Abstract

PT. Evercoss Technology Indonesia, is a technology-based company that requires valid data to carry out safe and comfortable business activities. Based on the results of interviews with the company that there have been several work accidents while the company is in the new industrial area. By designing documents using HIRARC, it is hoped that it will help PT Evercoss Technology Indonesia in identifying various potentially hazardous work activities, by conducting a risk assessment of potentially hazardous work activities, to providing suggestions for risk control of potential hazards. The results obtained from making the HIRARC document show that there are several work activities that have the potential to be hazardous, namely 12 work activities that have very low potential hazards, 23 work activities that have low potential, 21 work activities that have moderate potential hazards and 2 work activities that have potential hazards. tall. The last stage is to control risk in several ways, namely elimination, substitution, engineering/technical design, administration, and Personal Protective Equipment (PPE). The proposed risk control is expected to reduce the level of risk that occurs at PT Evercoss Technology Indonesia.

Abstract

PT. X is a company engaged in the manufacturing industry that produces various kinds of wooden furniture products such as dining tables, chairs, cabinets, etc. PT. X was established in 2012 and during PT. X is running there are several accidents at work. This final project was carried out using the method of observation and interviews with field managers and workers. The purpose of this final project is to identify hazards, assess risks, and carry out risks to the company. After doing the analysis, in the production area of 16 sub-activities there are low risk rating categories with a total of 12 categories, a medium risk rating category with 21 categories, and a high risk rating category with a total of 7 categories. As for the warehouse area, there is a low risk rating category with a total of 2 categories and a medium risk rating category with a total of 4. It has been proposed 20 administrative engineering controls, 25 controls using personal protective equipment, and 1 control by technical design. If risk control is carried out, it is estimated that the risk rating will change to 0 extreme cases, 0 high cases, 12 moderate cases, and 34 low cases.

Abstract

The company needs an update on the HIRARC document because after conducting the audit, the company needs an update. The method to be used is Hazard Identification, Risk Assessment, Risk  Control (HIRARC) for further identification, assessment, and control. HIRARC is a method used to find out what are the potential hazards and risks that can occur in an activity or activity, after knowing the potential hazards and risks, a risk assessment will be carried out to determine the level at which the potential hazards and risks are located. Risk control is a way to reduce the value of risk by means of elimination, substitution, mechanical engineering and administration, and the use of PPE in accordance with the risk control hierarchy. In the stainless production area, there are 9 machines used for the production process with the initial assessment results of 38 sub-activities in the low risk category, 18 sub-activities in the moderate risk category, 9 sub-activities in the high risk category, and 1 sub-activity in the extreme risk category. The estimated value after controlling becomes 60 sub-activities in the low risk category, 3 sub-activities in the moderate risk category, and 3 sub-activities in the high risk category.

Abstract

PT. X is a manufacturing company engaged in printing flexible packaging since 2017. PT. X produces plastic packaging as finished product packaging, one example of which is making packaging for food & beverages products. PT. X does not yet have an Occupational Health and Safety Management System (SMK3). The first step in realizing SMK3 at PT. X is designing a hazard identification, risk assessment, and risk control (HIRARC) document. HIRARC documents identify risks, provide risk assessments, and determine risk controls. HIRARC document design using observation and interview methods. The results of the HIRARC document show that there are 15 potential hazards, with 2 cases in the extreme category, 4 cases in the high category, 8 cases in the moderate category, and 1 in the low category. The proposed risk controls are 25 controls consisting of technical design (9 cases), administrative (11 cases), and PPE (5 cases). The design of the HIRARC document is expected to reduce the risk of hazards in the warehouse and office area. The estimated risk rating after risk control is carried out is 3 cases in the high category, 7 cases in the moderate category, and 5 cases in the low category.

Abstract

This thesis aims to create a design of occupational safety and health management system in PT. Ridwan Jaya Agung that in accordance with the Government Regulation Number 50 of 2012. The design of the system consists of several stages which are initial audit, interviews related to the company's capabilities, analysis of the company's initial gap, policy determination, design of K3 implementation, design of K3 monitoring, introduction of K3, and new audits. In the initial audit, the company could only fulfilled 1 out of 122 criteria; equivalent to 1%, which according to the Government Regulation is not a good or ideal auditing results. Therefore, through this thesis, the researcher had created a design of occupational safety and health management system that meets 119 criteria; equivalent to 98%, and can be said to have fulfilled the implementation of occupational safety and health management system.

