Abstract

Jumlah penderita penyakit gagal ginjal kronik (PGK) di Indonesia tampak semakin meningkat. Angka pertumbuhannya diperkirakan sekitar 20% setiap tahunnya. Kondisi  ini menjadi masalah  karena biaya pengobatannya besar sekali. Penyakit gagal ginjal kronik adalah  kerusakan  atau  gangguan  fungsi  dan struktur  ginjal  selama  tiga  bulan  atau  lebih  dengan  atau  tanpa penurunan  laju  filtrasi  glomerulus (LFG) disertai  manifestasi  kelainan patologi  ginjal  atau  kerusakan  ginjal  meliputi  komposisi  darah  atau urin  dan  kelainan  pada  uji pencitraan ginjal. Penyakit gagal ginjal kronik  terjadi  bila ginjal mengalami penurunan fungsi LFG di bawah 60 ml/menit/1.73m² dengan  atau  tanpa  kerusakan  ginjal. Intervensi berupa terapi pengganti ginjal dilakukan pada saat keadaan LFG mencapai <15 ml/menit/1.73 m2. Beberapa faktor risiko yang menjadi penyebab PGK telah diketahui seperti usia, gagal jantung, sirosis hepatis, glomerulonefritis kronik, diabetes mellitus (DM), sistemik lupus erimatosus (SLE), polikistik, pielonefritis, nefrolitiasis, nefrosklerosis, dan obstruksi traktus urinarius. Terbatasnya jumlah donor ginjal untuk transplantasi dan tingginya komplikasi yang mungkin terjadi akibat peritoneal dialisis membuat hemodialisis (HD) cenderung menjadi pilihan yang utama apabila fungsi ginjal penderita sudah sangat menurun. Prinsip dasar HD adalah mengalirkan darah dari tubuh ke ginjal buatan untuk dilakukan penyaringan darah melalui suatu membran semi-permiabel agar terjadi proses difusi dan ultrafiltrasi darah di dalamnya. Darah yang sudah disaring kemudian dikembalikan ke dalam tubuh. Hemodialisis rutin dalam jangka panjang memerlukan pemasangan akses vaskular permanen. Fistula arteriovenosa (FAV) masih dianggap sebagai akses vaskular terbaik untuk HD, terutama karena angka patensinya yang tinggi, lebih rendah insidensi infeksi dan komplikasinya dibandingkan dengan kateter vena sentral atau graft arteriovenosa. Kelebihan dan kekurangan FAV telah dibahas dalam berbagai penelitian. Abstract

Obesitas merupakan kondisi ketidakseimbangan antara masukan dan penggunaan energi, dimana kelebihan energi diakumulasikan dalam / berupa jaringan lemak. Meskipun, obesitas dapat dipicu oleh faktor-faktor lain dalam tubuh, namun obesitas juga menjadi faktor risiko utama penyakit-penyakit metabolik dan kardiovaskular; yang jumlah penderitanya  terus bertambah di seluruh dunia. Kondisi obesitas dapat dinilai dengan menggunakan beberapa metode pengukuran antropometrik, diantaranya indeks massa tubuh (IMT). Hipertensi merupakan penyakit yang dapat dipicu oleh kondisi obesitas. Beberapa publikasi telah berusaha mengungkap hubungan antara obesitas dengan hipertensi. Penelitian ini bertujuan mengkaji kembali hubungan antara kondisi gizi lebih terhadap tekanan darah dengan fokus utama pada kelompok populasi remaja akhir-dewasa awal. Desain penelitian ini deskriptif observasional, dimana subjek mahasiswa program studi kedokteran FKIK Ukrida Angkatan 2023 berjumlah 63 subjek. Penelitian dikerjakan dengan mengukur tekanan darah, berat badan dan tinggi badan untuk menentukan nilai IMT. Berdasarkan hasil uji Fisher didapatkan p-value = 0,092 (>0,05) yang berarti tidak terdapat hubungan bermakna antara nilai IMT terhadap tekanan darah. Hal ini mungkin disebabkan karakteristik usia subjek penelitian yang bersifat homogen, yaitu pada kisaran usia, dimana obesitas “belum cukup” memperlihatkan manifestasi klinis yang bermakna. Namun demikian, ditemukan 4,8% subjek yang mengidap hipertensi memang merupakan penderita obesitas 2.

Abstract

Genetic therapy is a revolutionary innovation in the medical world that aims to treat or prevent diseases through the modification of the patient's genetic material. This literature review examines the latest advances in genetic therapy, including the biomedical laboratory techniques used. Techniques such as Polymerase Chain Reaction (PCR), DNA sequencing, CRISPR-Cas9, and Prime Editing have provided the ability to modify genes with high precision. Genetic therapy has shown success in various diseases, including monogenic genetic diseases such as hemophilia and Duchenne Muscular Dystrophy, as well as cancer through Chimeric Antigen Receptor T-cell (CAR-T) therapy. Additionally, research shows the potential of genetic therapy in addressing complex diseases such as cardiovascular disease and diabetes, as well as in regenerative treatment for degenerative diseases like Parkinson's. Despite being promising, challenges related to ethics, safety, and accessibility still need to be addressed. Case studies indicate that although there are many successes, there are still obstacles to be overcome to improve the effectiveness and safety of this therapy. This literature review concludes that genetic therapy has great potential to become an important pillar of modern treatment in the future, provided that further research and development are needed to overcome the various existing barriers.

Abstract

Learning experiences related to emergency response are essential for health sciences students as they prepare for clinical responsibilities. This study explores students' reflections on their experiences in learning emergency response interventions, emphasizing perceived challenges and expectations for improved learning processes. A descriptive qualitative design was adopted, and data were collected through in-depth interviews with 12 health sciences students who had recently completed a formal emergency response learning module. Thematic analysis revealed four key areas: difficulty in maintaining correct procedural techniques, limited familiarity with emergency devices, performance anxiety under simulated pressure, and insufficient opportunities for repetition and feedback. Students expressed the need for extended practice time, realistic scenario-based learning, and formative assessments to reinforce competence and confidence. Abstract

Sleep quality plays an essential role in maintaining overall health, including the proper function of the reproductive system, which is influenced by sex hormones and circadian rhythm regulation. Medical students frequently encounter high academic demands that may disrupt their sleep patterns. Previous research has suggested a possible association between sleep quality and menstrual cycle characteristics; however, findings have been inconsistent. This study aimed to examine the relationship between sleep quality and menstrual cycle regularity among female medical students from the 2023 at the Faculty of Medicine, Universitas Kristen Krida Wacana. A descriptive-analytic design with a cross-sectional approach was applied. Data were obtained through total sampling in November 2024 using the Pittsburgh Sleep Quality Index (PSQI) questionnaire to assess sleep quality and a self-reported menstrual cycle history. Of 67 respondents who met the inclusion and exclusion criteria, 51 (76.2%) reported poor sleep quality, while 16 (23.8%) had good sleep quality. Furthermore, 40 participants (59.7%) had normal menstrual cycles, and 27 (40.3%) reported irregular cycles. Statistical analysis revealed no significant association between sleep quality and menstrual cycle regularity (p = 0.794) among these students.
