Abstract

Pelaksanaan Jaminan Kesehatan Nasional (JKN) Indonesia dengan skema pembayaran kapitasi pada pelayanan kesehatan tingkat pertama merupakan sistem baru bagi dokter gigi. Evaluasi model pembiayaan kapitasi perlu dilakukanuntuk mengetahui apakah dokter gigi puas terhadap sistem pembiayaan kapitasi yang telah ditetapkan pemerintah. Jenis penelitian adalah observasi deskriptif. Subjek penelitian ini adalah dokter gigi Puskesmas Kabupaten Sleman Yogyakarta. Instrumen pengukuran berupa kuesioner persepsi dan harapan terkait pelaksanaan Jaminan Kesehatan Nasional di Puskesmas dengan skala biner. Secara umum sebagian besar dokter gigi masih melihat pembiayaan secara kapitasi dalam program JKN membawa potensi konflik yang besar. Walau demikian, para dokter gigi menyadari bahwa sistem ini menekan biaya kesehatan masyarakat. Secara mutlak seluruh dokter gigi mengharapkan adanya perhitungan ulang risiko dan meningkatnya peran organisasi profesi untuk negosiasi kapitasi dengan pihak Badan Penyelenggara Jaminan Sosial. Penelitian ini menunjukkan bahwa para dokter gigi sudah dapat menerima sistem kapitasi dengan baik tetapi tidak puas dengan pelaksanaannya.

Abstract

The quality of communication and patient satisfaction in dental radiography. The quality of communication is one of important components in health service. The aim of this study is determine the relationship between the quality of communication and the level of patients in dental radiography service at Prof. Soedomo Dental Hospital Faculty of Dentistry Universitas Gadjah Mada. This study was conducted on 100 selected respondents. The quality of communication questionnaire consisted of 19 questions involving 4 aspects of communication assessment (introduction, explanation, careful listening, and empathy) and patient satisfaction questionnaires with 20 questions involving 4 aspects of satisfaction assessment (open-endedness, empathy, abilities, and general satisfaction). The score results of the questionnaire were measured by a Likert scale and the category of the assessment was based on criterion- referenced interpretation. Spearman correlation test showed that a signicant relationship (p <0.05) between the quality of communication and the level of patient satisfaction with the positive direction of correlation and the strong correlation (r = 0.856). Patient assessment for the quality of radiographer communication mostly showed good category (64%) with the most inuential communications aspect found in empathy (r = 0.842) and introduction (r = 0.752). The level of satisfaction assessment by the majority of patients was satised (71%) with the most inuential satisfaction aspect found in empathy (r = 0.807) and general satisfaction (r = 0.706). The better communication used by the radiographer could lead to the higher the level of patient satisfaction in dental radiography service.ABSTRAKKualitas komunikasi merupakan salah satu komponen yang penting pada layanan kesehatan karena berhubungan dengan kepuasan pasien. Penelitian ini bertujuan untuk mengetahui hubungan antara kualitas komunikasi pada pelayanan radiogra kedokteran gigi dengan tingkat kepuasan pasien Rumah Sakit Gigi dan Mulut Prof. Soedomo Universitas Gadjah Mada. Penelitian ini dilakukan pada 100 responden terpilih. Kuesioner pada penelitian terdiri dari kuesioner kualitas komunikasi yang terdiri dari 19 butir pertanyaan yang mencakup 4 aspek penilaian komunikasi (introduce, explanation, listen carefully, dan empathy) dan kuesioner tingkat kepuasan pasien yang terdiri dari 20 butir pertanyaan yang mencakup 4 aspek penilaian kepuasan (open-endedness, empathy, abilities, dan general satisfaction). Skor hasil kuesioner penelitian diukur menggunakan skala likert dan penetapan kategori penilaian diukur menggunakan penilaian acuan patokan. Hasil uji korelasi Spearman menunjukkan hubungan yang bermakna (p<0,05) antara kualitas komunikasi dengan tingkat kepuasan pasien, dengan arah korelasi positif dan kekuatan korelasi sangat kuat (r = 0,856). Penilaian pasien terhadap kualitas komunikasi radiografer sebagian besar menunjukkan kategori baik (64%). Aspek komunikasi yang paling berpengaruh adalah empathy (r = 0,842) dan introduce (r = 0,752). Sebagian besar pasien memberikan penilaian kepuasan pada tingkat puas (71%), dengan aspek penilaian kepuasan yang paling berpengaruh adalah (r = 0,807) dan general satisfaction (r = 0,706). Semakin baik komunikasi yang digunakan oleh radiografer maka akan semakin tinggi pula tingkat kepuasan pasien terhadap pelayanan radiogra kedokteran gigi Rumah Sakit Gigi dan Mulut Prof. Soedomo Universitas Gadjah Mada.

