Abstract

Abstrak.  Beberapa  penyakit  seperti  kanker,  penyakit   herediter  dan  genetik    dan juga  penyakit  akibat virus rnenunjukan hasil yang kurang memuaskan  pada pcngobatan secara konvensional.   Pada beberapa tahun terakhir ini para ahli dan perusahaan  farmasi berusaha  untuk menemukan  suatu bentuk terapi  yang berbasis pada gen. Adapun  mckanisme  dari terapi  gen secara  ringkas berupa penambahan  gen normal  ke dalam  gen yang tidak normal, melenyapkan gen yang abnormal,  mereparasi gen yang abnormal,   dan mengendalikan  regulasi  ekspresi gen  yang  abnormal.  Untuk  melaksanakan    terapi  gen  sangat  diperlukan   adanya  vektor  untuk  mernasukkan materi gen terapi ke dalam tubuh host. Perkembangan terapi gen semakin nyata dengan ditemuannya  metoda­ metoda terapi gen terbaru seperti  RNA interference yang diharapkan  akan menghasilkan   tingkat  keberhasilan yang tinggi dalarn  terapi gen.  Kanker dan penyakit kardiovaskular  merupakan  pcnyakit­penyakit   yang paling sering mendapatkan  uji klinis terapi gen.  Sampai saat ini terapi gen masih terbatas  pada uji  klinis,  mengingat masih banyaknya kelemahan  padajenis  terapi ini, disamping adanya alasan  etikal  yang sampai sekarang masih diperdebatkan.  (JKS2008; 3:167-174)Kata kunci  : terapi gen, vektor, uji k:linisssAbstract. Some diseases, such as cancer, hereditary   and genetic  diseases,   as well  as viral  infectious   diseases, have been treated unsatisfied by the conventional   therapy, and even more, by the gene therapy.  Together with the  pharmaceutical   industries,   researchers  put   their  best  effort  to  hunt  some  molecules  that  can  be more favorable  for such kind  of therapy.  The  mechanism   of gene therapy  is  the added  the normal gene into the abnormal gene,  delete  the abnormally  gene,  reparation the  abnormally  gene,  and  regulate  the expression of abnormally gene.  Gene therapy need the vector to add gene material   to host. The development of  gene therapy is  increase  with new methode of gene therapy such as RNA interference.  Cancer  and cardiovascular   disease  are the diseases of more common get gene therapy.  Now,  the gene therapy  still limited   in  clinical trial  and make more debate to use as the modalities therapy. Abstract

Abstrak. Tuberkulosis (TB) merupakan penyakit infeksi kronis yang masih menjadi masalah kesehatan dunia sehingga perlu mendapatkan perhatian yang besar. Sampai saat ini belum ada satu negara di dunia yang telah bebas dari penyakit TB. Di Indonesia, jumlah kasus TB merupakan peringkat ke-3 terbanyak di dunia dalam 22 negara yang dikategorikan sebagai High Burden Countries terhadap TB. Sulitnya pengobatan penderita TB menyebabkan terjadinya kegagalan pengobatan. Pengobatan yang tidak teratur, penggunaan obat anti tuberkulosis (OAT) tidak adekuat ataupun pengobatan terputus menimbulkan kuman yang resisten.Tujuan dari penelitian ini adalah untuk mengetahui pengaruh dukungan keluarga, pengetahuan dan pendidikan penderita tuberkulosis terhadap kepatuhan minum obat. Penelitian dilakukan di kota Banda Aceh dengan mengambil sampel seluruh penderita TB di tiga kecamatan yaitu Ulee Kareng, Batoh dan Kuta Alam. Penelitian dilakukan dengan metoda wawancara terpimpin. Adapun hasil yang didapat berupa 90,5% dari responden tidak patuh dalam  mengkonsumsi  obat,  33,3% responden  memiliki pendidikan menegah,  61,9%  responden  memiliki pengatahuan yang rendah tentang TB, dan 48% responden memiliki dukungan keluarga dalam tingkat sedang. Dijumpai hubungan yang bermakna antara dukungan kelurga dengan kepatuhan minum obat. Hal ini menunjukkan   bahwa   keluarga   memiliki   peranan   penting   dalam   proses   pengobatan   penderita   TB. (JKS 2010; 3:131-136) Kata Kunci: Tuberkulosis, kepatuhan minum obat, dukungan keluarga, pengetahuan, pendidikan Abstract.  Tuberculosis is the chronic infectious disease that make common health problem in the world. At the time no one country free from this disease. Indonesia is the 3rd rank of TB population in the world. The difficulties to manage this problem are make increasing of treatment failure. lngradual treatment, inadequate therapy and lost of therapy make bactery resistence. The objective in this research is to know influence of familiy support, knowlegde ang education grade in tuberculosis patient towards drugs compliance. For this purpose indepth interview has been done to all of TB patient in three district in Banda Aceh regency i.e, Ulee Kareng, Batoh and Kuta Alam. The result of this research show 90,5% respondence have no good compliance in TB treatment, 33,3% in moderate grade in education level, 61,9% have lower knowledge in TB and 48% have a moderate level in family support. It have founded relation between family support and patient compliance. The result show that family support have an impotant role in TB patient treatment. Abstract

