Navigating Surgical Strategies for Symptomatic Tarlov Cysts: A Case Report of Successful Microsurgical Excision and Imbrication Arip Heru Tripana; Tondi Maspian Tjili; Afdal; Ismar; Eko Setiawan

Abstract

Background: Symptomatic Tarlov (perineural) cysts are a rare but debilitating cause of chronic radicular pain, arising from cerebrospinal fluid (CSF) accumulation within the nerve root sheath. The diagnostic journey is often complex, requiring a definitive link between the lesion and the patient's symptoms, and the optimal surgical strategy remains a subject of considerable debate. Case presentation: A 56-year-old male presented with a six-month history of intractable right S2 radiculopathy (VAS 8/10) and severe functional impairment (Oswestry Disability Index 78%), which had failed a comprehensive trial of conservative management. Magnetic resonance imaging revealed a large cystic lesion at the right S2 level, with features characteristic of a Tarlov cyst, causing severe nerve root compression. Following a thorough discussion of the risks and benefits, the patient underwent an S2 laminectomy with microsurgical partial cyst excision and wall imbrication. Postoperative histopathology confirmed the diagnosis of a perineural cyst, identifying nerve fibers within the fibroconnective tissue of the cyst wall. Conclusion: The patient experienced immediate and sustained resolution of his radicular pain (VAS 0/10) and a profound improvement in functional status (ODI 12%) at one-year follow-up, with radiological confirmation of successful cyst obliteration. This case highlights the potential of a direct microsurgical approach, guided by a strong clinical-radiological correlation and confirmed by histopathology, to provide a durable and life-altering cure for patients disabled by symptomatic Tarlov cysts.

Abstract

Keamanan kata sandi (password) merupakan aspek krusial dalam menjaga integritas sistem informasi digital. Namun, banyak pengguna masih menggunakan password yang lemah dan mudah ditebak, sehingga rentan terhadap serangan siber. Penelitian ini bertujuan untuk mengklasifikasikan kekuatan password dan menjelaskan hasil prediksi dengan pendekatan Explainable Artificial Intelligence (XAI), khususnya SHAP (SHapley Additive exPlanations) dan LIME (Local Interpretable Model-agnostic Explanations). Dataset yang digunakan berisi 100.000 password yang telah dilabeli dalam tiga kelas kekuatan password. Proses pra-pemrosesan mencakup ekstraksi fitur berbasis struktur karakteristik password, seperti panjang, huruf besar, angka, dan karakter spesial. Model Naive Bayes yang dibangun menunjukkan performa klasifikasi yang sangat baik dengan skor akurasi: 97,66%, Precision: 94,51%, Recall: 98,23%, dan F1-score: 96,22%. Selanjutnya, analisis XAI dilakukan untuk mengungkap kontribusi fitur terhadap keputusan model, baik secara global maupun lokal. Hasil visualisasi menggunakan SHAP dan LIME menunjukkan bahwa fitur panjang dan keberadaan karakter kapital memberikan pengaruh signifikan terhadap kekuatan password. Selain itu, juga dilakukan interpretasi terhadap prediksi lokal (analisis per-password) sehingga memberikan gambaran kontribusi fitur terhadap setiap kekuatan password. Sebagai implementasi akhir, sebuah antarmuka interaktif berbasis Streamlit dikembangkan untuk memungkinkan pengguna melakukan prediksi dan interpretasi kekuatan password secara real-time.

Abstract

The website of Pondok Pesantren Al-Kamal Kebumen serves as a medium for delivering information and providing pesantren services. However, its utilization remains suboptimal due to limited content updates, which has led to low visitor interest. Despite this, the website continues to make a positive contribution by expanding communication reach, facilitating information access, and enhancing transparency in pesantren activities. This study aims to evaluate the website's success level using the DeLone and McLean Model (2003), which includes six dimensions: system quality, information quality, service quality, use, user satisfaction, and net benefit. A descriptive quantitative method was employed through the distribution of a 4-point Likert scale questionnaire to 210 student respondents. The data were then analyzed using Structural Equation Modelling (SEM) based on Partial Least Square (PLS). The results show that out of nine hypotheses tested, six were accepted and three were rejected. Service quality and system quality significantly influence both use and user satisfaction. Use also has a significant effect on user satisfaction, while user satisfaction significantly affects net benefit. However, information quality does not significantly affect use or user satisfaction, and use does not significantly affect net benefit. Overall, the website's success level falls into the moderate category, with the R-square value for the net benefit variable at 42.5%. These findings indicate that a portion of the benefits perceived by users can be explained through the research model, although other external factors beyond the model also have an influence.

Abstract

Sexual crimes of pedophilia will have a negative impact for children.Not only undermine the future of the physical, but also mental andpsychological damage the child, such as major depressive disordermay be brought later into adulthood. Moreover, most people withpedophilia caused because he had been the victim of sexual abuse similarin childhood. The criminal acts of pedophilia is very detrimental to thevictims and the wider community. Therefore, the victims need care andprotection of the law. The law in Indonesia that ensnare the practice ofpedophilia was not serious. So that the penalty for pedophiles is notcommensurate with what has been done and the risk of future damageto the victims. Besides the protection of the public for the victims ofpedophiles are also very less.

Abstract

Madura grocery stalls are found in many corners of major cities in Indonesia. In general, it only sells a variety of daily necessities at relatively affordable prices but has unique operating hours of 24 hours. So that it can compete in the midst of the siege of modern franchises, the turnover obtained can be said to be quite fantastic. This type of research is a case study (case study) part of a qualitative method that aims to understand a phenomenon by providing a complete picture, views, and behavior of informants in detail based on natural situations. Data collection methods use observation, unstructured interviews, and documentation. Data analysis using interactive models, through the stages of data reduction, data presentation, and conclusion drawing. The results showed that the existence of Madurese grocery stalls could grow rapidly and dare to challenge well-known franchise minimarket networks because they applied kinship-based business patterns. They build social cohesion based on mechanical solidarity, describing how strong the cohesiveness or attachment of society is.

Abstract

Blended Learning practiced by the PalComTech Institute of Technology and Business in the teaching and Learning activity system describes a combination of 3 teaching and Learning activity systems face-to-face (offline), online (online), and application support. The PalComTech Online Learning Android application is an application for teaching and Learning activities used by the PalComTech Institute of Technology and Business in supporting the ease of teaching and Learning activities online (online). To support teaching and Learning activities digitally, this application provides Learning modules in the form of video material and pdf material. The application of technological sophistication along with the development of the user-friendly PalComTech Online Learning Android Application will run better in carrying out the teaching and Learning process and training easily and can also create a generation of students who are aware of the use of digital technology. Therefore, this research tested the level of Usability in using the PalComTech Online Learning Android application with the PACMAD (People at the Center of Mobile Application Development) method which consists of factors of Effectiveness, Efficiency, Satisfaction, Learnability, Errors, Memorability, and Cognitive Load.