Abstract

Background: Adolescents from Islamic boarding school adopted simpler life than those not from Islamic boarding school. It can be a trigger of knowledge, behavioural, and oral diseases differences. Neglecting oral diseases will affect the quality daily lives. The aim of this study is to know the differences of oral health related knowledge, behaviour, and quality of life between adolescents from Pesantren and non Pesantren. Method: This study was observational with a cross-sectional design. The study was conducted on 242 adolescents at Madrasah Aliyah Pesantren and non Pesantren in Banjarmasin City, South Kalimantan. The data of research variables were obtained through online questionnaires. Result: Statistical analysis indicated there were no significant differences of oral health knowledge (p=0,727>0,05) and quality of life (p=0,711>0,05), but there was significant difference of oral health behavior (p=0,038<0,05) between adolescents from Pesantren and non Pesantren. Adolescents from Pesantren have lower oral health behaviour when compared to adolescents from non Pesantren.Conclusion: The oral health knowledge and quality of life possessed by adolescents from Pesantren were not different from those not from Pesantren. Oral health behavior of adolescents from Pesantren was different than adolescents from non Pesantren. It still needs to be improved.

Abstract

A good bleaching process need to be seen from its time distance, the longer time process to done the better result will release. The aim of this research is to know the influence of apple extract in different time for bleaching process (tooth whitening), that used as an alternative materials for tooth whitening. Design of this research is laboratory experimental, by using 15 tooth specimen and Rome Beauty apple extract of 5 0%, 100% and aqudes as negative control. Three groups have given diflerent continue treatment, they are groups of soaked 24 hours, 48 hours and 72 hours with concentration of extract 50% and 100%. Afier soaked, the changing colour value would be observed by using spectrophotometer:Research result show that value degree of change of colour before and after soaked with apple extract 50% and 100% with group of soaked 24 hours, 48 hours and 72 hours. Based on paired T-Test result showed that there was changing of tooth colour that turn into whiten Based on Friedman Analysis result show that P=0.324 (p0,05), which means that there was not significant influence time in tooth whitening changing with apple extract in tooth whitening process with time 24 hours, 48 hours and 72 hours with extract concentration 50% and 100%. Several factor influence the result, such as too short time distanceso that the calculation of statistic did not show any significant changing the sample is fewer so that there was not influence in calculation of statistic.

Abstract

Background: A high prevalence of caries at ages ≥ 12 in Yogyakarta province (DMFT = 6.5), raises the question of the effectiveness of the school-based dental program (SBDP) which, as a national oral health program in schools, is organized by community health centers (CHCs). Purpose: The aim of this study is to explore the possible constraints on work processes which might affect the performance of SBDPs in controlling caries. Methods: In-depth interviews was conducted in twelve CHCs, covering all five districts both in urban and rural areas. Subjects were 41 dentists and dental nurses working in these CHCs. The interviews were structured according to the following themes: resources and logistics; program planning; target achievement; monitoring and evaluation; and suggestions for possible improvements. The data were analyzed using content analysis. Results: The main constraints identified were limited resources and inflexible regulations for resource allocation in the CHC, and inadequate program planning and program evaluation. Inadequate participation of parents was also identified. Another constraint is thatpolicy at the district level orientates oral health towards curative intervention rather than prevention. Suggestions from interviewees include encouraging a policy for oral health, task delegation, a funding program using school health insurance, and a reorientation towards prevention. Conclusion: The weakness of management processes and the unsupported policy of the SBDP at the local level result in a lack of effectiveness. The constraints identified and suggestions for improvements could constitute a basis for improving program quality.Latar Belakang: Tingginya prevalensi karies pada usia ≥ 12 tahun (DMFT = 6.5) di Provinsi Daerah Istimewa Yogyakarta (DIY) menimbulkan pertanyaan akan efektifitas Usaha Kesehatan Gigi Sekolah (UKGS). UKGS adalah salah satu program nasional di bidang kesehatan gigi dan mulut yang dilaksanakan oleh Puskesmas. Tujuan: Tujuan dari penelitian ini adalah untuk mengeksplorasi hambatan pada pelaksanaan UKGS yang dapat mempengaruhi kinerja dari UKGS untuk mengontrol karies. Metode: Penelitian kualitatif dengan menggunakan in-depth interview dilakukan di 12 Puskesmas, meliputi lima kabupaten di DIY baik di daerah pedesaan maupun perkotaan. Subyek terdiri dari 41 dokter gigi dan perawat gigi di Puskesmas. Struktur tema dari wawancara adalah sumber daya dan logistik; perencanaan program; pencapaian target; monitoring dan evaluasi; saran untuk perbaikan program. Data dianalisis menggunakan analisis konten. Hasil: Hambatan pada program UKGS yang teridentifikasi adalah sumber daya yang terbatas dan regulasi yang tidak fleksibel untuk alokasi sember daya di Puskesmas, perencanaan dan evaluasi program yang tidak memadai dan rendahnya peran serta orang tua. Hambatan lain adalah kebijakan pada tingkat kabupaten yang berorientasi pada pelayanan kuratif daripada preventif. Saran yang dikemukakan adalah penguatan kebijakan untuk kesehatan gigi, delegasi tugas, asuransi kesehatan sekolah dan reorientasi pelayanan preventif. Kesimpulan: Proses manajemen yang lemah dan kurangnya dukungan kebijakan untuk UKGS pada tingkat daerah menjadi salah satu sebab kurang efektifnya program. Hambatan dan saran yang teridentifikasi pada studi ini dapat menjadi dasar untuk meningkatkan mutu program.