Abstrak.    Plumbum  merupakan  salah   satu  zat polutan yang ada di sekitar   kita,   Plumbum  dapat  berasal  dari proses  industri   dan  transportasi.  Keberadaan  plumbum  sebagai  kontaminan  saat  ini sudah  semakin  meluas sehingga   plumbum  sudah  dapat  k.ita temui  pada  ikan  segar, sayur-sayurarn,  buah-buahan,   bahka  pada  air minum.  Anak-anak  sebagai   kelompok  masyarakat  yang sangat rentan  terhadap  pencemaran  plumbum   akan dapat mengalarni  cfek buruk dari pencernaran plumbum apabila paparan plumbum  mencapai  kadar tertentu.  Hal ini disebabkab  anak-anak  lebih  banyak  mcngabsorpsi   plumbum  dibandingkan  kelompok  dewasa,  Pengaruh buruk  plumburn   terhadap  anak-anak  meliputi    hambatan  perkembangan  anak  pada    sistem  syaraf  termasuk penurunan  intelegential  quotient  (IQ), hambatan  pada sistem respirasi,dan  gastrointestinal.   Proses  toksik dari paparan  plumbum   dapat  diatasi   dengan  pemberian  EDTA  intravena,  namun  hal  yang   terpcnting   adalah kemampuan  kita  dalam  mencegah  dan  meminimalkan  efek  negatif plumbum  khususnya  terhadap  anak-anak, (JKS 2009; 1:51-56) Kata Kunci  :  Plumbum, lntelegential   Quotient (IQ), Perkembangan anak Abstract.   Plumbum  is one of the pollutant that is free around us.  Plumbum can come from industrial  processes and  transportation.   The  existence  of plumbum  as  contaminant  at this  time  is  more  widespread.  Plumbum already met in the fresh fish, vegetable, fruits,  and also  in drinking water.  Children as a group of people who are very vulnerable  to polJution  plumbum  can have bad effects of pollution exposure.  P!umbum  bad influence on children  include  child  development   constraints  on the system, including   a decrease  in  lntelegential   Quotient (IQ), constraints  on the  system  respiration,  and gastrointestinal.  Toxic  process  of exposure  can be overcome with plumbum  of EDTA intravena,  but the most important is our capacity to prevent and minimize the negative effects plumbum   especially  on children.  (JKS 2009; 1:51-56) Keywords: Plumbum, lntelegential  Quotient (IQ), children development

Abstract

Pendahuluan: Praktik Interprofessional Collaboration (IPC) merupakan bentuk kerjasama antar tenaga kesehatan yang dilakukan secara kolaboratif untuk memastikan asuhan pasien berjalan secara optimal dan berkesinambungan. Profesional Pemberi Asuhan (PPA) dengan profesi dan keilmuan yang berbeda dituntut untuk bisa berkontribusi secara individu maupun dalam tim. Penelitian ini bertujuan untuk mengetahui gambaran praktik tim dan kerjasama tim antar PPA dalam implementasi Interprofessional Collaboration di RSUD Meuraxa Kota Banda Aceh.Metode: Penelitian ini merupakan penelitian kualitatif dengan pendekatan fenomenologi menggunakan metode in-depth interview dan Focus Group Discussion (FGD). Data direkam, ditranskripsi kemudian dianalisis menggunakan analisis tematik. Informan penelitian ini sebanyak 12 orang PPA yang terdiri dari Dokter Penanggung Jawab Pasien (DPJP), perawat, apoteker, dan dietisien. Penelitian dilakukan di RSUD Meuraxa Kota Banda Aceh selama bulan Juni-Juli tahun 2020.Hasil: Hasil penelitian menggambarkan bahwa tim dan kerjasama tim secara interprofessional di RSUD Meuraxa belum efektif. Kegiatan visite bersama yang melibatkan keempat profesi belum pernah dilakukan. Profesi apoteker dan dietisien yang dibutuhkan dalam melakukan kolaborasi tidak cukup dari segi kuantitas. Perawat sesuai kompetensi klinis dalam melaksanakan kolaborasi belum sesuai. PPA belum memahami konsep IPC dengan baik sehingga dominansi kerjasama tim dilakukan oleh profesi dokter-perawat.Diskusi: Rumah Sakit harus melakukan tela’ah staf untuk menyediakan dan mendistribusikan PPA yang cukup secara kuantitas maupun kualitas sesuai kebutuhan kolaborasi. Sosialisasi tentang regulasi dan pelaksanaan Interprofessional Collaboration perlu dilakukan secara berkala agar PPA memahami konsep IPC dengan baik.

