Abstract

Latar Belakang: Diabetes mellitus merupakan penyakit metabolik dengan karakteristik hiperglikemia yang terjadi karena kelainan sekresi insulin, gangguan kerja insulin yang dapat menimbulkan berbagai komplikasi kronik. Tujuan: Penelitian ini bertujuan dengan melaksanakan asuhan keperawatan diabetes mellitus tipe II pada Tn. M dan Tn. B di RSUP Wilayah Jakarta. Metode: Karya tulis ilmiah ini menggunakan metode deskriptif analitik dengan pendekatan studi kasus dan mengambil dua pasien dengan diagnosa keperawatan yang sama. Metode pengambilan data meliputi wawancara, pemeriksaan fisik, studi dokumentasi. Instrumen pengumpulan data menggunakan format asuhan keperawatan medikal bedah. Hasil: Setelah dilakukan asuhan keperawatn selama 3x24 jam didapatkan hasil pada pasien Tn. M hasil masalah teratasi sebagian pada 3 diagnosa yaitu ketidakstabilan kadar glukosa darah karena hasil 208mg/dl, nyeri akut karena skala nyeri 3 (sedang) dan integritas kulit karena hasil luka terbuka, panjang luka 7cm, kedalaman 2cm dan berwarna kemerahan, kemudian pasien masih dalam perawatan. Sedangkan pada pasien Tn. B didapat hasil masalah teratasi sebagian pada 3 diagnosa yaitu ketidakstabilan kadar glukosa darah karena hasil 198mg/dl, nyeri akut skala 2 dan integritas kulit karena hasil luka terbuka, panjang luka 5cm, kedalaman 2cm dan berwarna kemerahan, kemudian pasien masih dalam perawatan. Kesimpulan: Berdasarkan hasil diagnosa keperawatan diatas diprioritaskan implementasi selama 3x24 jam dalam menangani masalah yang terjadi pada pasien DM Tipe II dengan melakukan tindakan monitor hasil kadar gula darah, memberikan terapi insulin, memberikan terapi relaksasi nafas dalam untuk mengurangi rasa nyeri dan memberikan perawatan luka dalam proses penyembuhan pasien dan diperbolehkan pulang.

Abstract

Background: Sharing information about nursing perception on the EHR is important. Nursing is a high utilizer of the EHR and without accurate and complete information in the EHR, patient safety risks increase. Aim: The objectives of this article was to explore nurses’ experience using Electronic Health Record (EHR). Method: Qualitative semi-structured interviews were conducted with 14 nurses in Jakarta hospital. Nurses were recruited from the inpatient room. The study was analyzed using Colaizzi's method of thematic analysis. Results: Nurses using EHRs in the inpatient room through the following items: 1) Data Processes; 2) barriers using EHR; and 3) nurse communication. Conclusion: Nurses are aware of the significance and standards of checking patient data, but their commitment is mitigated by some considerations including need or risk evaluation, the effect on their patient relationship, and hospital climate policies that protect the privacy of patients.

Abstract

Background: Type II DM is a chronic disease due to metabolic disorders which cause an increase in blood glucose levels. Diabetic ulcers are a complication of type II DM which occurs due to decreased peripheral circulation which is greatly influenced by high blood glucose levels and is closely related to peripheral arterial disease. Therefore, it is necessary to care for diabetic wounds and adhere to diet to avoid other complications. Research aim: The aim of this research is to apply nursing care to Type 2 DM patients with diabetic ulcers. Method: This research uses a descriptive analytical method with a case study approach which takes 2 patients with the same case. The research used a medical- surgical nursing care format including 5 nursing processes. Data collection techniques used include interviews, observation, physical examination and documentation studies. Results and discussion: After carrying out nursing actions in the form of wound care on Mr. After 3 x 24 hours the patient still complained of pain in the wound on the back when moving, the wound was still producing pus and the skin area around the wound was red and smelly. Leukocytes are still high at 15.12 10^3/uL. After carrying out nursing actions in the form of wound care on Mr. F, for 2x24 hours the patient still complained of pain in the leg wound when moving, there was no pus seepage but the skin around the wound looked reddish, dry, peeling. Leukocytes had decreased to 10.35 10^3/uL. Conclusion: The results of this case study concluded that the nurse's role is to recommend and teach diet, monitor blood sugar, exercise, take medication and care for wounds independently with diligent and routine check-ups with health services