Abstract

Background: In general, children with physical disabilities have a lower level of oral hygiene compared to able-bodied children because their access to dental health care services is affected by their physical limitations. The level of oral hygiene available to children with disabilities can be improved with the involvement of parents/caregivers equipped with good knowledge and attitudes regarding oral health. Purpose: Determine the difference between the effectiveness of school and home visit-based dental health promotion in improving the knowledge and attitudes of parents/caregivers and students regarding oral hygiene as well as lowering the dental plaque scores of students with mild physical disabilities. Methods: This study is quasi-experimental and uses the two-group pretest–posttest design. The study’s design was created with a model of one observation before intervention (O1), two interventions (X1-2) and two observations after intervention (O2-3). The study samples are students with mild physical disabilities aged 8–15 years old in special needs school, or sekolah luar biasa (SLB), in Sleman, Yogyakarta, and their parents/caregivers. The locations of the study were SLBs and the respondents’ homes. The study instrument was a knowledge test for students with mild physical disabilities and their parents/caregivers, along with a questionnaire on the parents/caregivers’ attitudes towards oral and dental health. Plaque control examinations for students with mild physical disabilities were conducted using the O’Leary index. Results: School-based oral health promotion was better at improving the attitudes of parents/caregivers to oral hygiene than the home visits (p<0.05). Both school-based and home visit-based oral health promotion was effective in furthering the oral hygiene knowledge of students with mild disabilities and their parents/caregivers as well as in improving the students’ dental plaque scores (p>0.05). Conclusion: The school based-oral health promotion model was more effective in improving attitudes of the students’ parents/caregivers. Both the school-based and the home visit-based oral health promotion models were equally effective in enhancing the knowledge of parents/caregivers, along with the knowledge and dental plaque scores of students with mild disabilities.

Abstract

The evidence of relation between nutrition and oral health has addressed nutrition issues on a new urgency indental practices as a part of efforts in developing quality of care. Nutrition is a modifiable factors and could beadviced to patients that ensure progression and maintainance of oral health status. Dietary advice to patientsshould be positive and personalized if possible and can be in line with general health. This paper is intended toprovide an overview of management of dietary advice for patients with dental caries, periodontal disease and oralsurgery which could be implemented in dental practice setting. The appropriate and specified nutrition advice canhelp patient recover from dental disease and achieved the best result of dental treatment and prevention.

Abstract

Background: Tooth loss leads to decreased masticatory function, which affects nutritional intake. Tooth loss accompanied by poor nutritional status affects quality of life both physically and psychosocially. Purpose: This study was conducted to identify the relationship between number of teeth lost due to nutritional status and quality of life. Methods: A study conducted at Gulai Bancah Primary Health Care in Bukittinggi. Participants were recruited using a non-probability purposive sampling technique. 178 pre-elderlies aged between 45 and 59 years took part in this study. Tooth loss was reported in the dental examination format while nutritional status and quality of life pertaining to dental and oral health were determined using the Mini Nutritional Assessment (MNA) and the Geriatric Oral Health Assessment Index (GOHAI). Pearson correlation and path analysis were utilized to analyze the data. Results: The Pearson correlation test indicated a moderate correlation between tooth loss and nutritional status (r = -0.549) and a strong correlation between the number tooth lost and quality of life (r = -0.742). Pathway analysis confirmed that the direct effect value (beta coefficient = 0.552) was greater than the indirect effect value (beta coefficient = 0.189). The findings indicated that, indirectly, the amount of tooth loss mediated with nutritional status had no significant relationship to quality of life. Conclusion: An increase in tooth loss leads to pre-elderly poor nutritional status and quality of life. However, nutritional status is not an intermediate factor between the number of teeth lost and pre-elderly quality of life.