Abstract

Objective: To determine the relationship between the quality of informed consent in terms of ethics and medicolegal with patient health service satisfaction at the dr. Zainoel Abidin Hospital (ZAH) Banda Aceh.Method: This study is an observational analytic with a cross-sectional method. Subject selection through a total sampling method. This study involved patients who were treated at dr. Zainoel Abidin Hospital (ZAH) Banda Aceh and a total of patients were 100 (52 men and 48 women) who met the inclusion criteria carried out on 16 December 2019 to 31 December 2019.Results: As many as 90% of respondents rated informed consent made at ZAH as good quality and 85% expressed the satisfaction with health services at ZAH. An ethical and medicolegal review of informed consent emphasizes the basic principles of bioethics. As many as 84% of respondents stated that their autonomy rights had been fulfilled, 92.50% thought that the doctor had done beneficence, nonmaleficence, and justice well in the process of informed consent and 92.33% of respondents rated the three elements of informed consent as being well implemented. Based on the results of the chi-square statistical test showed a significant relationship between the quality of informed consent in terms of ethics and medicolegal with patient health service satisfaction under the value of p = 0.001 (p≤0.05).Conclusion: Good quality of informed consent in terms of ethics and medicolegal has an important role in increasing patient satisfaction with health services at the dr. Zainoel Abidin Hospital of Banda Aceh.

Abstract

Background: Interprofessional collaboration for patient care is integrated cooperation between health workers in ensuring optimally consecutive patient care. Interprofessional decisions should be made in two ways between professional caregivers.Aims: This study aimed to analyze communication between professional caregivers in establishing interprofessional collaboration at Meuraxa District General Hospital in Banda Aceh.Method: This study was qualitative research and used a phenomenological approach. In-depth interviews and FGD were conducted with professional caregivers consisting of doctors in charge, nurses, pharmacists, and dietitians. All of the in-depth interviews and FGD were recorded and transcribed. Data obtained were analyzed thematically.Results: Communication between professional caregivers in establishing interprofessional collaboration at the hospital had not been well-performed. Overall, they had not optimally utilized integrated patient progress records as they focused on taking care of their patients individually. They also lacked literacy about the information in the records. Inadequate time in serving patients and the insufficient number of human resources were presumed to hamper interprofessional communication.Conclusion: Promoting interprofessional collaboration and effective communication regularly is required by the hospital management. They also need to evaluate the sufficiency of human resources, especially pharmacist positions. Besides, they can formulate supervision and evaluation systems of interprofessional communication between professional caregivers. Keywords : communication, integrated patient progress record, interprofessional collaboration, professional caregivers

Abstract

Background: Hospital is a health care institution that provides public services for patients. It needs computerized information technology, for example, a hospital information management system.Aims: This study was aimed at analyzing the implementation of hospital management information system using the human organization fit model at the Outpatient Department of Sabang General Hospital.Methods: This study was conducted from March-June 2020 using a cross sectional design. The study’s population was the hospital managers, administration staff and health workers at the hospital. The study’s sample size was 106 respondents selected using a purposive sampling. This study used a quantitative design in which the data analysis was done using a spearman correlation test with P-value of =<0.05. The independent variables were the quality of system, quality of information, and quality of care. While the dependent variables included system user, user satisfaction and organization. The data were collected through questionnaire distributed to the respondents.Results: The quality of system did not have a relationship with the system user (P=0.585), user satisfaction (P=0.541), and organization (P=0.256). The quality of information had a relationship with the  system user(P=0.004) and user satisfaction (P=0.000), but it did not have a relationship with organization (P=0.132). The quality of care had a relationship with the system user(P=0.000), user satisfaction (P=0.000) and organization (P=0.000).Conclusion: The Sabang General Hospital, Aceh, needs to improve the hospital management information system with the HOT-fit model, especially the quality of system. They can conduct training for users and provide applicable and attractive manuals.Keywords: hospital, information management system, quality of care, quality of information, quality of system.