Abstract

CV. Santoso Jaya is a company engaged in logistics. The company never identified any hazards before, work accidents that often occur in the company are workers hit by goods when they are about to load goods, goods fall, and lifting positions are not ergonomic. Work accidents such as falling goods very often occur, within 1 month there can be more than 1 incident. Therefore, the purpose of making this final project is risk identification, risk assessment, and risk control (HIRARC) in CV. Santoso Jaya to minimize and eliminate the risks. The risk identification process is carried out on loading and unloading activities in the company's warehouse. Collecting data by observing and interviewing workers. After conducting observations and interviews, the authors then carried out risk identification, risk assessment, and risks control. The results from this research activity are that of the 13 sub-activities in central warehouse and branch warehouses, there are 2 sub-activities with a moderate risk rating, 6 with a significant risk rating, and 5 with a high risk rating. From the potential risks that have been identified, the researcher proposes risk control in the form of 1 type of elimination, 5 substitution, 5 design, 6 administration, and 6 PPE.

Abstract

The purpose of this Final Task is to design OHSMS based on Government Regulation number 50 of 2012 which is still within the company’s capabilities. OHSMS design method consists of several stages, namely initial observation, design of initial audit materials, interviews with the company to know capability limits, initial audit, initial recapitulation, OHSMS policy setting, designing OHSMS and design validation. The initial audit results showed the fullfilment of OHSMS criterias by the company was 0 out of 122 criterias set (0%). With the new OHSMS design, the company must meet a minimum of 74 out of 122 criterias set (60%).If it fails, then the design must be reworked. The final audit results showed the company has potential to meet 119 out of 122 criterias set (97.6%). With that being said, the company has fulfilled the requierement needed.

Abstract

PT. Cahaya Citra Alumindo is a manufacturing company that produce alloy wheels for car and golf cart. Based on interview, company work’s accident are 3-4 minor accidents every month and 1-2 major accidents every year. The goal of HIRARC is to help the company to detect hazard in work’s activity, risk assessment and documenting risk control for worker’s safety and health. HIRARC is based on ISO 45001:2018 and goverment regulations No. 5/2012 about Management System Occupational Health and Safety (SMK3) as the early stage to implement SMK3. The results of HIRARC shows that there are still many activities that endangers worker. There are 30% activities classified as moderate risk, 17% as significant risk and 18% as high risk. Continued by risk control which goal is to minimize risk by elimination, substitution, engineering controlm administrative control and PPE. The estimation of decreased risk rating posibility if risk controls implemented are 15% as moderate risk, 2% as significant risk and 0% as high risk.

Abstract

PT Pelabuhan Indonesia IV Makassar new port branch has a high potential danger. There will be 16 work accidents reported in 2020. The company has never identified a hazard in all of its business activities. Therefore the objective of the final task is to identify hazards, assess risks, control risk (HIRARC) in the company's main activities, namely loading and unloading activities. The initial stage of this final task is to conduct observations and interviews of workers directly in the company. After observation and interview, the study will identify hazards, assess risks, and control risks against risks that have high and extreme values. Hirarc's review on loading and unloading is expected to prevent similar accidents from happening again and reduce the impact of accidents. The results obtained after observation there were 10 activities with extreme category risk rating, 3 sub-activities with high category risk rating, and 9 sub-activities with medium category risk rating. After HIRARC the results obtained are 16 sub-activity risk rating medium category, 6 sub activity risk rating low category.

Abstract

PT. X is a company engaged in manufacturing, namely as a producer of expanded metal and hollow. So far, the company has not implemented K3 properly in its company. Therefore, the purpose of this final project is to identify hazard risks, assess the level of risk, and provide risk control proposals (HIRARC) in the company's production process. In the initial stage of this final project, the authors will conduct direct observations in the field and interviews with field supervisors and workers. After conducting interviews and observations, the authors will identify hazards, assess the risks of hazards, and provide suggestions for controlling the risks of hazards. The results obtained from the analysis are that there are 24 potential hazards that have a significant risk rating category, 8 moderate risk ratings, and 4 low risk rating. The risk controls provided are 4 technical engineering, 43 administrative steps, and 5 PPE. After the risk control is implemented, it is estimated that the risk rating will change to a lower level.
