan pada kelompok PGK stage 4-5 (p= 0,041; OR=4,744) dan lama rawat inap >6 hari (p= 0,032; OR= 5,067) serta kejadian potensi interaksi obat antibiotik lebih banyak dialami oleh pasien berjenis kelamin perempuan (p= 0,007; OR= 4,667), PGK stage 4-5 (p= 0,024; OR=3,208), jumlah riwayat komorbid selain PGK >2 (p= 0,046; OR= 2,827), dan jumlah obat >10 (p= 0,031; OR= 3,061). RSUD tersebut sudah menerapkan praktik pemberian antibiotik pasien ISK-PGK dengan baik namun dapat ditingkatkan lagi dengan melakukan pemantauan obat dan pasien secara ketat, terutama terkait penyesuaian dosis dan potensi interaksi obat antibiotik.

Abstract

Cough is a symptom that people often treat with self-medication. Self-medication practice can be formed from knowledge and attitudes. The development of this instrument aimed to obtain a valid and reliable Indonesian language questionnaire to measure the level and relationship of knowledge, attitudes, and cough self-medication practice in adult patients. The questionnaire was prepared based on literature studies and developed by the Dagusibu concept and cough management. Three pharmacists from academia and the community became validators in the face and content validation test. Construct validation and reliability tests were obtained from 30 adult respondents who had carried out cough self-medication in Jabodetabek. Content validation was carried out in 2 stages. The results of the first stage test had an S-CVI score of 0.93, with 26 questions having an I-CVI value <0.79, so stage 2 validation was carried out, which met the I-CVI and S-CVI scores. The total number of questions from content validation was 143, divided into 6 parts. The final questionnaire had 137 questions consisting of sections A-B (8 questions) for personal and sociodemographic data; part C (30 questions) related to self-medication profile; part D (36 questions) about knowledge with a 3-point Guttman scale; part E (30 questions) related to attitudes with a 5-point Likert scale; and part F (33 questions) about behavior with a 2-point Guttman scale. The final questionnaire met the criteria of validity and reliability and could be filled in independently by respondents in Indonesia.

Abstract

Antihypertensive and antidiabetic drugs in chronic kidney disease (CKD) patients undergoing hemodialyThe high demand for telepharmacy services led to the urge for proper training and education to enhance its quality. This review aimed to assess the implementation and outcomes of telepharmacy training and education programmes. This scoping review was conducted on ScienceDirect, Sage Journal, SpringerLink, and Google Scholar databases using keywords “training” OR “education” AND “telepharmacy,” “training” OR “education” AND “digital competency” AND “pharmacy.” Only English-written articles published between 2000 – 2023, original research and brief report were included in this review. Eight of 171 articles met the criteria and the study’s objectives. Those studies discussed telepharmacy learning programmes for pharmacy students in the USA (5 articles), UAE (2 articles), and Malaysia (1 article). The learning methods included didactic learning, case-based study, simulation/roleplay, and clerkship employing technological tools. Rubrics, quizzes, questionnaires, and objective structured clinical examination (OSCE) were used as assessment methods. All studies reported improved students’ knowledge, perceptions, and telepharmacy competencies. In conclusion, the telepharmacy learning programmes have effectively upgraded students’ knowledge and skills by various methods at every level. However, there remains a considerable need for evidence on suitable training for pharmacists to improve their telepharmacy competencies and service quality.

Abstract

Antihypertensive and antidiabetic drugs in CKD patients on hemodialysis may cause medication-related problems requiring monitoring. This study aimed to evaluate the selection, dosage, and potential drug interactions of antihypertensive and antidiabetic drugs in stage 5 CKD patients with hypertension and/or type 2 diabetes mellitus undergoing hemodialysis in a hospital in Jakarta. A crosssectional study used the medical records of adult in patients from January - December 2022 with a total sampling method. Out of 101 patients, 97.0% received appropriate drug selection. Dosage adjustments were appropriate in 74.3% of cases. Potential drug interactions between antihypertensive and antidiabetic drugs were found in 90.1% of patients, mostly pharmacodynamic interactions, moderate severity, and requiring monitoring. Statistical analysis showed that age, gender, number of drugs, and length of stay were not associated with the appropriateness of antihypertensive and antidiabetic drug selection (p>0.05). However, there was a relationship between number of drugs (p=0.033; OR=2.996) and length of stay (p=0.024; OR=3.171) with the appropriateness of drug dosage. The length of stay was also associated with potential drug interactions (p=0.040; OR=8.426). Drug selection has been done well, but there is a need for improvement in monitoring dosage adjustments and potential drug interactions by pharmacists in the hospital.