Abstract

Background: Basic medical sciences are essential foundation for the constructive learning of clinical sciences. Faculty of Medicine, Universitas Syiah Kuala is currently implementing Problem-based learning (PBL) for basic medical sciences. Studies have been conducted in many places with inconsistently different results that it is impossible to draw theoretical and practical conclusions regarding the implementation of PBL for basic medical sciences. This study aimed to evaluate the implementation of PBL for basic medical sciences at Faculty of Medicine, Universitas Syiah Kuala from the perspectives of pre-clinical teachers, clinical teachers and medical students.Methods: This is a qualitative study with a phenomenological approach. Respondents were pre-clinical teachers, clinical teachers and medical students. Data collection were carried out by using focused-group discussion (FGD). Data analysis was carried out qualitatively by using thematic analysis.Results: This study identified three themes: (1) integrated thematic blocks are not adequate for the learning of basic medical sciences, (2) inadequacy of basic medical sciences practice activities, and (3) possible improvements through curriculum revision.Conclusion: The implementation of PBL in the form of integrated blocks for basic medical sciences is not adequately constructive to prepare students to learn clinical sciences. Hybrid curriculum which incorporates conventional methods for basic medical sciences and PBL for clinical sciences is recommended to improve students’ constructive learning. Further studies on hybrid curriculum is needed to evaluate its effect of basic medical sciences. Keywords: Basic medical sciences, Problem-based learning (PBL), practical session, hybrid curriculum, constructive learning, self-directed learning.

Abstract

Background: The importance of the interprofessional education (IPE) program has been depicted through various forms of the IPE curriculum. The COVID-19 pandemic has forced a change in the implementation of IPE; this has caused the method of implementing IPE to change online. This study aims to assess the readiness of three different healthcare professions for implementing IPE using the Readiness for Interprofessional Learning Scale (RIPLS).Methods: The RIPLS survey was completed by 108 medical students, 40 pharmacy students, and 30 nursing students at Universitas Syiah Kuala, Indonesia. The survey was done after the students carried out the online IPE intervention, which was held during the COVID-19 pandemic. They also completed open-ended questions reflecting their attitude towards and experience from the online IPE implementation.Results: There was no significant difference regarding student readiness for interprofessional learning among the three academic disciplines. Generally, as many as 57.9% of students showed a positive perception of IPE. Separate analysis for each study program showed that all of them were in the high range of scores for positive perception. Pharmacy students have the highest positive perception of IPE (60%), while medical and nursing students’ scores were 54.6% and 53.3%, respectively. Qualitative interviews revealed that: 1) the scheduling of IPE implementation was not suitable for the students, 2) the online communication between professions was not as effective as expected challenging, and 3) there was a growing awareness to respect other professions.Conclusion: It can be concluded that conducting the IPE program during the COVID-19 pandemic experienced many obstacles, especially communication. However, it still maintains the main objective of IPE, which is to respect other professions.

Abstract

In 2019, the coronavirus disease 2019 (COVID-19) pandemic, caused by severe acute respiratory syndrome corona virus 2 (SARS-CoV-2), has killed more than 6.6 millions of people around the world as of end of 2022. The long-term impact of COVID-19 is persisted, including its impact on male reproduction. SARS-CoV-2 enters into host cells using the angiotensin-converting enzyme 2 (ACE2) and transmembrane serine protease 2 (TMPRSS2) receptors. Both of these receptors are expressed more in men, and therefore men are more susceptible to SARS-CoV-2. COVID-19 potentially cause infertility by damaging testicular tissues and interfering with the process of spermatogenesis. A decrease in serum levels of testosterone, follicle-stimulating hormone (FSH) and luteinizing hormone (LH) as well as a decrease in sperm quality in men with COVID-19 compared to healthy men of the same age has been reported in several studies. Utilizing existing research data, this study aims to explore in detail of how SARS-CoV-2 tends to affect male fertility.
