Abstract

Abstract Schizophrenia is a severe mental disorder characterized by significant distortions in thoughts and perceptions, accompanied by inappropriate emotions Antipsychotics are the mainstay of schizophrenia and require a long healing time and require high adherence to treatment. Most alternative antipsychotic therapies require pharmacoeconomic studies, particularly Cost Effectiveness Analysis which is useful in balancing patients’ expenditure by determining alternative treatments. The purpose of this study was to determine the effectiveness, eficiency and unit cost of the antipsychotic drug combination risperidone-clozapine dan risperidone-olanzapine. This research method used cross-sectional, analytical descriptive design and data collection is carried out prospectively from the medical record of schizophrenics, while detailed data on treatment costs are obtained from the financial section of inpatients at the hospital Dr. H. Marzoeki Mahdi Bogor from May to July 2019. The results of the study showed that the effectiveness of the largest antipsychotic combination therapy to increase the value of GAF was the combination of risperidone-olanzapine by 36 patients with an average difference in Initial GAF and Final GAF of 14.11, while the combination of risperidone-clozapine ie as many as 35 patients with an average difference in value Initial and Final GAF of 10.06. While the cost effectiveness based on the increase in final GAF value in the combination of risperidone-clozapine antipsychotic and risperidone-olanzapine antipsychotic combination in sequence is IDR.6,312,800. and IDR. 5,628,684. This value can be obtained from the calculation of the unit cost, the calculation of costs incurred in real where the cost is calculated for the service unit. It can be concluded that the risperidone-olanzapine antipsychotic combination is more cost effective than the risperidone-clozapine antipsychotic combination. Keywords: antipsychotics, effective and efficient, risperidone-clozapine combination, risperidone-olanzapine combination, schizophrenia.

Abstract

ABSTRACT Mental disorders are medical illnesses, not magic or mysticism (many Indonesian people still regard mental disorders as non-medical illnesses). The organ that is disturbed in people with mental disorders is the brain. The purpose of this study was to identify improvements in the SRQ score in mental patients undergoing TMS and Neurofeedback at the Marzoeki Mahdi Mental Hospital, Bogor. This study used a descriptive cross-sectional study, a sample of 30 patients in the TMS and Neurofeedback therapy rooms at Marzoeki Mahdi Mental Hospital, Bogor, West Java Province, using a purposive sampling technique. Data collection tools are SRQ 29 before and after TMS and Neurofeedback therapy. In a study conducted on 30 patients at the Marzoeki Mahdi Mental Hospital in Bogor, it can be concluded that there was an increase in the value of Self Reporting Questionnaire 29 (SRQ 29) after Transcranial Magnetic Stimulation (TMS) and Neurofeedback therapy for each interpretation. (Mental Emotional Disorders, Addictive Disorders, Psychotic Disorders and Post Trauma Syndrome (PTSD)), although the increase in SRQ scores was not very significant, this was partly due to the patient's irregularity in undergoing TMS and Neurofeedback therapy according to the scheduled time, and could also be due to medication inadequate pharmacological, psychoeducational & other therapies. Keywords: Self Reporting Questionnaire, Mental Disorders, Transcranial Magnetic Stimulation  ABSTRAK Gangguan jiwa adalah penyakit medis, bukan gaib atau mistis (masyarakat Indonesia masih banyak yang menganggap gangguan jiwa adalah penyakit non medis).

Abstract

Mental disorders are one of the public health problems in Indonesia. In general, mental disorders are caused by psychological pressure both from outside the individual and from within the individual. Some of the things that cause this is the ignorance of the family and society about this mental disorder (Hawari, 2007). According to WHO or the World Health Organization (2002) states that the prevalence of mental health problems is currently quite high, 25% of the world's population has suffered from mental health problems, 1% of which are severe mental disorders. The potential for a person to be susceptible to mental disorders is indeed high, every time 450 million people around the world are affected by mental, nervous, and behavioral problems. One form of mental disorder that is found throughout the world is a severe mental disorder, namely Schizophrenia (Hawari, 2007). Schizophrenia comes from two words, namely schizo which means cracked or broken and phrenia which means soul. Thus, people who suffer from schizophrenic mental disorders are people who experience mental fractures or personality cracks. Schizophrenia can arise because of the integration between biological, psychosocial and environmental factors (Sinaga, 2007). In schizophrenic patients there is personal disintegration and personal division. Emotional and intellectual behavior becomes ambiguous (compound), and has serious disorders and has regression or total dementia. Schizophrenic patients escape from the reality of life and remain silent in their fantasy world (Kartono, 2010).
