Abstract

Liver cirrhosis is the last stage of chronic liver disease that is in the path of the disease can cause damage of liver function and/or portal hypertension. The prognosis of liver cirrhosis can be assessed by manifestations of liver function disorders with Child Pugh score that include the presence or absence of ascites, encephalopathy, bilirubin serum, albumin serum, and prothrombin time or INR, while portal hypertension also occurs in patient of cirrhosis can lead the formation of esophageal varices. APRI score has platelet count and AST serum as its variable is suspected to be related to the Child Pugh score in assessing prognosis of cirrhosis patients. This study purposed to find the relation between APRI score (Aspartate Aminotransferase-to-Platelet Ratio Index) and Child Pugh score. Purpose of this research is to find the relation between APRI score (Aspartate Aminotransferase-to-Platelet Ratio Index) and Child Pugh score and degree of esophageal varices in patients with cirrhosis. Method used is a retrospective analytic study with cross sectional approach. The data source is secondary source. The sample in this study were patients with liver cirrhosis who were diagnosed by a doctor and had complied the inclusion criteria. The data obtained was analyzed using ANOVA. The result of this study is include 48 cirrhosis patients, male 81,3%, with a mean age 49,98 years. The most common cause of cirrhosis is hepatitis B obtained from reactive HBsAg examination in 33 patients (68,8%). The APRI score was found to have a weak correlation (r = 0.044) and not significant (p = 0.868) with the Child Pugh score. The APRI score do not able to assess the prognosis in advanced cirrhotic patients.

Abstract

Interesterification of castor oil with methanol using base catalyst gave mixture of fatty acid methyl esther (FAME) castor oil. Methyl risinoleate as major composition of methyl esther castor oil was subjected to column chromatography using petroleum ether:diethyl ether (19:1, v/v) as eluent, to give yield 73%. Amidation of methyl risinoleate with ethylendiamine under refluks condition using benzene as solvent for ± 12 hours and catalyst NaOCH3, gave 1,3–Dirisinoleil-Etilendiamida compound and 59% yield. The reactioned product 1,3–Dirisinoleil-Etilendiamida has been confirmed its structure using FT-IR spectroscopy, and Hidrofile Lipofile Balance (HLB) value was determined by titration method 12,56

Abstract

Research has been carried out on the synthesis of methyl malate through esterification reactions using methanol solvents and in situ reactions with acid catalysts. Esterification of malic acid was carried out with a ratio of methanol 1:3 to produce pale yellow methyl malate with a yield of 94.06%. The formed methyl malate then characterizes using the FT-IR instrument and tests its HLB value. The result of FT-IR analysis which will indicate that methyl malate has formed is that the carboxylic group from malic acid has been converted into an ester group from the esterification reaction. The wavenumber of the carboxylic group of malic acid is 1720.50 cm-1 and the ester group is the result of an esterification reaction at wave number 1735.93 cm-1, indicating that methyl malate has been formed. Furthermore, for the HLB value based on the results of the study obtained at 11.2, which indicates that methyl malate is included in the O / W emulsifier (Oil in Water).

Abstract

Cardiorespiratory endurance is the ability of the body's circulatory and respiratory system to meet the oxygen needs of the muscles as a source of energy production during sustained physical activity. One of the factors that influenced the level of cardiorespiratory endurance of a person was physical activity. Objective: To determined the relationship between physical activity and cardiorespiratory endurance. Methods: It was an analytic observational with a cross-sectional study. Data were taken from primary data that were measured directly on 97 students of the Medical Study Program at Mulawarman University who were selected using a purposive sampling technique. The data collected included physical activity using the Global Physical Activity Questionnaire (GPAQ) and cardiorespiratory endurance using Queen's College Step Test. The statistical analysis used was Spearman's correlation test with a significance value of p <0.05. Results: The statistical test showed that the relationship between physical activity and cardiorespiratory endurance was p = 0.003 and r = 0.295. Conclusion: There was a significant positive relationship between physical activity and cardiorespiratory endurance, and the relationship's strength was weak.Keywords:  cardiorespiratory endurance, global physical activity questionnaire, physical activity, queen’s college step test

