Abstract

Fatigue adalah kondisi fisiologis yang ditandai dengan penurunan kapabilitas performa fisik dan mental akibat kehilangan tidur atau terjaga berkepanjangan, perubahan irama sirkadian dan beban kerja yang dapat mengganggu kewaspadaan dan kemampuan personil penerbangan dalam mengoperasikan pesawat atau tugas penerbangan dengan selamat. Insiden atau kecelakaan penerbangan dapat timbul akibat kesalahan di tingkat individu (penerbang atau awak pesawat lainnya), dimana fatigue menjadi salah satu faktor penyebab utama.International Civil Aviation Organization (ICAO) telah merekomendasikan Fatigue Risk Management System (FRMS) kepada negara-negara anggotanya, termasuk Indonesia. FRMS ini merupakan suatu sistem adalah suatu sistim pengawasan dan pengelolaan faktor-faktor risiko terkait kelelahan berbasis bukti, yang dilakukan secara berkesinambungan. Di Indonesia saat ini belum banyak maskapai yang menerapkan FRMS. Sebagai upaya lain untuk mengelola fatigue di penerbang, maka dibutuhkan panduan langkah-langkah praktis untuk mengelola faktor risiko fatigue di penerbang, khususnya terkait aktivitas fisik, aktivitas di waktu senggang atau istirahat dan pengaturan waktu tidur.Buku Panduan Praktis Pengelolaan Fatigue bagi Penerbang dengan Aktivitas Fisik, Latihan Fisik dan Waktu Tidur ditargetkan untuk menjadi acuan pengelolaan fatigue bagi penerbang, maskapai dan dapat menjadi rekomendasi bagi pemangku kebijakan untuk dapat dijadikan bagian dari prosedur keselamatan penerbangan di Indonesia.

Abstract

Industri penerbangan bekerja sama dengan pihak pemerintahan setempat dan otoritas kesehatan masyarakat bertanggung jawab untuk mengkomunikasikan potensi risiko kesehatan masyarakat secara tepat dan sesuai. Bagi orang yang akan bepergian dapat melaksanakan tanggung jawabnya untuk membantu meminimalkan risiko penularan penyakit dengan menunda atau menghindari bepergian, terutama bagi orang yang sakit, lanjut usia, orang dengan penyakit kronis atau penyakit dasar lainnya. Orang dengan penyakit menular yang aktif tidak boleh bepergian melalui udara. Maskapai penerbangan juga dapat menolak penumpang naik pesawat bila diduga mempunyai penyakit infeksius yang menular.

Abstract

Introduction: Pilots have job characteristics that are at risk of causing fatigue. Since fatigue is not a disease, it is important to manage the risk factors. We developed a self-assessment application to manage the fatigue risk for pilots, including physical activity and sleeping difficulties. This application aims to increase pilots’ awareness as one of the health maintenance measures.Methods: The application was developed in collaboration with Garuda Indonesia Training Centre (GITC) to ensure suitable educational content for pilots. The users were asked to fill in data inquiring about their flight characteristics, the Checklist Individual Strength-20 for fatigue screening, General Physical Activity Questionnaire for physical activity, and Jenkins Sleep Scale for sleep difficulties. Afterward, a recommendation will be given based on the received input.Results: The application was able to screen fatigue risks, physical activities, and sleeping difficulties, and provide recommendations based on the questionnaire that was filled in, all adjusted to the user’s work characteristics. The finalized version was then available for download as an Android-based application for the pilots in GITC.Conclusion: The FRAMES is an Android-based self-assessment application for pilots. With the use of this application, hopefully, fatigue can be detected early in pilots, hence counteract measures may be implemented sooner to mitigate fatigue.

Abstract

Introduction: Helicopter pilots are potentially at risk of Low Back Pain (LBP) and are twice as likely as fighter pilots to experience it. Helicopter pilots’ LBP is caused by a combination of risk factors: engine and rotor vibration, ergonomic risks due to cockpit design, static seating position due to operations, and individual factors. These factors are incorporated into cumulative aviation risk exposure, which can be measured by total flight hours. The prevalence of LBP in Caucasians is high, at 50.5%-92%. However, there has not been any research on Indonesian military helicopter pilots who have a different posture from Caucasians.Objective: To determine the prevalence of LBP, to analyze the relationship between total flight hours and individual factors (age, height, BMI, exercise habits, and smoking habits) with LBP in Indonesian military helicopter pilots.Methods: The study was conducted at the Indonesian Army and Indonesian Air Force with total sampling method as many as 124 people. An explanation was carried out followed by filling in the consent to become a research subject, filling in personal data, anamnesis, Numeric Rating Scale (NRS), physical examination, neurological examination, and total flight hour data.Results: There were 124 subjects, consisting of 37.9% pilots and 62.1% copilots, 87.9% of pilots operate large helicopters, have average total flight hours of 450 hours, age of 30 years, the height of 172.66 cm, low intensity exercise habits of 61.3%, and smokers as much as 45.2%. A total of 57 people (46%) experienced LBP. Statistical results showed that total flight hours had an association with LBP (p = 0.035) where each 1 unit increase in total flight hours had a 1.02 times greater chance of LBP.Conclusion: The prevalence of LBP is 46% and there is an association between total flying hours and LBP in Indonesian military helicopter pilots.

Abstract

Fatigue is defined as a physiological state of reduced mental or physical performance capability resulting from sleep loss or extended wakefulness, circadian phase, or workload (mental and/or physical activity) that can impair a crew member’s alertness and ability to safely operate an aircraft or perform safety-related duties. An aircraft incident or accident may arise as a result of an error at an individual level (aviator or other crew), where fatigue is one of the main contributing factors.The International Civil Aviation Organization (ICAO) has recommended Fatigue Risk Management System (FRMS) to its member States, including Indonesia. This FRMS is a monitoring system and risk factors management related to fatigue-based burnout, which is carried out continuously. In Indonesia, there are currently few airlines that implement FRMS. It takes practical steps to manage fatigue risk factors in the aviator, especially concerning physical activity, activity in leisure or resting time and time management Sleep.A practical guideline of Fatigue Management for aviators with physical activity, physical exercise and sleeping arrangement is targeted to become a reference to the Fatigue management for pilots, airline management and can be a recommendation for policy stakeholders to become part of the flight safety procedures in Indonesia.

Abstract

Introduction: Military pilots require good cardiorespiratory fitness due to the need to perform tasks in difficult and varied environmental conditions. Cardiorespiratory fitness is usually expressed in terms of maximal oxygen consumption (V̇O2 max). It is important to know the factors associated with V̇O2 max in order to maintain the fitness of Indonesian Air Force pilots. This study was conducted to assess the association between body fat percentage with V̇O2 max of Indonesian Air Force pilots.Methods: A cross-sectional study was conducted on active Indonesian Air Force pilots who carried out periodic medical examinations from October to November 2022 at Lakespra dr. Saryanto, Jakarta. The research subjects underwent body composition examinations using Bioelectrical Impedance Analysis (BIA), and V̇O2 max assessment was conducted using the Bruce protocol. The association between body fat percentage and V̇O2 max value was assessed by Pearson’s correlation test.Results: Of the 64 subjects analysed, the average value of V̇O2 max was 44.6±6.2 ml/kg/min and the average body fat percentage was 23.4±5.0 %. There is a significant correlation between body fat percentage and V̇O2 max. (r=-0.334; p=0.007)Conclusion: There is a significant negative correlation with moderate strength between the body fat percentage and the V̇O2 max of Indonesian Air Force pilots who carried out medical examination at Lakespra dr. Saryanto.

Abstract

Introduction: Burnout is a work-related stress condition commonly associated with long working hours and high job demands, making health quarantine workers particularly vulnerable. Evidence on the role of age in burnout is inconsistent, although older workers are often assumed to be at higher risk. This study investigates the relationship between age and burnout among health quarantine workers to determine which age group is more prone to burnout and to provide evidence that can guide early preventive interventions. Methods: A cross-sectional study was conducted in September–October 2025 using secondary data from a survey by PT Y. Burnout was measured using the Maslach Burnout Inventory (MBI) questionnaire. Results: Burnout prevalence among health quarantine workers was 25.4%. Multivariate analysis showed a p-value of 0.049 for age. Statistical tests showed p-values > 0.05 for the relationships between gender, education level, marital status, having children under five years old, work patterns, length of service, work area, employment status, or commuting distance with overall burnout. Conclusion: More than one-fourth of health quarantine workers experienced burnout, with younger workers showing the highest risk. Age was the strongest predictor of burnout, indicating greater vulnerability among younger staff. Targeted support for early-career workers is essential to reduce burnout risk and strengthen workforce well-being.

Abstract

Background: Low back pain (LBP) is a common musculoskeletal disorder among informal sector workers, including blacksmiths. particularly because of non-ergonomic work positions.Objective: This study aims to identify the prevalence of low back pain (LBP) and its associated characteristics among blacksmith workers in Mekarmaju Village, Bandung Regency.Methods: A cross-sectional study was carried out among 105 blacksmiths with interview, the Bodymap Discomfort questionnaire, the Visual Analogue Scale (VAS), physical examination, and measurement of trunk flexion angle using APECS version 8.05.07. In this study, LBP among blacksmith workers was characterized as chronic non-specific low back pain.Results: LBP prevalence among blacksmiths was 58.1%. Factors significantly associated with LBP included forward-bending posture (p=0.034; aOR 3.7; 95% CI 1.1–12.5), static sitting posture for ≥4 hours (p=0.009; aOR 3.6; 95% CI 1.4–9.4), rest duration <0.5 hours (p=0.006; aOR 7.9; 95% CI 1.8–35.3), and age ≥35 years (p=0.006; aOR 4.6; 95% CI 1.6–13.6). Other occupational and individual factors didn’t have a significant correlation with LBP.Conclusion: These results indicate the necessity for immediate ergonomic interventions and health assessments at the beginning of employment for preventing LBP among informal sector workers.