Abstract

Coptosapelta flavescens Korth. is a liana plant from the Rubiaceae family. In East Kalimantan, it is also known as “Akar Tambolekar” or “Merung”, it is called “akar” or root as its trunk spreads like a root. The plant’s stems are used by ethnic Dayak in East Kalimantan to overcome high blood pressure and it has been proven to induce vasorelaxation on blood vessels, but the its action mechanism in the endothelial or vascular smooth muscle cells (VSMC) are unknown. Vasorelaxation on the blood vessels could be mediated by endothelial or by the VSMC. This research aims to study in vitro which of them is the mechanism of action of Coptosapelta flavescens Stems (CFS) extract. CFS were taken from the secondary forest in Paser Regency, East Kalimantan Province. Its simplicia were macerated with methanol solvent for three days and twice repeated. Vasorelaxation activity of the blood vessels was tested with rats’ isolated thoracic aorta with endothelial and with the endothelial removed (endothelium-denuded). Both aorta of 3 mm length were soaked into Krebs-Henselheit solution at 37°C, pH 7.4 and aerated with carbogen gas. After they have acclimatized, both aortas were contracted with phenylephrine solution, after reaching peak contraction and plateau, the solution of extract or its solvent (Control) was administered in cumulatively increasing doses. The results show that CFS extracts induce vasorelaxation both in the endothelial-intact aorta and in the endothelial-denuded aorta. At high concentrations, the vasorelaxation activity in the endothelial-intact aorta was weaker than that on endothelial-denuded aorta. This study proves that action mechanism of blood vessels’ vasorelaxation induced by the methanol extract of CFS were more dominantly mediated by the blood vessels smooth muscle; and at high concentrations the endothelial actually weakened the VSMC vasorelaxation activity. Abstract

Pain due to smooth muscle stiffness can occur in various circumstances of patients, ranging from normal circumstances such as menstrual pain, to a pathological condition of gastrointestinal infections. To overcome the pain it can be given medication to reduce muscle stiffness as spasmolytics, but there are side effects of the drug. Thus, the need for developing new spasmolytics drugs based on medicinal herbs with fewer side effects. This study was conducted to examine the potential of rhizome extract of jeringau (Acorus calamus L.) that is traditionally used by Dayak tribe to heal upset stomach. Spasmolytic activity of plant extracts tested with isolates organ methods of guinea pig ileum induced by histamine. Result of the study showed 0.3 mg/ml A. calamus rhizomes extract could decrease ileum contraction better than papaverine 0.03 mg/ml (p=0.00). The conclusion of this study is rhizome extract of jeringau (A. calamus) has the potential to be used as spasmolytics in treatment of health problems caused by smooth muscle contraction

Abstract

In the setting of acute myocardial infarction (AMI), an increased in cardiac biomarker levels is a parameter supporting the diagnosis of AMI. There are several types of cardiac biomarkers, including CK-MB, troponin T, troponin I, LDH, and myoglobin. Troponin is one of the recommended cardiac biomarker owing to its high specificity. Besides indicating the occurrence of AMI, troponin T is also known to have an association with ischemic duration, degree of ischemia, extent of infarction, complications, and even mortality among AMI patients. These parameters are thought to be the factors that influence the length of stay and in-hospital mortality in AMI patients. Therefore, the author was interested to determine the relationship between troponin T levels with treatment duration and in-hospital mortality in AMI patients. This analytical study was conducted using cross-sectional approach and secondary data. The study samples are AMI patients treated at Abdul Wahab Sjahranie Hospital in Samarinda between January 1, 2018 - December 31, 2018 who met the inclusion and exclusion criteria. In total, this study involved 86 people. The results show that the average troponin T levels in AMI patients is 1159.93 pg/ml, the average length of stay is 5.11 days, and the percentage of mortality during treatment is 10.5%. Bivariate analysis shows that troponin T does not have a significant relationship with length of stay (p = 0.422) and in-hospital mortality (p = 1,000). Thus, it can be concluded that troponin T levels are not associated with length of stay and in-hospital mortality in AMI patients. Abstract

Coronary Artery Bypass Graft (CABG) is a surgical procedure used to create bypass around the blocked coronary artery). Packed Red Cell (PRC) is required during CABG to balance hemoglobin level and the amount of red blood cell tranfused is basically estimated by observing preoperative hemoglobin level. Based on these reasons, the researcher aims to find the correlation between preoperative hemoglobin level and intraoperative red blood cell transfusion. The observational analytical design using cross sectional method was applied. The purposive non-probability sampling was used to select the research samples. Thirty-five samples fulfilling the inclusive and exclusive criteria were obtained. The findings revealed that patients’ preoperative hemoglobin level was normally transfused with 0 – 249 cc red blood cell for 18 intraoperative patients (51.4%). Patients’ preoperative hemoglobin level normally transfused with 250 – 500 cc red blood cell was only five patients (14.3%). The abnormally preoperative hemoglobin level transfused with 250 – 500 cc red blood cell was for 10 intraoperative patients (28.6%), whereas the abnormally preopreative hemoglobin level transfused with 0 – 249 cc red blood cell was only two patients (5.7%). It can be concluded that there is a correlation between preoperative hemoglobin level and intraoperative red blood cell transfusion with p value = 0.000 (p <0,05).
